NEW QUESTIONS

 

 

 

Question: We've been using Riecken's PQ for years, but with the changes to the original formula that have been made, our lab techs are finding it to be increasingly more difficult to use and predict the firmess/cushion of the finished product.  I've tried QuikSil and Viscolas in the past, but don't like them because you can't glue to them.  I also tried Acton, but it is difficult to keep in place.  Does anyone have any other suggestions for alternative viscoelastic polymers to be used in foot orthoses? Thanks.

Question: A 56 y/o female presents with post surgical pressure under distal phalanx of 1st and 2nd toes as a result of fusion of 2nd toe and bunion surgery of right foot. Her left foot still has bunion and overlapping 2nd toe and pronates during propulsive phase of gait. She is very active and wants to go back to dancing, running and aerobics. The Rx calls for semi-rigid orthoses, medial arch support. We are making a full length device with cut outs under 1st and 2nd toes on right. The left will include medial posting and met pad. Rx did not address her footwear needs. She is currently wearing roomy shoes with double insoles in each to relieve her pain in her forefeet. The prescriber is open to our ideas. What are some additional ideas to help this patient?

Question: At this time of year, we usually give a token of thanks to our prescriber's for the holiday season. Some years we give a gift food basket. In the past we gave shoe trees and other practical gifts. We are looking for new ideas that have proven to be moderately priced and memorable.

Response: Regarding gifting as a show of appreciation: a donation to a charity such as Souls4Soles, American Diabetes Association, Arthritis Foundation or a local cause in the names of the physician or store can really make an impact when considering the amount spent.

Response: In response to Robert Schwartz's question as to holiday gifts, may I suggest this one: 1) In October send your marketing representative (perhaps yourself) to the practitioner's office and take a group photo of them and their staff. You can of course combine this with a marketing visit. 2) Using the printer of your choice (possibly a low cost internet company), print that office up a batch of photo holiday cards for them to mail out to their professional contacts. You would want to hand deliver them in early November. Altogether a turn-key solution to an annual problem they probably have to deal with on their own. I've never actually done this myself, but since it's my idea, I believe it to be a good one-as long as it doesn't cost too much.

Response: You asked for ideas regarding small tokens of appreciation for referral sources. I used to get some really good quality hand lotion in quarts (from Verne Bintz or FootSmart if I remember correctly). I could get it labeled with "A special gift for you from your friends at "your store name here" in appreciation for all you do throughout the year." Our hand lotion was kept in front of the office staff and doctors all year long, and I would keep a gallon handy and offer to refill their bottles any time I was in their office. I also sold it in the store and would often end up with the doctors' staff coming in to buy it so they would have some at home.

Question: I have been in healthcare for over thirty years as a respiratory therapist and opened a HME company 10 years ago but I am new as a C.Ped. My question is, if a patient comes in and molds are made for custom orthotics yet does not take possession, does Medicare provide a way to cover your expense? Thanks

Response: Here is an excerpt from the Medicare Claims Processing Manual that addresses this scenario.
20.3 - Artificial Limbs, Braces, and Other Custom Made Items Ordered But Not Furnished
(Rev. 1, 10-01-03)
B3-2005.3 

A. Date of Incurred Expense
If a custom-made item was ordered but not furnished to a beneficiary because the individual died or because the order was canceled by the beneficiary or because the beneficiary's condition changed and the item was no longer reasonable and necessary or appropriate, payment can be made based on the supplier's expenses. (See subsection B for determination of the allowed amount.) In such cases, the expense is considered incurred on the date the beneficiary died or the date the supplier learned of the cancellation or that the item was no longer reasonable and necessary or appropriate for the beneficiary's condition. If the beneficiary died or the beneficiary's condition changed and the item was no longer reasonable and necessary or appropriate, payment can be made on either an assigned or unassigned claim. If the beneficiary, for any other reason, canceled the order, payment can be made to the supplier only. 

B. Determination of Allowed Amount
The allowed amount is based on the services furnished and materials used, up to the date the supplier learned of the beneficiary's death or of the cancellation of the order or that the item was no longer reasonable and necessary or appropriate. The Durable Medical Equipment Regional Carrier (DMERC), carrier or intermediary, as appropriate, determines the services performed and the allowable amount appropriate in the particular situation. It takes into account any salvage value of the device to the supplier.

Where a supplier breaches an agreement to make a prosthesis, brace, or other custom-made device for a Medicare beneficiary, e.g., an unexcused failure to provide the article within the time specified in the contract, payment may not be made for any work or material expended on the item. Whether a particular supplier has lived up to its agreement, of course, depends on the facts in the individual case.

NIH Study Shows WalkAide Device by Hanger Orthopedic Group Significantly Improves Walking Ability In Children With Cerebral Palsy


There is new hope for children with a form of lower leg paralysis known as "foot drop" due to cerebral palsy (CP). At the 65th Annual Meeting of the American Academy of Cerebral Palsy Developmental Medicine on Saturday, October 15, researchers from the National Institutes of Health (NIH) presented a study that shows the WalkAide System successfully improves mobility by significantly increasing ankle control during walking in children with foot drop due to CP. This is the first study presenting the group results of a commercially-available foot drop stimulator in children with cerebral palsy. Full Text

Share prices fizzled for a few orthopedic device makers in the days following news of the Centers for Medicare & Medicaid Services' pre-reimbursement review project. Researchers at Canada's McMaster University have found that brief high intensity workouts rapidly lower blood sugar levels in type 2 diabetics, offering a potential fix for patients who struggle to meet exercise guidelines. The small proof-of-principle study, conducted on eight diabetics, is reported in the December issue of the Journal of Applied Physiology. Dietary intake was carefully standardized and recorded, both before the baseline 24-hour blood sugar and muscle tissue mitochondrial capacity measures (from thigh muscle biopsies), and during the trial.
