Question: I just finished a CE class in Philadelphia last week where the subject of documentation came up. Specifically, the topic of Physician Progress Notes relating to the exam and need for shoes and inserts was of interest. Most (yes, most) of the attendees were surprised by this requirement and stated that they had not heard that the copy of notes were required. Am I missing something here? In all of my busy time did I miss a change somewhere? Documentation has always seemed to be the lion chasing us in this field. Any feedback with sources that I could cite would be appreciated.

Question: Stretching exercises are often recommended for plantar fasciitis. What has proven to be effective stretching regimens and how often should one do them?

Question: Are you a pedorthic owned Medicare supplier business, that as a participating supplier, you have had an audit of TSD claims? If so, please respond yes and share a little about the audit (how many files, success rate of not having payment recouped, challenges of supplying info required, etc.). I am trying to advise another pedorthic shoe store owner about this issue, and he believes that being a participating supplier means he will be less likely to be audited on his TSD claims. I believe the advantages of being a non-participating supplier far outweigh the advantages of being a participating supplier. In fact, I am not sure there is any meaningful advantage to being a participating supplier. Question: Are any PNW readers stocking and dispensing off-the-shelf walker boots, post-op shoes, Darco healing wedge-type shoes, etc? If so, how are you handling billing/reimbursement? Is there any reimbursement for a pedorthist?

Response: We do sell off-the-shelf post-op and surgical shoes and boots. The patient pays us and submits to their insurance company. For us, it's a very small business.

Question: I am really interested in how many people are not going to the PFA Symposium next month, and why. Many of the folks I have talked to have surprisingly said they are not going. Most have said it is the economy. Others have said they have found other CE opportunities closer to home via local associations. Yet others cite the 15 points being offered as a bang for the buck issue. What I would like to know is if people would take advantage of CE offerings if they were offered regionally, and what kind of topics would you like to see?

Response: As the sole pedorthist at our pedorthic shoe store, it is the loss of revenues from not being able to be at my place of business in order to attend PFA that is of issue. Each day that I do not see clients is a financial hit. There is a need for good pedorthic locum tenens service (experienced pedorthists that do not want to work full time or even part time but could cover a store for a 3-4 day stretch). Perhaps a registry of these "retired" pedorthists and their contact info would be an asset to businesses across the country that would enable more pedorthic business owners to attend PFA seminars. This registry could help store owners for other time off situations (vacation, illness) much like physician locum tenens businesses do. My wife used to run a $3.5 million branch office for a national locum tenens company. It specialized in OBGYN physicians. These specialists go on vacation, attend continuing education seminars, and do fall sick, yet babies do not wait to arrive. No one ever thought that women would be willing to have a different OBGYN deliver their baby but there are times when women across the country have to accept this substitute physician. A registry of fill-in pedorthists could be a real benefit to PFA members. - PNW Advocate PREVIOUS QUESTION WITHOUT A RESPONSE Question: I was reading a Medicare article this morning. It stated Round 2 for competitive bidding for O & P has begun. Maybe I have had my head in the sand or just skimming to many articles, but I was unaware of Round 1. Orthopedic surgeons have received hundreds of millions of dollars from joint implant manufacturers in recent years, according to a recent report. In 2007, five device makers said they had paid surgeons more than $198 million, with 43 payments exceeding $1 million. While the number of payments appears to have dropped since 2007, the average dollar amount has gone up, based on data from the three manufacturers that disclosed physician payments made in the last several years. Women living in poor neighborhoods are more likely to be obese and have type 2 diabetes than those who move into more advantaged areas, new research suggests.
In the first randomized trial of its kind, researchers provided women living in high-poverty areas with vouchers and counseling so they could move into better neighborhoods. After 10 years of living in the new areas, those women were 19 percent less likely to be morbidly obese, and 22 percent less likely to have developed type 2 diabetes compared to the control group that stayed in high-poverty neighborhoods. The Centers for Disease Control and Prevention will recommend on Oct. 25 that patients with diabetes receive immunization with the hepatitis B vaccine, following findings that U.S. adults with diabetes patients face a roughly twofold increased risk for hepatitis B infection.
Results from CDC investigations identified inadequately cleaned blood glucose monitors as a major route of hepatitis B virus transmission among patients with diabetes, Meredith L. Reilly, a CDC epidemiologist, said while presenting a poster at the meeting. Full Text

Many people will develop diabetes in their lives. The causes of Type II diabetes are most often related to weight gains from taking in too many calories and not getting enough exercise. A lack of balancing "calories in" versus "calories out" can contribute to the development of this life-changing condition. In diabetes, the pancreas does not produce enough of the hormone insulin to help the body break down carbohydrates to useable sugars from foods. Glucose is a breakdown of theses sugars. Chanting "om" might help ease your aching back, but only if it comes at the end of yoga practice. A new study finds that the physical act of doing yoga - but not its meditative aspect - may help reduce symptoms of chronic back pain.
Based on smaller previous studies, researchers thought it was some combination of yoga's stretching and its mental elements - deep breathing and relaxation - that improved back sufferers' function. But the current study found that yoga worked no better than intensive stretching alone, suggesting that it's the physical exercise, not the mindfulness component of yoga, that matters. Full Text

Children's emergency room visits for concussion increased 60% over eight years, according to a new study by the Centers for Disease Control and Prevention (CDC) - but that's not necessarily bad news.The authors said that the increase was likely due to broader awareness of concussion and head injuries, not because the actual number of concussions was increasing.
