Question: Among the brands we carry now at our pedorthic shoe store, we are missing a cordovan men's dress shoe style in narrow, medium, wide, and XW that comes with the cordovan leather upper that has a shiny finish.  Men who travel for the job are asking for this color/style to wear with either gray suits or blue suits so they can take just one pair of shoes on their business trips.  PW Minor has a cordovan shoe in their offering but the style and finish are not what business men are looking for in this shoe.  Aetrex has a brown shoe in its Lexington collection that has a slight reddish tone but it is not a true cordovan. Obviously we want a shoe that has firm heel counters and torsional rigidity to provide proper support.  Drew does not offer this color in a dress shoe style.  Any suggestions?

Response: Sequoia still has shiny cordovan dress shoes, either with a smooth finish or wingtip styles with removable insoles, multiple widths, and strong counters.  While the leather and vibram soles are initially fairly stiff, they do break in nicely.   

Tauer & Johnson is a made to order company that we enjoy doing business with:  choose from 15 or more styles from plain to cap to wingtips, leather type and colors, depth or not, then soling options range from traditional flex leather soles to several vibram sole choices.  Extra strong counters are an option.  These can even be ordered mis-mated for retail under $400.  Initial fitting inventory takes an investment but often they have programs to help finance.  These could work well in Colorado since they have lug sole options also.  The vibram 1743 soles are good for comfort dress wear.  

Alden is still making this style of shoes, also, but costs and delivery can be tough.  
We would like to see Dunham and/or Aetrex offer this basic men's color that is still desirable in the Midwest at least.

Question:  I've asked this question in the past and I still can't seem to get a satisfactory answer. Some of the major running shoe companies have, for some reason decided to make neutral cushion shoes that actually function as pronation control shoes. My idea of a neutral cushion shoe is that it should be neutral and provide more cushion via lower durometer materials. What I am seeing is shoes that are thicker soled on the medial v lateral side. The most puzzling part is they are using very soft durometer material on the lateral side, especially in the crash pad portion of the shoe.

As any buyers guide will show, a neutral cushion shoe is typically suited for supinators, cavus or under-pronators. As a result, I am seeing many patients that will prematurely crush and break down the shoe and in some cases they will grossly invert in a brand new one right out of the box. This is also occurring in spite of having them in a cavus orthotic with lateral posting and a lowered medial arch. I have had to return/swap several pairs in one brand but I have seen it in some Asics, Nike, Brooks and New Balance models over the past 1.5 years or so.

My only guess is that some testing or theory I am not aware of, dictates making the crash pad and lateral border much softer to attenuate shock. However, doing so in an underpronator/cavus person, only encourages inversion and supination.....precisely what we are trying to mitigate. I also realize, in true running scenarios, there is less time spent in pure heel contact before it transfer to forefoot loading. That said, I would imagine that the large majority of running shoes are not actually used for any significant running.

It's not only low durometer EVA. I've also seen this issue in some shoes that utilize pods made from visco-elastic polymer type gel material that is not robust enough to control inversion. I would hope this trend is corrected soon because it makes absolutely zero bio mechanical sense to market these as neutral shoes

Response: I had the situation of an orthotist wanting a "neutral" athletic walking shoe recently so he could add a lateral buttress for a patient with a foot deformity that caused a very inverted foot position during gait and stance.  My response to the orthotist was that it did not matter if the athletic walking shoe was labeled for neutral or stability, there was going to be heel strike cushioning midsole in shoes.
 
After discussing various options with the orthotists, the best solution was for him to remove the cushioning material on the lateral side of the rearfoot portion of midsole and replace it with firmer material before adding the buttress modification.  This midsole modification would be the equivalent of doing just the opposite of a SACH heel mod, plus being more laterally placed.

Question: I was speaking with another pedorthic shoe store owner that is going through the process of looking to become a Medicare supplier. The issue of requesting a non-participating supplier status versus a participating status came up.  He was under the impression that being a participating supplier had significant advantages over a non-participating supplier.  He made a reference to fewer audits of participating providers.  When I search the database of Medicare suppliers in our state, I find very few that have elected to be participating providers.  Does anyone have a definitive answer about "fewer audits" or any other real advantage to requesting participating status?

Reebok International Ltd has agreed to pay $25 million to settle charges that it made unsupported claims that its "toning shoes" provide extra muscle strength, the U.S. Federal Trade Commission said on Wednesday.The penalty will go toward consumer refunds. The FTC said in its complaint that Reebok, a unit of Adidas AG, falsely claimed in advertising that its EasyTone and RunTone shoes "strengthen and tone key leg and buttock (gluteus maximus) muscles more than regular shoes." Patients with gout tend to wear shoes that lack cushioning, provide minimal stability and motion control, limit gait efficiency, fit poorly, and show excessive wear, according to a report in the October issue of Arthritis Care & Research.

Since all of these problems can exacerbate patients' pain and impairment, "We suggest that footwear should be considered in the management plan of patients with gout," said Keith Rome, Ph.D., professor of podiatry at Auckland (New Zealand) University of Technology, and his associates.

The investigators performed a cross-sectional observational study to assess the characteristics of gout patients' footwear. They enrolled 50 patients, predominantly middle-aged men who had longstanding disease that had been diagnosed by a physician according to American College of Rheumatology criteria. Full Text

Despite the best efforts of food companies to convince consumers otherwise, 100-calorie packs aren't a magic-bullet solution to healthy eating, experts say. "You're not getting much quality food, and it's not really filling," said Indi Maharaj, a registered dietitian at Erlanger's Lifestyle Center. "It's Dieting 101 for lazy people."
About five years ago, snack food manufacturers such as Nabisco (Oreo, Chips Ahoy! and Ritz Crackers) and Frito-Lay North America (Cheetos, Fritos and Sun Chips) began producing lines of individually packaged, 100-calorie snacks.

Experts said a healthy snack should weigh in at less than 400 calories. While 100 calories falls well under that limit, crackers, cookies and chips have low nutritional density, which doesn't improve by coming in a smaller pouch. Full Text

Rates of Diabetes-related Amputation vary across U.S.

About 26 million Americans have diabetes, and an estimated 65,000 had a lower limb amputation in 2006, the most recent year with available data. One recent study found that the rate of amputation may be declining among Americans with diabetes. But the new findings, reported in the journal Diabetes Care, suggest that in some parts of the country that rate can be almost double the national average -- at least among older Americans.

Amputation is a complication of diabetes because the disease often causes nerve damage over time. When people lose sensation in their feet and legs, they may more easily get a cut, blister or sore -- even from ill-fitting shoes -- and be less likely to notice it until it's infected. Full Text

When contending with diabetes, most people focus concern on the pancreas, where insulin is produced, often in inadequate quantities. But with Type 1 and Type 2 diabetes, a key culprit could be the liver. So says H. Henry Dong, who holds a doctoral degree and serves as associate professor of pediatrics at the University of Pittsburgh School of Medicine. Fast Eaters at Twice the risk of Developing Diabetes

People who wolf down their food are twice as likely to develop impaired glucose tolerance, known as pre-diabetes, according to a new study.
The increase was not seen in people with other eating patterns, including snacking and late-night eating.
Type 2 diabetes is caused when not enough insulin - which converts blood glucose into energy in cells - is produced for the body to function properly.
In those with impaired glucose tolerance (IGT), blood glucose levels are higher than usual, but not high enough to cause diabetes. Full Text
