Question: In our state, the pool of good certified pedorthists that are not already gainfully employed is nonexistent. Which makes sense.  If you are good at what you do, you have a job or have your own business. I recently spoke with a very experienced CPed that works for a large O&P office in our area, and he said that the young, new CPeds that are being hired there do not have any real patient care experience to make good decisions about how to service a challenging foot condition properly so he is spending his day returning calls to angry physicians and trying to make excuses for why their patient was not serviced properly and to ask physician to give them another shot at taking care of that patient.  This comment of his reinforced my feeling that it hard to find "good" pedorthists to hire.
 
Our store has an opportunity in front of it that would require the addition of 3-4 more pedorthists to service a new large account.  Between alcoholism, drug use, poor work ethics, over-inflated egos, ex-spouses throwing employee in jail for back child support, Monday flu, the dog ate my car keys, and other mindboggling issues, we have had only one pedorthist out of many that worked out long term for our business.  Are other pedorthic business owners finding a similar challenge in finding quality pedorthists to employee and having to grow their own CPeds too?  If so, what type of individual are you hiring that would later want to become a pedorthist and that you would want to invest in helping to get certified?  Or is it the basic problem that the owner of any type of business that I talk to about this issue is facing now?  There are a lot of people out of work and there may be a good reason that they are.

Question: We have a 48yo male with Rx: Evaluate and fit with custom foot orthoses.
Hx:  Distal Hallux amputation due to crush injury at age 18.  Pain persistent @ peroneus brevis and peroneus longus  with E-stim and ultra sound therapies providing relief for only a few hours.  Never worn any type of orthoses.
Exam:  (L ) heel varus 15 degrees, supination, and forefoot varus.  Good ankle ROM and flexible metatarsals, callusing at enlarged cuboid only.(R) slight supination
Temporarily we modified his slip-on loafers with lateral heel straights which provided some immediate relief. What type of orthoses would you fabricate?  Any shoe modifications needed?  Footwear suggestions?

Question: I was wondering how many C. Peds accept insurances other than Medicare. Where can I find information on how to sign up with insurances? How difficult is it to become a provider. and what is the biggest hurdle?

Response: Depending on the types of insurance plans (HMO, PPO, MCO, PIP, workers comp, major medical, etc) that are sold in your area, I would think that calling the plan and asking for Provider relations or Provider contracting department would be the starting point.  Our experience is the larger the insurance plan, i.e. Kaiser Permamente, is in an area, the more likely they are to contract with large, national companies and exclude everyone else.  We went after a provider contract with a Medicare HMO, PacifiCare (now owned by United Healthcare) and lost money servicing their patients.  The fees in the contract were actually higher than Medicare allowables but the claims would be denied for lack of prior authorization even though prior authorization was faxed to us, claims would be underpaid (pay for inserts but not shoes, pay for shoes but not inserts, even though shoes and inserts were on prior auth for delivery), and delays in payments for months on end (claims staff would say that they never got claim even though we had faxed it in with other claims that did get paid (or underpaid).  If one is going to bill insurances, you almost surely need a dedicated person in charge of submitting claims, following up on why claim was not paid/was not paid correctly, etc.  Factor in the cost of this person being dedicated to this process and ask yourself, is this book of business really profitable?  You almost have to budget 25% of fee schedule payment to cover personnel costs of getting paid.  If the allowables are low and 25% is going to cost of collecting or become bad debt, can you really make a profit billing insurance plans.  Doubtful.  Better to have customer self pay and give copy of Rx and tell to submit copy of Rx and copy of your retail receipt to try to get reimbursed directly.

Response: We do not accept insurance and don't plan to. The administrative, profitability and cash flow challenges are not worth it for us.

Question: I make what we call traditional foot orthoses, no computers, hands on. Recently our distributor of EFM has changed to a product they call FM. They said EFM was no longer available. I tried using the FM and found it not to mold like the EFM. It is also not as strong or as dense. Any suggestions?

Response: Ask the distributor what the difference is in the compound properties of EFM versus EM. Once you know the properties of each you can research to find out what's available in the marketplace.

Question: I am trying to help another shoe store become a new Medicare TSD supplier. One of the issues that is puzzling to me is that the legislated TSD benefit is for up to 3 inserts per shoe per year yet a L5000 partial foot filler actually acts as a replacement for an insert for reimbursement purposes but is composed of the same soft material under the residual limb as a diabetic insert.  One would think that 3 each partial foot fillers could be supplied per year since they are partially a prosthesis and partially a diabetic insert.  But I believe this is not the case.  I believe I have heard that Medicare will reimburse for only one partial foot filler per year.  So how do practitioners reconcile that the material under the residual at-risk limb can pack down and become ineffective for protecting the remaining foot tissue but because it is part of a L5000 partial foot filler the patient is limited to just 1 L5000 per year?  That does not make sense clinically.  Do practitioners pay labs for 3 each partial foot fillers and bill Medicare for ony one?  Or bill the beneficiary using an ABN for one or two more L5000/inserts as excess services rendered. That does not make sense financially.  How are other practitioners handling the supply of partial foot fillers in a way that makes sense clinically and financially?

Response: The L5000 is part of the Local Coverage Determination for Lower Limb Prostheses, a coverage benefit separate from the TSD Local Coverage Determination with different allowances and thus doesn't allow crosswalking between A and L codes.  So, while the TSD allows for up to six units of diabetic inserts per year based on medical necessity, the L5000 is dispensed as needed, and replaced on order from the treating physician when there is change in the physiological condition of the patient; or, there is irreparable wear of the device or a part of the device; or, when the condition of the device, or part of the device, requires repairs and the cost of such repairs would be more than 60% of the cost of a replacement device, or of the part being replaced.  However, under no circumstances should more than one unit per day per side be billed for the same date of service.

Orthotics Expert Started Learning About Them at an Early Age

Henry Orthotics in Comox has recently changed its name to SOS Orthotics, legal name Strathcona Ortho Solutions Ltd. Eugene Henry, whose name has been synonymous with orthotics in Campbell River and the Comox Valley, is transitioning into retirement and conducting business solely out of his Campbell River office. Affordable Footwear Act Introduced in the House 

 

Congresswoman Lynn Jenkins (R-KS), Congressman Joe Crowley (D-NY), Congressman Kevin Brady (R-TX), Chairman of the Trade Subcommittee, and Congressman Earl Blumenauer (D-OR) have introduced H.R. 2697, the Affordable Footwear Act into the House of Representatives.

If passed, the Affordable Footwear Act (AFA) would eliminate approximately $800 million in duties on a variety of footwear types, including lower-priced children's, outdoor and performance footwear. Full Text

Ski/Snowboard Bootfit & Stance Alignment Focus of Unique Continuing Ed Course for Pedorthic Professionals
BRIARCLIFF MANOR, NY-Masterfit University, the world's only center for the study of snowsports boot fitting, offers pedorthists, orthotists and other allied fit professionals a unique opportunity to learn the skills of fitting and modifying "the most challenging footwear to fit on the planet." All while earning a significant number of continuing education credits. 
Designed and taught by a group of Certified Pedorthists with over 225 years of experience fitting snowsports boots, Masterfit U is a fast-paced, hands-on experience with focus on biomechanics and alignment, orthotics fabrication and boot fit modification techniques. "Our less talk, more action and 'Learn It, See It, Do It' format is fun and effectively simulates fitting and back shop environments," says MFU curriculum director Mark Elling, C. Ped.
Attendees practice their skills in Masterfit's traveling boot labs that are outfitted with the most advanced fit analysis and modification tools and direct mold orthotic casting and grinding equipment.
