NEW QUESTIONS AND COMMENTS What is the best casting technique for an 8-year-old patient with moderate, flexible flatfoot deformity (pes valgoplanus), bilaterally? Patient has had UCBL-type devices since age 3 with rectus casting technique (as described by OSI Lab). At this age (or any in age, for that matter ) is the Root technique (STJ neutral, MTJ locked, etc.) preferred? Any other recommendations? Thank you. -Jeff Watson, DPM PREVIOUS QUESTIONS WITH RESPONSES Question: In our state, the pool of good certified pedorthists that are not already gainfully employed is nonexistent. Which makes sense. If you are good at what you do, you have a job or have your own business. I recently spoke with a very experienced CPed that works for a large O&P office in our area, and he said that the young, new CPeds that are being hired there do not have any real patient care experience to make good decisions about how to service a challenging foot condition properly so he is spending his day returning calls to angry physicians and trying to make excuses for why their patient was not serviced properly and to ask physicians to give them another shot at taking care of that patient. This comment of his reinforced my feeling that it hard to find "good" pedorthists to hire. Our store has an opportunity in front of it that would require the addition of 3-4 more pedorthists to service a new large account. Between alcoholism, drug use, poor work ethics, over-inflated egos, ex-spouses throwing employee in jail for back child support, Monday flu, the dog ate my car keys, and other mindboggling issues, we have had only one pedorthist out of many that worked out long term for our business. Are other pedorthic business owners finding a similar challenge in finding quality pedorthists to employee and having to grow their own CPeds too? If so, what type of individual are you hiring that would later want to become a pedorthist and that you would want to invest in helping to get certified? Or is it the basic problem that the owner of any type of business that I talk to about this issue is facing now? There are a lot of people out of work and there may be a good reason that they are. -John Mishasek, BOCPD, C.Ped. Response: I have found that the majority of companies out there that are in need of pedorthists tend to spend about 6 weeks looking for someone certified before they finally give up and train an existing employee or someone that has expressed interest. In my opinion, the best candidates are existing employees who may not currently be doing patient care. It is easier to replace the gal up front or the kid in back than it is trying to find a trained pedorthist. When looking outside, the key is to find people with parallel skills. Anyone working in patient care (past or present) can easily make the transition. I know more engineers (myself included) that have found pedorthics as a compatible second career. The biggest hurdle seems to be the ability to understand the verbiage involved with pedorthic practice. Verbal skills are essential to patient care and communicating with referral sources. I get 2-3 calls a week for people looking for pedorthists. All I can say is if you are going to get them trained, now is the time. -Rick E. Sevier - BS Ed. BOCO C. Ped Response: Good people are hard to find, not just good pedorthists. We look for the best personal qualifications, rather than pedorthic qualifications and train from within. We are looking for individuals who like people, are patient and good listeners and really want to help change people's lives. We look for stability, and career goals in the individuals we hire. Then, we start training them the day they arrive and don't stop until they move on. Few people stay in one job forever so we provide our pedorthists with the tools to succeed and offer competitive benefits to encourage them to stay. We find that all people have one or more of the problems you described. We do our best to encourage them to take more responsibility for their agreements with us. -Robert Schwartz, C.Ped. Response: Finding good help that wants to problem solve, know inventory, have skilled hands, and is for hire is RARE. I've not hired anyone with all of these existing skills. I have trained and made competitors galore. I am grateful when a few of them speak and act respectfully after their departures. Pedorthists tend to be poor business people who are independent. Obviously we are poorly organized and have lost significant ground with insurances. We do the best job with footwear, from lotions to socks to orthoses and shoes, and customers/patients who value their experience are willing to pay. Educating and informing is our long suit so give them good "pedorthic theater" and customers will refer others. So look for quality individuals to train. Attitude is everything. A non-compete might be added in your hiring. It has to be reasonable but 1 year for 20 miles around your locations will generally be reasonable for providing a specialized career. Even in this economy, finding people who really want to work is difficult. Excuses and addictions are rampant, I agree. But I believe there are still good people out there. Pedorthists tend to be forever positive, changing how people move one foot at a time. Hx: Distal Hallux amputation due to crush injury at age 18. Pain persistent @ peroneus brevis and peroneus longus with E-stim and ultra sound therapies providing relief for only a few hours. Never worn any type of orthoses. Exam: (L ) heel varus 15 degrees, supination, and forefoot varus. Good ankle ROM and flexible metatarsals, callusing at enlarged cuboid only.(R) slight supination Temporarily we modified his slip-on loafers with lateral heel straights which provided some immediate relief. What type of orthoses would you fabricate? Any shoe modifications needed? Footwear suggestions? -Jesse J Fischer, Comfort Shoe Specialists Response: Since he has good ankle range of motion provide lateral posting to evert his foot to neutral. Provide dispersion relief under the callus. Add lateral flares to his shoes so that the widest part of the shoe supports and stabilizes his lateral malleolus. Stress and Appetite Link Explained by Calgary Research
Researchers at the University of Calgary's faculty of medicine think they have solved the riddle of why some people turn to food in stressful times. Stress prompts nerve cells in the brain to transmit more urgent hunger signals - a process that probably dates back to humankind's earliest days, when finding food was a key source of stress. The hypothalamus regulates appetite and metabolism, as well as being a primary region of the brain responsible for the responses to stress. (CBC) The scientists at the Hotchkiss Brain Institute made the discovery by inducing stress in rats and observing their increased drive for food. Published online this week in the journal Neuron, the discovery might provide important insight into why stress is thought to be one of the underlying contributors to over-eating and obesity, says Quentin Pittman, one of the researchers. Red Bluff Prosthetic Expert Helps Haiti Quake Victims

