PREVIOUS QUESTIONS WITHOUT RESPONSES

Question: We've been using Riecken's PQ for years, but with the changes to the original formula that have been made, our lab techs are finding it to be increasingly more difficult to use and predict the firmess/cushion of the finished product.  I've tried QuikSil and Viscolas in the past, but don't like them because you can't glue to them.  I also tried Acton, but it is difficult to keep in place.  Does anyone have any other suggestions for alternative viscoelastic polymers to be used in foot orthoses? Thanks.

- Erick J. Janisse, C.O., C.Ped.

Question: A 56 y/o female presents with post surgical pressure under distal phalanx of 1st and 2nd toes as a result of fusion of 2nd toe and bunion surgery of right foot. Her left foot still has bunion and overlapping 2nd toe and pronates during propulsive phase of gait. She is very active and wants to go back to dancing, running and aerobics. The Rx calls for semi-rigid orthoses, medial arch support. We are making a full length device with cut outs under 1st and 2nd toes on right. The left will include medial posting and met pad. Rx did not address her footwear needs. She is currently wearing roomy shoes with double insoles in each to relieve her pain in her forefeet. The prescriber is open to our ideas. What are some additional ideas to help this patient?

Sears Holdings on Tuesday reported a sharp drop in holiday sales compared to a year ago, and said the results will force it to close 100 to 120 Sears and Kmart stores. The stores to be closed have yet to be identified. The company is one of the nation's largest retailers with more than 4,000 stores in the United States and Canada. Sears Holdings said sales at stores open at least a year, known as same-store sales, tumbled 5.2% in the eight weeks leading to Christmas Day. That came from a 4.4% drop in sales at Kmart stores and a 6% slide in sales at domestic Sears stores. Full Text

A California footwear manufacturer has found a way to create jobs during a stagnant economy. The Ugg boot line, which originated in Australia in the 1970s, has grown more than 5,000 percent since being purchased by a U.S. corporation. Ugg's founder, Australian surfer Shane Steadman, sold his company to Deckers Outdoor Corp. of California in 1995. Medicine is being fundamentally transformed. Greater government intervention as dictated by the Patient Protection and Affordable Care Act and other federal changes will forever change how the physician, hospital and patient interact. The future for government-funded healthcare programs is to bundle hospital-physician payments. This may take the form of an accountable care organization (ACO), gain sharing or co-management agreements. Full Text

FDA wants another dermaPACE study from Sanuwave

The Food & Drug Administration sends a "major deficiency letter" to Sanuwave on its pre-market approval bid for the dermaPACE diabetic foot ulcer treatment, saying it wants the company to run another clinical trial on the extra-corporeal shock wave technology.Designed to show whether the extra-corporeal shock wave technology could deliver 100% wound closure after 12 weeks, the data showed no statistically significant difference between sham treatment and actual treatment with the device. Survey: Bidding program really does limit access

By: Theresa Flaherty

Beneficiaries in Round 1 competitive bid areas have limited access to the most widely used diabetes testing supplies, says the American Association of Diabetes Educators (AADE). A survey by the association found that mail order contract suppliers, on average, offered only 38% of the product brands that are listed on www.medicare.gov. Of the nine brands identified by the Office of Inspector General in a 2010 report as the top mail order brands by market share, contractor suppliers offered, on average, only 1.44 of the brands, or 16%. "We were getting all of these stories from our educators who were hearing about problems, or having their patients tell them they don't have a certain meter any more," said Martha Rinker, chief advocacy officer for the AADE. "When we talked to CMS about it, or any other party, they'd say it was just anecdotal. We thought this was the best way to get concrete information." Full Text

Holiday travel tips for people with diabetes

If you have diabetes and plan on traveling this holiday season, there are certain things you need to do to safeguard your health, says a University of Alabama at Birmingham expert."I tell patients, especially type 1 diabetics, to be diligent about blood sugar control when they travel -- especially across time zones -- because it's easy to lose control of your glucose management when you get out of your usual routine," Dr. Fernando Ovalle, director of the UAB Multidisciplinary Diabetes Clinic and a senior scientist in the UAB Comprehensive Diabetes Center, said in a university news release. Reporting from Joint Base Lewis-McChord, Wash.- Brig. Gen. Rhonda Cornum found out what combat stress was in the back of a pickup during the first Gulf War in 1991 when one of her Iraqi captors unzipped her flight suit and, as she lay there with two broken arms and an injured eye, sexually assaulted her. As the Army struggles to deal with an increase in suicides and combat-related stress stemming from 10 years of war in Afghanistan and Iraq, Cornum has been deployed to lead the military's new mental fitness program. BECOMING CERTIFIED IN PEDORTHICScped
Pedorthics is a professional field covering the design, manufacture, modification and/or fit of footwear. Pedorthics encompasses shoes, orthoses and foot devices that prevent or alleviate foot problems caused by disease, overuse, injury or congenital defect. Certified Pedorthists work with patients and their footwear to conform to a doctor's footwear prescription as part of the patient's treatment

To earn certification in Pedorthics, you must take a pre-determined number of course hours in pedorthics and pass a certifying examination overseen by either The American Board for Certification in Orthotics, Prosthetics & Pedorthics® or Board for Certification/Accreditation, Int'l.  Both the ABC and BOC are independent certification bodies that set standards for the profession and offer certifications that are recognized by the National Commission for Certifying Agencies.
