Question: Is anyone aware of a manufacturer that carries an A5500 qualifying shoe in an athletic lace style in a size 1 youth. I have come across some that have the necessary depth, materials and closure but are not offered in the 3 widths necessary to satisfy the A5500 definition. Thanks.

Question: A few weeks ago, I posted a question asking how many people were not planning on attending the PFA Symposium and why. I received many responses via email to my question. Most cited either poor economy or lack of bang for their buck. Many said they had to choose between PFA, Medtrade, AOPA because of limited time and budget. After attending this year's event, being graciously invited by one of the foundational vendors, I both observed and heard many attendees reaction to this year's show. Being an educator, many former students, and peers stopped to talk to me. Most were in shock that attendance was so low, both on the vendor and general attendee side. Several were disappointed with the quality and relevance of specific presentations. Many others voiced that on a per credit earned basis, attending the symposium was not very economical. Many of the vendors I talked to expressed the same sentiment of disappointment. If you attended, I would like your opinion on the economics and quality of this year's event, as well as plans to attend subsequent events in the future. Is distance education a more viable option?

Question: Stretching exercises are often recommended for plantar fasciitis. What has proven to be effective stretching regimens and how often should one do them?

Response: Before one can establish the quantity in the stretching program, one must first understand the necessity of the quality of the stretch. If the foot is allowed to move into a pes planus or pronated posture the effect of the stretch is less than fifty percent, and could require at least 10 times a day or more and will never accomplish the expected goal. Mechanically one can internally rotate the foot and then externally rotate the leg. This will supinate the foot and increase the effect of the stretch. Secondarily the quantity, based on experience, is just as important. If the weight is in access of the norms for the height, one should be stretching even more.

 

The greater the body weight the greater the energy requirement in the gastroc, up to one and one and a half times body weight, with every step. Jogging for exercise can increase the forces in the gastroc up to three times body weight. In other words the more you weigh and the more active you are, the more stretching is required, and the quantities and efficiencies in stretching are parallel. In my practice I find that most are not willing to go the course, because of the time requirement and consequently are candidates for a surgical gastroc release.


To be repetitive, the patient must be committed to their treatment program.

Question: I generally recommend my runners to replace their running shoes every 300-400 miles. But there are so many factors involved, and I was wondering if I should be more specific?

Response: I refrain from telling my patients/customers anything as far as using mileage as a gauge for replacing running shoes. I hear sales people all the time go on and on about replacing a shoe after X number of miles. I disagree with that theory on the simple principle that you could easily have two people with the exact same size feet and one can weigh fifty pounds more than the other; but I'm supposed to believe they'll both wear their shoes out after the same number of miles? Of course not. Other variables include the running surface and the person's running style.

Instead of assigning a mileage, I spend time discussing with my patients what signs to look for that would indicate that the shoe is worn out.


If I'm missing something here, please let me know.

Response: Mileage is one of the important signs that a shoe is no longer functional and balanced. So is the type of foot and mechanical forces applied to the shoe. The fit of the shoe is ultimately the most important reason to get new running shoes since so many people wear their running shoes too short. The wear pattern of the midsole, outsole, counter, and linings are the places to check to see if it's time for new shoes.

In its 10Q filing with the Securities & Exchange Commission filed last Thursday, Skechers USA Inc. indicated that it expects to be found liable by the Federal Trade Commission for false and deceptive advertising in connection with its toning products. It said it anticipates a "comparable, or higher" fine as the $25 million Reebok had to pay in its settlement with FTC over toning claims. 

Skechers wrote in the filing, "Based on discussions with the FTC staff, the company does not believe that the FTC's pending inquiry into the company's toning products will likely end in a closure letter assuring no further regulatory action." Noting that Reebok recently reached a $25 million settlement with the FTC after a similar dispute, Skechers said, :It is reasonably possible that [we] could be subject to a comparable, or higher, exposure as a result of these proceedings ... [and] it is possible that costs associated with them could have a material adverse impact on the company's consolidated earnings, financial position, or cash flows." Full Text

Soft consumer confidence and lingering unemployment have the nation's largest retailers looking to the 2011 holiday season with caution, according to a recent BDO USA survey of chief marketing officers. The survey found CMOs with those retailers expect a modest 2.9 percent increase in total holiday sales this year. This is a less optimistic point of view compared to the projected 3.5 percent increase in 2010 (and the actual 5.2% increase reported by the National Retail Federation). Overall, 41 percent of CMOs expect an increase, down from 2010 (52%) and 2009 (47%). However, CMOs at the top 100 largest retailers included in the sample are more optimistic, with 67 percent expecting an increase in overall holiday sales. Full Text

What happens when a city councilman's wife is promised a shoe discount, then doesn't get it? Her husband sues. Lorraine Koppell was invited to a "Fall Private Shoe Sale" at Bloomingdale's that promised 35% to 40% off on the enormous fall collection - but only offered the steep discount on a few shoes. G. Oliver Koppell, who has a private law practice, filed a class-action lawsuit claiming Bloomingdale's used false advertising to lure customers into the store. Anyone whose foot rolls inward (pronation) during a stride is a candidate, but the real high risk for runners are people with extremely flat feet, a large, pronating forefoot, or a so-called "Morton's foot" (where the second toe is longer than the first, causing an exaggerated pronation). Older women who got more exercise and less television time were the least likely to be diagnosed with depression, according to a U.S. study of thousands of women with physical activity having the biggest impact.
 
According to findings published in the American Journal of Epidemiology, researchers found that women who reported exercising the most in recent years were about 20 per cent less likely to get depression than those who rarely exercised. Full Text
