Announcement: In preparation for PFA's 52nd Annual Symposium and Exhibition in Albuquerque, New Mexico, we are pleased to request nominations for the Dawn Janisse Award. The award will be presented during the Pedorthic Foundation's Reception on Wednesday, November 9th, between 6:00 PM and 9:00 PM. The reception will be held in the atrium of the Hyatt Hotel. All attendees are invited to the reception.

Each year before the annual PFA symposium, the Foundation encourages the pedorthic community to submit nominations for the Dawn Janisse "Making a Difference" Award. The award is given to a person, company or group that has made a significant or extraordinary difference in or for the pedorthic profession. Recipients of the Dawn Janisse "Making a Difference" Award are selected by a Foundation committee. Nominations for the award are submitted by Foundation Board members and by other members of the pedorthic community. Nominations must include a brief description of the individual's, company's or group's contributions to the field and the reason those contributions have made a difference for the pedorthic profession. Question: I'm looking for local or national educational courses about fitting shoes and foot orthotics for kids. We specialize in custom footwear and have many child patients to work with and would love to help them at the highest quality. Any information?

Response: PNW's inquiry brings back an experience that retells itself over and over again. Some time ago I became a C.Ped. and my primary goal was to focus on children only. Each year I would attend the Pedorthic Symposium only to finally realize that issues pertaining to kids were rarely if ever discussed.

However I refused to give up. It's one thing to take the courses and another to use your knowledge to create an Orthotic business for kids only. It takes time to develop an eye to prescribe and fit kids in proper Orthotics. Through trial and error I taught myself to evaluate kids and prescribe different kinds of off the shelf Orthotics. In many instances the type of off the shelf Orthotics eliminates the need for customs.

Pretty soon word of mouth brought so many kids who needed help. While so many independent shoe stores are struggling, a portion of our business is immune to competition, discounters and online sales. Creating this component is a primary way to market our business. We do not sell Orthopedic shoes. We use national and private brands that come in wide and extra wide widths. The families that come through our door want to leave with footwear that looks exactly like their peers. We have also morphed into the special needs market. It is personally rewarding to know that we make a difference in one's life.

The point here is although we are a regular shoe store we are thriving because of our niche marked. Half of our Orthotic market is from outside of our area.

Question:  I've asked this question in the past and I still can't seem to get a satisfactory answer. Some of the major running shoe companies have, for some reason decided to make neutral cushion shoes that actually function as pronation control shoes. My idea of a neutral cushion shoe is that it should be neutral and provide more cushion via lower durometer materials. What I am seeing is shoes that are thicker soled on the medial v lateral side. The most puzzling part is they are using very soft durometer material on the lateral side, especially in the crash pad portion of the shoe.

As any buyers guide will show, a neutral cushion shoe is typically suited for supinators, cavus or under-pronators. As a result, I am seeing many patients that will prematurely crush and break down the shoe and in some cases they will grossly invert in a brand new one right out of the box. This is also occurring in spite of having them in a cavus orthotic with lateral posting and a lowered medial arch. I have had to return/swap several pairs in one brand but I have seen it in some Asics, Nike, Brooks and New Balance models over the past 1.5 years or so.

My only guess is that some testing or theory I am not aware of, dictates making the crash pad and lateral border much softer to attenuate shock. However, doing so in an underpronator/cavus person, only encourages inversion and supination.....precisely what we are trying to mitigate. I also realize, in true running scenarios, there is less time spent in pure heel contact before it transfer to forefoot loading. That said, I would imagine that the large majority of running shoes are not actually used for any significant running.

It's not only low durometer EVA. I've also seen this issue in some shoes that utilize pods made from visco-elastic polymer type gel material that is not robust enough to control inversion. I would hope this trend is corrected soon because it makes absolutely zero bio mechanical sense to market these as neutral shoes

Response: Brian you are so right. There are no running shoes made for the underpronator. Instead of rebuilding the rear quarter, what I do, is take a stock running shoe and using a knife, cut a slit 2" - 4" long (depending on the size of the shoe). It needs to be about 1/4" to 1/2" in on the lateral side of the shoe, from the heel to the midfoot. The slit is about 1/2" -5/8" deep. Then using a 1/16" thick piece of polypropylene, cut to the same length and width as the slit, I insert the plastic into the slit and trim excess material off so the plastic is slightly recessed into the sole. Next, you need to rough plastic on both sides. Then, using Fast Fix or contact cement coated on both sides of the plastic, spread the slit in the shoe open and insert plastic shim. Press sides together and let dry. - viola! a lateral stiff sole.

