NEW QUESTIONS Question: In the latest edition of Current Pedorthics that I just received, there was an informative article about the changeover in educational requirements to become a certified pedorthist. In the article, it referenced that some of the current 120 hour programs would be affiliating with colleges or universities, some programs would be leaving the market, and one program planned to continue as is without intending to affiliate with a college or university. Specific programs were not mentioned as to who was doing what except for Northwestern's program and one other that escapes me at the moment. Would someone in the know please elaborate on what each program plans to do so business owners can start planning on where to send or look for employees that have an interest in becoming a pedorthist? And feedback on the program that plans to stay active but not be affiliated, it would be interesting to hear what is the strategy there to see if it remains a viable option to businesses looking for new pedorthists. Also, there was the question raised and somewhat addressed about a two year gap occurring that there may not be any "new" CPeds until people work their way through an associate degree program. Does that mean a potential shortage and potential increase in income for pedorthists as businesses compete for pedorthists for several years? -John Mishasek, BOCPD, C.Ped.
PREVIOUS QUESTIONS WITH RESPONSES Question: What is the best casting technique for an 8-year-old patient with moderate, flexible flatfoot deformity (pes valgoplanus), bilaterally? Patient has had UCBL-type devices since age 3 with rectus casting technique (as described by OSI Lab). At this age (or any in age, for that matter ) is the Root technique (STJ neutral, MTJ locked, etc.) preferred? Any other recommendations? Thank you.

Response: We use both methods depending on height of arch, level of deformity and flexibility. We like the Root method for maximum control. We use foam when motion control is more easily achieved with a deep heel cup and posting. We like high top shoes for additional support.

Response: I would cast him with an off-weightbearing plaster cast. Plantarflex the 1st ray and make an extended cut out for the 1st in the orthotic. A medial buttress is essential. Another option is a graphite medial upright SMO.

Response: Everything I have read about the Root technique of casting is either positive or negative, we have had reasonably good results using this technique.Having said that, I have found that when positioning the calcaneous it may not be just moving the calcaneous medially or lateralley but also dorsiflexing the calcaneous and toes simultaneously. Since most hyper-mobile flat feet do not necessarily evert at the longitudinal arch, dorsiflexing the calcaneous starts the upward curve of the orthotics arch under the calcaneous` plantar tuberosities keeping the calcaneous in a dorsiflexed position. Since most orthoses arch starts ahead of the distal calcaneal tuberosities there is more plantar pressure on the arch which many patients can`t tolerate, by dorsiflexing the calcaneous the arch pressure may be more tolerable. Also pay attention to the plantar surface of the metatarsals when dorsiflexing the toes, if the first ray is in a varus position a varus forefoot wedge extending under the first and second metatarsals may also reduce arch pressure. This technique does not always work due to the individual patients tolerance.

New Orthopedic Device Helps Wounded Warriors Run

A new energy-storing orthopedic device, along with an extensive rehabilitation program, is allowing wounded warriors who previously had difficulties walking or standing due to lower leg injuries to run again. The Intrepid Dynamic Exoskeletal Orthosis, or IDEO, was designed by prosthetist Ryan Blanck at the Center for the Intrepid. It is a custom-fit device made from carbon and fiberglass that supports the foot and ankle and resembles an amputee's running prosthetic. The ability to run can be the difference whether a wounded warriors is allowed stay on active duty and to return to their units after having been considered for a medical discharge from the military. Mercedes Gives British Teen a Hand: A Branded Prosthetic One

Mercedes-Benz has found yet another advertising medium in an already fragmented landscape: branded prosthetics! It wasn't the automaker's idea, the Telegraph reports, but that of Matthew James, a clever 14-year-old who was born without a left hand-and invited Mercedes to fit him with a high-tech artificial one if he promised to slap its logo on it. Likely spurred by some mix of genuine benevolence and the PR possibilities, the brand jumped at the offer, paying roughly $57,000 for the prosthesis. James, who wasn't enamored of his last artificial arm, is, of course, delighted. ''It is going to make such a big difference to my life," he tells the Telegraph. Full Text

| From the Flight Deck: Diabetics, Watch your Insulin

Changes in cabin pressure during flights may cause insulin pumps to deliver too much or too little of the medication -- possibly putting sensitive diabetics at risk, researchers report. They recommend disconnecting the pump before take-off and after landing and making sure there are no air bubbles in the insulin before reconnecting it. But an outside researcher said the concern might only apply to some diabetic patients. Full Text

Primary Care Outpatient Protocol Reduces Acute Hyperglycemia

An outpatient protocol allowed for successful management of acute hyperglycemia in 20 of 27 patients who presented in a primary care setting with blood sugar levels 400 mg/dL or higher. Inpatient protocols for managing diabetic ketoacidosis (DKA) and hyperglycemic hyperosmolar syndrome (HHS) are well defined, with a strong evidence base, yet no such protocols exist for managing acute hyperglycemia in outpatient settings. Some data suggest that type 1 diabetes patients with mild DKA can be managed with subcutaneous rapid-acting insulin analogs in an outpatient setting (Diabetes Care 2004;27:1873-8), but it is currently unknown if symptomatic, mild hyperglycemia in type 2 diabetes patients can be managed adequately in such settings, said certified diabetes educator Becky Armor, PharmD., of the University of Oklahoma, Oklahoma City. As she recovered from a hysterectomy last year, Dee Ogden was eager to resume running but nervous about the post-surgical impact on her running form. So she did something that surprisingly few runners do: She hired a coach. "When you learn tennis, you learn about your stroke and form," says Ms. Ogden, 44, a registered nurse in Victoria. "Why wouldn't you do that with running?" The coach she hired was Marilyn Arsenault, a distance runner who has represented Canada in international championships and is one of the top masters (over-40) runners in North America - an obvious choice, since Ms. Ogden had seen Ms. Arsenault's "fluid and beautiful form" en route to victory in road races around Victoria.