June 12 was a day that changed Mark O'Sullivan's life. That day, he arrived in Haiti with a team from Hanger Prosthetics and Orthotics as a part of the 36th rotation arriving to serve in the Hanger Clinic there. Located in Deschapelles, Haiti, 60 miles from Port-au-Prince, this clinic provides aid for victims of the devastating January 2010 earthquake. It opened in February 2010 and has provided service to nearly 1,000 Haitians. O'Sullivan, a six-year employee of Hanger Prosthetics and a Red Bluff resident, has been a prostheticist at their offices in Redding and Red Bluff. Full Text

New Running Store Promotes the Bare Minimum

As a reformed orthotics-maker, Kyle Roberts pitches a strong argument for the benefits of the minimalist running shoes he sells at his new store in Wauwatosa. It was ego, though, that persuaded me to pull out a credit card and add a pair of lightweight, low-heel Inov-8 flats to my shoe collection. The decisive moment came on a training run along the Oak Leaf Trail in Bay View last month, when I spotted a guy running, not on the path, but on the narrow concrete wall alongside of it. | Red Meat Consumption may Increase risk of Diabetes

Fat 'disrupts sugar sensors causing type 2 diabetes'

US researchers say they have identified how a high-fat diet can trigger type 2 diabetes, in experiments on mice and human tissue. Writing in the journal Nature Medicine, they say that fat interferes with the body's sugar sensors. The authors argue that a deeper understanding of the processes involved could help them develop a cure. Diabetes UK said the study was interesting and a "theory worth investigating further". One of the main risk factors for type 2 diabetes is being overweight - rising obesity levels have contributed to a doubling of diabetes cases in the last 30 years. Full Text

Need for Diabetes Educators Likely to Rise

The need for diabetes educators will rise sharply between now and 2025 as the diabetes epidemic worsens, according to a new study by the American Association of Diabetes Educators. The study, conducted by Dobson DaVanzo & Associates, found that the need would increase by at least 60% over the next 14 years, with new career opportunities likely to emerge as private insurers and Medicare recognize the role the educators play. As this happens, the range of work settings for diabetes educators will expand and include retail clinics, home health and long-term care centers and others in addition to hospital outpatient and physician office positions.