Care should be taken when cutting the slit so you don't cut into the lasting board, sock liner or pop any gel or flow pockets imbedded into the heel. Note: the plastic piece shouldn't extend thru the bottom of the sole. It should be cut 1/6" to 1/8" shy of the sole as not to block all shock absorbing properties of the midsole.

Question: I was speaking with another pedorthic shoe store owner that is going through the process of looking to become a Medicare supplier. The issue of requesting a non-participating supplier status versus a participating status came up.  He was under the impression that being a participating supplier had significant advantages over a non-participating supplier.  He made a reference to fewer audits of participating providers.  When I search the database of Medicare suppliers in our state, I find very few that have elected to be participating providers.  Does anyone have a definitive answer about "fewer audits" or any other real advantage to requesting participating status?

Response: This is in response to the question John has posted about Medicare and Participating Provider vs. Non- Participating Providers. We went with Non because you can charge the patient up front and file non-assigned, and they get the check from Medicare. With Participating Providers you have to accept what Medicare pays. As far as audits, I am not sure but would love to find out how that coincides with participating providers.

Older women taking the breast cancer drug tamoxifen may have an increased risk of developing diabetes, a new study suggests. The findings, reported in the journal Cancer, do not prove that tamoxifen directly leads to diabetes in some women.

But researchers say it is plausible that in women with known risk factors for diabetes-like obesity or family history of the disease-tamoxifen furthers the risk somewhat. Full Text

Diabetes Education Seems to Help Improve Blood Sugar Control

Three new studies show that diabetes education and behavioral interventions can help lower blood sugar levels in people who are having trouble managing their diabetes.

  

Each study looked at different types of educational and behavioral interventions and found some improvement in long-term blood sugar control. The results are published in the Oct. 10 online edition of the Archives of Internal Medicine.

  

"Diabetes education is fundamental. And, the time and effort spent with patients relates nicely to better outcomes," said Dr. Joel Zonszein, director of the clinical diabetes program at Montefiore Medical Center in New York City. "In these studies, the strategy is a little bit different in each, but it shows that education works." Full Text.

Coaching Fails to Improve Diabetes in Disadvantaged Patients

A diabetes education intervention that focused on individual telephone "coaching" failed to improve hemoglobin A1c levels in a study of poor, uninsured, ethnic minority patients with poorly controlled diabetes that was reported online Oct. 10 in Archives of Internal Medicine.

 

"The intervention tested in this trial appeared to be insufficient to make a statistically significant difference compared with the control condition, despite anecdotal reports from participants that the intervention was valuable to them and provided support they had not previously received as part of their diabetes care," said Dominick L. Frosch, Ph.D., of the department of health services research, Palo Alto (Calif.) Medical Foundation Research Institute, and his associates. Full Text

Gender of Eating Companions Influences How Much People Eat, Says Study

It's not just the company we keep that influences how much we eat. A new study suggests it's the sex of the people around us that leads us to consume more or less food.

Researchers from Indiana University of Pennsylvania and the University of Akron found that the average number of calories college students consumed varied depending on whether they ate with men, women or a mixed-sex group.

"What we found was that on average, when men were eating with women, they tended to purchase more calories than their counterparts who were eating with other men. Women tended to purchase fewer calories when with men as compared to when they're with women," said Marci Cottingham, a co-author of the study and a graduate student at the University of Akron. Text messages help with weight loss

College students who used cellphones to monitor calories and physical activity and received personalized feedback about how they were doing dropped significantly more weight than those who didn't, new research shows.

"Cellphones are a powerful intervention tool for weight loss," says lead author Melissa Napolitano, a research scientist at the Center for Obesity Research and Education and an associate professor of kinesiology at Temple University in Philadelphia.
She and colleagues recruited 52 overweight college students, mostly women, who weighed an average of 190 pounds, and assigned them to one of three plans. Full Text
