NEW QUESTIONS
Question: I am trying to help another shoe store become a new Medicare TSD supplier. One of the issues that is puzzling to me is that the legislated TSD benefit is for up to 3 inserts per shoe per year yet a L5000 partial foot filler actually acts as a replacement for an insert for reimbursement purposes but is composed of the same soft material under the residual limb as a diabetic insert. One would think that 3 each partial foot fillers could be supplied per year since they are partially a prosthesis and partially a diabetic insert. But I believe this is not the case. I believe I have heard that Medicare will reimburse for only one partial foot filler per year. So how do practitioners reconcile that the material under the residual at-risk limb can pack down and become ineffective for protecting the remaining foot tissue but because it is part of a L5000 partial foot filler the patient is limited to just 1 L5000 per year? That does not make sense clinically. Do practitioners pay labs for 3 each partial foot fillers and bill Medicare for ony one? Or bill the beneficiary using an ABN for one or two more L5000/inserts as excess services rendered. That does not make sense financially. How are other practitioners handling the supply of partial foot fillers in a way that makes sense clinically and financially? John Mishasek, BOCPD, CPed
Question: I feel that there is some gray area on the matter of billing Medicare for custom diabetic shoes and inserts. This is a rare case, but there are times when a patient is evaluated, cast for and the shoes/inserts are completed; however, the patient fails to return to pick up his/her shoes. It is my understanding that when a patient becomes deceased (God forbid) or has substantial changes in his/her feet, that the provider can bill for the completed item. Obviously for custom shoes, they cannot be returned or used for another individual.
To reiterate the question, it is Medicare's requirement that the provider bill for the goods once the goods are delivered and a delivery slip has been signed, but what about when the goods are completed and the patient fails to return for the goods? It's unfortunate, but there are "flaky" patients out there.

PREVIOUS QUESTIONS AND COMMENTS WITHOUT RESPONSES Question: Has anyone out there found any Pedorthic related apps for the IPad that they would be willing to share experiences with? I use several to teach. IMed Sliders is a great program, as well as an app I use that is a bubble level for LLD/pelvic obliquity assessment. There are lots of terminology and anatomy apps as well. Do I use my IPad? I am sitting on the beach in Florida writing this on my home laptop in Tulsa, using gotomypc right now. Great technology! Utilize it!

Question: I make what we call traditional foot orthoses, no computers, hands on. Recently our distributor of EFM has changed to a product they call FM. They said EFM was no longer available. I tried using the FM and found it not to mold like the EFM. It is also not as strong or as dense. Any suggestions?

Comment: A month or so ago I had put out a posting on PNW to get help on locating sponge rubber neuroma pads. After much searching, I have found a source. Vern Bintz has in their new catalog on page 22 a listing for Metatarsal Pads - Sponge Rubber. The photo shows a standard oval met pad but under the photo are listed not only product numbers for the oval met pads but also for Right and Left met pads, which in reality are "neuroma" met pads with a triangular shape to separate the metatarsal shafts. There's the trademarked "Cinderella Procedure"-a preventive bunion correction that makes feet narrower. The clinic also offers the "Perfect 10! Aesthetic Toe Shortening" that invisibly trims toes that hang over the end of sandals or have to be crushed into tight shoes. Gov. Rick Perry Ditches Cowboy Boots for Orthopedic Shoes
Gov. Rick Perry has ditched his trademark cowboy boots for a more comfortable pair of orthopedic-style black tennis shoes with thick soles.
This isn't a fashion alert - the governor underwent spinal fusion surgery to fix a recurring back ailment earlier this month and is still recuperating, according to the New York Times. Full Text

New Balance Struggles to Keep U.S. Shoe Factories Going

At the factory here owned by New Balance, the last major athletic-shoe brand to make footwear in the United States, even the workers recognize the operation doesn't make sense in purely economic terms.
The company could make far more money if, like Nike and Adidas, it shifted virtually all these jobs to low-wage countries.
So employees try each shift to make it up. Conversations on the shop floor are sparse at best, and the tasks at each work station have been stripped of waste and precisely timed.
Workers cut leather for a pair of shoes in 88 seconds, handle precise stitching in 37 seconds, and glue soles to uppers even faster. Full Text

| Put A Foot Forward for Diabetes
Complications related to diabetes are a real problem in Australia with 65 foot amputations related to diabetes occurring every week.
To raise awareness for National Diabetes Week (10-16th July 2011), the WA Country Health Service Pilbara Population Health unit and the Hedland Health Campus Dialysis unit joined together for 'Care for your Feet Day' on Thursday, June 30.
The focus was on dialysis patients who are twice as likely to develop foot ulcers or undergo a lower extremity amputation than those with minimal to no evidence of kidney disease. Full Text

Menopause has no Impact on Diabetes
Menopause has little or no impact on whether women become more at risk for diabetes, researchers at the University of Michigan Health System say.
Lead author Dr. Catherine Kim -- an associate professor of internal medicine and obstetrics and gynecology at the University of Michigan Health System -- says post-menopausal women had no higher risk for diabetes whether they experienced natural menopause or had their ovaries removed.
Kim used data from the national clinical trial of 1,237 women ages 40-65 at high risk for diabetes. Full Text

Your Mobile Can Help you Manage Diabetes
A new study by University of Maryland School of Medicine researchers has indicated that an interactive computer software program appears to be effective in helping patients manage their Type 2 diabetes using their mobile phones.
The study, one of the first to scientifically examine mobile health technology, found that a key measure of blood sugar control - the amount of hemoglobin A1c in a person's blood - was lowered by an average of 1.9 per cent over a period of one year in patients using the mobile health software. The findings support the further exploration of mobile health approaches to manage many chronic conditions, including diabetes. Full Text

Heralded as the secret to tightening your buns, thighs and abs, the shoes - equipped with uneven soles designed to put muscles into overdrive - flew off the shelves last year as Americans, by one estimate, scooped up nearly 10 million pairs amid the latest fad in the finicky footwear industry.
One year later, the shoes have fallen in popularity in a now-flooded market for the workout shoes and companies are focusing more on lighter-weight shoes. Meanwhile, studies and lawsuits have repeatedly questioned whether toning shoes may really whip a flabby body into shape. Full Text
