Question: I am a certified therapeutic shoe fitter with ABC. I started out fitting shoes with a DME company for the Medicare Therapeutic shoe program. Now, it seems to me, that a pedorthist certificate is required to fit seniors in the program.

 
Just wondering what other options or benefits I can explore? At this point it seems ABC is benefiting most from my annual fee and continuing education credits. Any response(s) would be greatly appreciated.

Question: Is anyone aware of a manufacturer that carries an A5500 qualifying shoe in an athletic lace style in a size 1 youth. I have come across some that have the necessary depth, materials and closure but are not offered in the 3 widths necessary to satisfy the A5500 definition. Thanks.

Response: Go to www.dmepdac.com/dmecsapp/do/search to obtain a list of all PDAC approved shoes. Beware that this will return over 5000 results, but may get you pointed in the right direction as far as some of the obscure vendors out there. Alternately, you may want to consider custom athletic shoes. I have had to resort to this for my patients with really small feet.

Question: A few weeks ago, I posted a question asking how many people were not planning on attending the PFA Symposium and why. I received many responses via email to my question. Most cited either poor economy or lack of bang for their buck. Many said they had to choose between PFA, Medtrade, AOPA because of limited time and budget. After attending this year's event, being graciously invited by one of the foundational vendors, I both observed and heard many attendees reaction to this year's show. 

 

Being an educator, many former students, and peers stopped to talk to me. Most were in shock that attendance was so low, both on the vendor and general attendee side. Several were disappointed with the quality and relevance of specific presentations. Many others voiced that on a per credit earned basis, attending the symposium was not very economical. Many of the vendors I talked to expressed the same sentiment of disappointment. If you attended, I would like your opinion on the economics and quality of this year's event, as well as plans to attend subsequent events in the future. Is distance education a more viable option?

Response: I have heard feedback from one vendor that attended PFA say that, from the vendor's perspective they were disappointed in the attendee turn out in the exhibitor's hall and the difficulty for attendees to have time with vendors. Obviously vendor support is a key component of the financial success of the seminar so one can hope that their needs are better met in future seminars. I know that as an attendee in past PFA seminars that it can be a challenge to leave a presentation, stop by a restroom, get to the exhibitors hall, see a few vendors, and then get back to the next presentation on time. Even with time blocked out for exhibit hall it was a challenge to have quality time with all the key vendors and then go exploring for new companies and their offerings. I know I would come back from PFA seminars feeling like I had been on the run for several days.


At other trade shows I have attended, the seminar management recognized that many individuals are early risers or do not mind working late so they allow exhibitor halls to open at 7:00 am and to go late to cater to both types of attendees. Hopefully vendors bring enough personnel to staff their booths with split shifts but the opportunity to increase contact with attendees should make it worthwhile.

Shinseki today announced the publication of a final regulation in the Federal Register on clothing allowance that expands the eligibility criteria for Veterans with multiple prosthetic and orthopedic devices or skin conditions caused by prescribed medications. "Veterans whose service-connected disabilities require prosthetic appliances and skin medications deserve financial assistance for the extra clothing they have to purchase," said Shinseki. "A federal appeals court has ruled that Veterans may be entitled to more than one clothing allowance, and we are adjusting our regulations to make sure Veterans get the benefit of that decision." Full Text

After a heavy oxygen tank fell on her foot while she was visiting her mother in a nursing home earlier this month, Tamra Williams was in pain all night-but dreaded what she figured would be an hours-long wait in a Dallas hospital emergency room. So the next morning she headed to a PrimaCare urgent-care clinic near a shopping mall in suburban Mesquite, Texas, where a doctor treated her fractured toe and sent her on her way within an hour. 
 
Clinics like PrimaCare's-and others in many drugstores that offer fewer services-are increasingly cropping up nationwide to address a widening deficit in care: The number of patients seeking emergency care rose more than 43% between 1990 and 2009, while the number of hospital emergency departments declined 27% over the same period, says Renee Hsia, an emergency-medicine researcher at the University of California, San Francisco. Full Text

The maker of Tootsie Roll candy is suing a small shoe company over its Footzyrolls brand of shoes that roll up into compact bundles. In a lawsuit filed last week in Illinois federal court, Chicago-based Tootsie Roll Industries accused Rollashoe of intentionally preying on its famous trademark used for lollipops and chewy candies since 1908. Footzyrolls "so resembles each of the Tootsie Roll Marks in sound, commercial impression and appearance that when used in association with footwear, it is likely to cause confusion, to cause mistake or deceive," Tootsie Roll said in its complaint. Hispanics are the focus of a campaign to promote foot care among people with diabetes, says the American Podiatric Medical Association (APMA). Hispanics are 66 percent more likely than whites to be diagnosed with diabetes, which can result in foot ulcers and lower limb amputations. Leading causes of type 2 diabetes include poor diet, lack of exercise and genetics, risk factors that are especially high among Hispanics, according to the association. Full Text

In 2009, the American Diabetes Association (ADA) recommended that Hemoglobin A1c be exclusively used for the diagnosis of diabetes in children. The simple test measures longer-term blood sugar levels -- without requiring patients to fast overnight. However, a new U-M study has shown that these tests are not very accurate in children. "We found that Hemoglobin A1c is not as reliable a test for identifying children with diabetes or children at high risk for diabetes compared with other tests in children," says Joyce M. Lee, M.D., M.P.H., lead author of the study and a pediatric endocrinologist at U-M's C.S. Mott Children's Hospital. "In fact, it failed to diagnose two out of three children participating in the study who truly did have diabetes." Although tests that require patients to fast are the most accurate tests, they are difficult to obtain in clinical practice, highlighting the need for improved nonfasting testing strategies. This is the first time that real-time Continuous Glucose Monitoring (CGM) has been studied in a large U.S.-based, randomized-controlled trial in people with Type 2 diabetes. Most studies have demonstrated that real-time CGM is effective in patients with Type 1 diabetes, or patients using intensive insulin treatment of both basal and meal time insulin. Full Text

Recently, I saw the news of a 100-year-old man, Fauja Singh, who finished the Toronto Marathon in 8 hours, 11 minutes. What struck me as interesting was that he started running at 81 years of age. I have been involved with marathons, professionally, for years and recently had the opportunity to watch the Ironman World Championships in Kona. I have observed for over a decade that the average age of "endurance" athletes is much older than one would expect. Twice, Diana Nyad, 61, made attempts to swim from Cuba to Florida... certainly, there have not been any 20-somethings performing such feats. But why is this? Full Text
