Question: I am a 30 year old runner who started running in my early 20's. Either due to genetics or improper footwear for the past 10 years, I have noticed that my left bunion gets worse every year. As the bunion is getting more pronounced, I have noticed that I will have dull pain at the end of the day or at the end of a run in my second toe. Is there something I should be doing to help relieve the bunion/second toe pain? Thank you.

Question: A 56 y/o female presents with post surgical pressure under distal phalanx of 1st and 2nd toes as a result of fusion of 2nd toe and bunion surgery of right foot. Her left foot still has bunion and overlapping 2nd toe and pronates during propulsive phase of gait. She is very active and wants to go back to dancing, running and aerobics. The Rx calls for semi-rigid orthoses, medial arch support. We are making a full length device with cut outs under 1st and 2nd toes on right. The left will include medial posting and met pad. Rx did not address her footwear needs. She is currently wearing roomy shoes with double insoles in each to relieve her pain in her forefeet. The prescriber is open to our ideas. What are some additional ideas to help this patient?

Response: You could recommend a mild rocker for the fusion. Disperse pressure to her other met heads and arch when you create the off load. Check to see if the toes are aligned, because you can cause more pressure on the fusion site with the off load. Every joint that is fused effects the next, including the ankle and knee during gait. Response: In reply to Mr. Schwartz, I have a diabetic patient with the same issues. She has a severe enough deformity and we put her in custom molded shoes. Because of past surgeries her 2nd and 3rd digits, bilateral, have calluses under the 1st IP joint. The orthotics made with her shoes are deep enough to create pockets on the bottom to add poron for cushion. She comes in monthly for re-checks and because of this she has kept her feet. She understands the importance of a good Pedorthist! Not to pat myself on the back, but we do help keep the feet attached to the legs! We know all the special little tricks to help relieve their pain.

Question: In regards to the salvage claim, does anyone know what modifier(s) need to be used? Thank you!

Response: As long as the item you are billing to Medicare is a covered service you must bill the item out as you normally would using your modifiers KX (required documentation on file), LT and/or RT (left or right side or both if the items involved both feet) and under the narrative note section of your claim lines you must add the words SALVAGE CLAIM to each custom item that was refused after it was made (the refusal date would be your date of service) or if you were informed that the patient has passed the patient's date of death would be your date of service under the narrative note section of your claim.

Again, I cannot stress the importance of a well documented file.  Make sure you have documented each attempt to deliver the item (schedules appointments, no shows for those appointments, etc...) and when you learned of the patient's refusal to take delivery or when you learned of the patient's passing and the patient's date of death.

HU research maps warnings for Type 2 diabetes

Hebrew University researchers have found the first proof of molecular risk factors for Type 2 diabetes that indicate susceptibility to the disease. This could provide an "early warning" sign and eventually lead to new treatment approaches, the researchers said, as well as mapping susceptibility at an early stage to other diseases as well. Type 2 diabetes, which has been diagnosed in 500,000 Israelis, and not yet diagnosed in an equal number at a preliminary stage called pre-diabetes - generally results from being overweight and a lack of exercise. Exercise doesn't prevent pregnancy-related diabetes

Researchers in Norway found that when they randomly assigned 855 pregnant women to either exercise three times a week or to stick with regular prenatal care alone, the exercisers were no less likely to develop gestational diabetes. By the third trimester, seven percent of the exercise group had been diagnosed with gestational diabetes, versus six percent of the comparison group. The findings were surprising, according to lead researcher Signe N. Stafne of the Norwegian University of Science and Technology in Trondheim. Full Text

According to an annual survey conducted by the American College of Sports Medicine, Educated, certified and experienced fitness professionals; strength training; and fitness programs for older adults were the top three fitness trends predicted for 2012. Other activities making the top-20 list included Zumba and other dance workouts, yoga, boot camp, outdoor activities and spinning. Running worked wonders for grieving mom

To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, interactive CD's and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at Eneslow Pedorthic Institute for 2 weeks from August 6th to August 19th, 2012. This class completes required education prerequisite to sit for national certification exam. It does not satisfy the requirement of 1000 documented hours of field experience.
