Question: I have been in healthcare for over thirty years as a respiratory therapist and opened a HME company 10 years ago but I am new as a C.Ped. My question is, if a patient comes in and molds are made for custom orthotics yet does not take possession, does Medicare provide a way to cover your expense? Thanks

Question: For the past decade, rocker bottom shoes have been in and out of vogue. What products are still being used by pedorthists now that they are no longer popular and why have you chosen the styles you are still using?

Response: Rocker bottom shoes can be highly beneficial for a wide variety of foot and ankle pathologies, and the key to selling these shoes is educating your customers and patients about their potential therapeutic benefit of these shoes. It's not about fitness and toning - it's about protecting the body against concrete and other hard, artificial surfaces that we walk on every day.
The rocker bottom shoe companies under the greatest scrutiny share three common traits:
1. They boldly advertised that the mere act of lacing up their shoes would help customers reduce cellulite, burn more calories and give them a more youthful, chiseled physique. "Skip the Gym" and "Fire Your Trainer" were common advertising themes. This drew the ire of the fitness community and medical experts. The American Council for Exercise (ACE), a trade organization for fitness clubs and personal trainers, launched a successful attack campaign against these companies, including the funding of a study challenging the fitness claims and suggesting potential health risks to wearing these shoes. Lawsuits have since been filed, and public perception of the entire category turned from favorable to unfavorable.
2. Most of the brands under scrutiny are "unstable rockers". By "unstable" I mean that they have EVA or PU SACH heels that provided no medial/lateral or frontal plane stability. Many people suffered injuries because they were unaware that wearing unstable shoes requires that you pay attention to your form, and to be mindful of the terrain you are walking on.
3. Many of these "unstable" rocker bottom shoes were mass marketed through department stores and via the internet, with little or no consultation by shoe fitters. Some brands provided an instructional DVD emphasizing the exercise and toning benefits of their shoes, but minimized or omitted any potential health risks or precautions. The rocker bottom shoes that still sell successfully are "stable" rocker sole shoes. These are heel-to-toe rockers that have good to exceptional medial/lateral stability. Heel and ball-of-foot pressure is offloaded, they are orthotic-friendly and they can be highly beneficial for hallux rigidus/limitus, mid-foot and ankle arthritis, plantar fasciitis and other pathologies. The wearer also feels a better sense of control when they wear "stable" rocker bottom shoes. Some of the best "stable" rockers, in alphabetical order, are Aetrex BodyWorks, Finn Comfort "Finnamic", Ryn and Sano by Mephisto. Prices at retail range from $120 - $340.
The key to selling rocker bottom shoes is to educate your patients/customers about the potential benefits of a rocker bottom shoe for their pain issue. If Ryn offloads ball-of-foot and heel pressure, it could be extremely beneficial for Hallux rigidus, sesamoiditis and metatarsalgia. BodyWorks, Sano and Finnamic can also produce the desired result for a wide variety of issues at various price points.
Customers are wary, and will not ask for these shoes - it is your responsibility to consult with them and let them know why you think a particular rocker bottom shoe may be beneficial. If you know why you have these shoes, and which pathologies they can be helpful with, then you should have no problems selling them. Your customers and patients don't know why they hurt or what to do about their pain. They are looking for you to help them find an answer. Rocker bottom shoes are not the panacea for foot, leg and back pain, but they can be a very useful tool in your toolbox.

Response: If the customer is healthy without stability issues, the MBT is still good. For everyone else, we like the rockers that are stable, such as those by Aetrex, PW Minor and Ryn. Although it doesn't look like a rocker, ZCoil is a rocker in how it functions. Although anyone can wear them, they are especially good for those with more serious pain issues.

Researchers at Queen Mary, University of London, have claimed that the lack of use of available prediction tools for identifying people at risk of diabetes is increasing the number of people developing the metabolic condition.
The study, which was published in the British Medical Journal, found there are many tools for and techniques that can predict diabetes risk in people with a reasonable chance of success, but that these are hardly ever being used. Full Text

Eating fish rich in omega-3 fatty acids, such as cod and salmon, may significantly lower a young woman's risk of developing heart disease, Danish researchers report. The researchers found that women of childbearing age who never ate fish had 50 percent more cardiovascular problems than women who ate fish often, and a 90 percent higher risk than women who ate fish weekly. "We found that even women who ate fish only a couple of times a month benefited," said lead researcher Marin Strom, a postdoctoral fellow at the Center for Fetal Programming at the Statens Serum Institute in Copenhagen.
