Question: Stretching exercises are often recommended for plantar fasciitis. What has proven to be effective stretching regimens and how often should one do them?

Response:  We outlined some plantar fasciitis strengthening and stretching exercises (with permission provided by The Running Injury Clinic) on our blog earlier this year including plantar fascia towel crunches and first ray stretches (complete with images).

Response: 1. Seat yourself in an upright position with the leg to be stretched at full extension in front of you.

(I usually suggest doing this with leg and back supported by a bed or sit in a chair with a second chair supporting the leg to avoid knee pain.)

2.    Using a towel, sheet, or Pilates band grasped in both hands, wrap/cradle around the ball of the foot.

3.    Internally rotate the forefoot so that your third toe appears to line up with the shin.

(Unbeknownst to the patient, this is to simulate a subtalar neutral position and prevent overpronation during the stretch.)

4.    Keeping head up, chest out, back and leg straight pull on the strap until the stretch is achieved.

(The feeling of the stretch will be different for each patient. Some are tighter in the Achilles/gastroc, some in the plantar fascia, some in the hamstring.)

5.    Hold this for 30 seconds then slowly release, switch to the other leg. Repeat this for five repetitions.

(Even though the PF may be unilateral, the other leg is typically tight too. Overcompensation from pain will put more stress on the other limb and may create bilateral fasciitis.)

6.    Do this twice a day. The best time to do so is as close to waking as possible and again before bed.

(Things begin to tighten during rest and one of the most common complaints for plantar fasciitis is extreme pain first thing in the morning)

 

Some of our local doctors still prescribe the UFOs if patient compliance is in question. Fortunately it usually isn't too much of an issue. By the time they are coming to us they are near their pain limit and just want it to stop. For most of the patients that actually do it, this stretch has worked quite well. I also suggest they freeze a water bottle and roll their foot on it for cold massage. Many will come in with pre-made orthotics that are visco-elastic in nature thinking they need to cushion the foot. I explain the need for support and fascial relief. We like to make a custom total contact orthosis using a Puff, EFM, Multicork tri-laminate that really gets into the arch and supports the foot well. An extra pad or localized depression can be used if spurring is involved. On rare occasions if there is an extremely tight Achilles, I may use a ¼" cork heel raise to gain quicker relief. This is thinned down over time as the patient gains more range. This modality seems to take care of most of our plantar fascial cases. I find it important to remind the patient that while an orthotic will make things feel better, the root problem (assuming a patient with tight ROM) is the tightness which must be addressed. I suggest the stretching be done religiously for about two weeks then changed to maintenance of once or twice a week. 

 

I also remind that things will tighten up again if not attended to at least occasionally. Even though the pain may go away it is good to be checked yearly for orthotic integrity and to see how ROM is progressing. A lot of plantar fascial cases that come to me are years after their orthotic "cure" from another company and don't understand why it came back. Evaluation, information, treatment and diligence win out most every time.

Response: I feel that the choice of stretches should be preceded by a good foot evaluation.  All plantar fasciitis patients don't need all of the exercises.  For example, if a patient does not have pf band tightness I will skip the standing with toes extended against the base of the wall.  This would also be true if the patient has any limitation of toe extension.  I modify the standard runner's calf stretch.  I feel that you can get a better stretch by first positioning the rear foot in the sagittgal plane and, if the patient is able, actively correcting the subtalar joint to neutral. This is especially important to those with significant pronation problems.  The stretch is then applied in the normal fashion by leaning forward, keeping the knee straight on the leg being stretched.  The time for holding a stretch is always being debated.  Some fairly recent research indicates that holding a stretch beyond 30 seconds doesn't give any additional benefit.  The number of repetitions can be debated.  I usually have my patients follow a 10x10 model - 10 repetitions for 10 secs each, for ease of remembrance and just to make sure that they are doing enough - no scientific basis.  Instructing the patient in correct technique is critical to success.
