Question: I was speaking with another pedorthic shoe store owner that is going through the process of looking to become a Medicare supplier. The issue of requesting a non-participating supplier status versus a participating status came up. He was under the impression that being a participating supplier had significant advantages over a non-participating supplier. He made a reference to fewer audits of participating providers. When I search the database of Medicare suppliers in our state, I find very few that have elected to be participating providers. Does anyone have a definitive answer about "fewer audits" or any other real advantage to requesting participating status?

Question: Among the brands we carry now at our pedorthic shoe store, we are missing a cordovan men's dress shoe style in narrow, medium, wide, and XW that comes with the cordovan leather upper that has a shiny finish. Men who travel for the job are asking for this color/style to wear with either gray suits or blue suits so they can take just one pair of shoes on their business trips. PW Minor has a cordovan shoe in their offering but the style and finish are not what business men are looking for in this shoe. Aetrex has a brown shoe in its Lexington collection that has a slight reddish tone but it is not a true cordovan. Obviously we want a shoe that has firm heel counters and torsional rigidity to provide proper support. Drew does not offer this color in a dress shoe style. Any suggestions?

PREVIOUS QUESTIONS AND COMMENTS WITH RESPONSES
Comment: In the latest edition of Current Pedorthics, there is a good article about pedorthists being suppliers SCFOs. I particularly liked seeing the photo of the person's leg that referenced the exact 'scope of practice' anatomy for pedorthists according to ABC's rules. Obviously, BOC's scope of practice for its pedorthists allows for the supply of full length AFOs. The SCFO designation for AFOs that fall within the scope of practice of ABC pedorthists seems designed to get around the political issue of not threatening ABC's large dues-paying membership of orthotists. As a BOCPD, my scope of practice was never limited to SMOs or SCFOs. Thanks to PFA for putting this clinical info into Current Pedorthics and for giving this non-diabetic shoe direction for its pedorthic members to grow their revenues and to service patients needing more than foot orthotics to control gait or deformity conditions. It is pedorthists that do AFOs/SCFOs who can now come out of the closet.

Response: Not sure where you a located but unfortunately the BOC scope of practice is not helping you at all if you live /work in a state that has O&P licensing, does not recognize Certified Pedorthists and uses the ABC Scope of Practice as a guideline. On top of this, it states in the O&P Act that the Certified Pedorthists are exempt, but in the real world he still has to follow the Licensing Regulation and its Scope of Practice. Please see the excerpt below:
(1) "Board" means the Texas Board of Orthotics and Prosthetics
(12) "Orthosis" means a custom-fabricated or custom-fitted medical device designed to provide for the support, alignment, prevention, or correction of a neuromuscular or musculoskeletal disease, injury, or deformity. The term does not include a fabric or elastic support, corset, arch support, low-temperature plastic splint, truss, elastic hose, cane, crutch, soft cervical collar, orthosis for diagnostic or evaluation purposes, dental appliance, or any other similar device carried in stock and sold by a drugstore, department store, or corset shop.
Sec. 605.304. EXEMPTION FOR PEDORTHISTS. (a) In this section:
(1) "Certified pedorthist" means a person certified by the Board for Certification in Pedorthics in the design, manufacture, fit, and modification of shoes and related foot orthoses below the anatomical ankle joint as prescribed by a licensed doctor of medicine or a doctor of podiatry for the amelioration of a painful or disabling condition of the foot; and
(2) "Foot orthosis" includes prosthetic toe fillers or orthoses for use below the ankle.
(b) This chapter does not apply to a certified pedorthist.

PREVIOUS QUESTIONS WITHOUT RESPONSES
Question: I've asked this question in the past and I still can't seem to get a satisfactory answer. Some of the major running shoe companies have, for some reason decided to make neutral cushion shoes that actually function as pronation control shoes. My idea of a neutral cushion shoe is that it should be neutral and provide more cushion via lower durometer materials. What I am seeing is shoes that are thicker soled on the medial v lateral side. The most puzzling part is they are using very soft durometer material on the lateral side, especially in the crash pad portion of the shoe.
As any buyers guide will show, a neutral cushion shoe is typically suited for supinators, cavus or under-pronators. As a result, I am seeing many patients that will prematurely crush and break down the shoe and in some cases they will grossly invert in a brand new one right out of the box. This is also occurring in spite of having them in a cavus orthotic with lateral posting and a lowered medial arch. I have had to return/swap several pairs in one brand but I have seen it in some Asics, Nike, Brooks and New Balance models over the past 1.5 years or so.
My only guess is that some testing or theory I am not aware of, dictates making the crash pad and lateral border much softer to attenuate shock. However, doing so in an underpronator/cavus person, only encourages inversion and supination.....precisely what we are trying to mitigate. I also realize, in true running scenarios, there is less time spent in pure heel contact before it transfer to forefoot loading. That said, I would imagine that the large majority of running shoes are not actually used for any significant running.
It's not only low durometer EVA. I've also seen this issue in some shoes that utilize pods made from visco-elastic polymer type gel material that is not robust enough to control inversion. I would hope this trend is corrected soon because it makes absolutely zero bio mechanical sense to market these as neutral shoes

It's no secret that people of the 1930s and 1940s had an unhealthy fascination with radiation. People put radium in health tonics, food, and toothpaste. They boiled themselves under UV lamps. Seniors Take to Metronome to add Stability to Gait
Marjorie Treadwell is fighting the odds. The Pelican Bay resident knows elders are prone to falls, which can often snowball into serious health problems. Control Of Diabetes Linked To Frequency Of Doctor Visits

According to a study by investigators at Brigham and Women's Hospital (BWH) published in the September 26, 2011, issue of Archives of Internal Medicine, high blood glucose, blood pressure and cholesterol levels increase the risk for complications related to diabetes, such as heart attack and stroke, and in order to reduce these risks these values should be reduced to ideal levels. In total only 27 per cent of adults questioned met the exercise guidelines, with women 10 percentage points more likely than men to keep fit.
