NEW QUESTIONS AND COMMENTS Question: Has anyone out there found any Pedorthic related apps for the IPad that they would be willing to share experiences with? I use several to teach. IMed Sliders is a great program, as well as an app I use that is a bubble level for LLD/pelvic obliquity assessment. There are lots of terminology and anatomy apps as well. Do I use my IPad? I am sitting on the beach in Florida writing this on my home laptop in Tulsa, using gotomypc right now. Great technology! Utilize it!

Question: I make what we call traditional foot orthoses, no computers, hands on. Recently our distributor of EFM has changed to a product they call FM. They said EFM was no longer available. I tried using the FM and found it not to mold like the EFM. It is also not as strong or as dense. Any suggestions?

Comment: A month or so ago I had put out a posting on PNW to get help on locating sponge rubber neuroma pads. After much searching, I have found a source. Vern Bintz has in their new catalog on page 22 a listing for Metatarsal Pads - Sponge Rubber. The photo shows a standard oval met pad but under the photo are listed not only product numbers for the oval met pads but also for Right and Left met pads, which in reality are "neuroma" met pads with a triangular shape to separate the metatarsal shafts. - John Mishasek, BOCPD, C. PREVIOUS QUESTIONS AND COMMENTS WITH RESPONSES Question: I am interested in the PWN's readership's thoughts on physical therapists providing foot orthoses - both OTC and custom. In my area, I am starting to see this trend taking off. Right now, it seems to be mainly OTC, but I know of a few groups who are molding, outsourcing the actual fabrication and then dispensing the custom devices. Can they really bill for these? I want to maintain good, symbiotic relationships with the PTs in my area, but I could see this encroaching into my territory more and more as time goes on. - Erick Janisse, C. Ped., CO
Response: At the risk of sounding somewhat negative on this issue, I am replying to Erik Janisee's request for response regarding P.T.'s supplying custom and OTC orthotics within what they consider to be their domain of practice:
1. PTs, in my experience, have little to no doubt that, because of their education, training, etc. they are well-qualified to dispense these products. And it doesn't stop there. Every few years, you will see them pushing for legislation and/or policy that would would allow them to prescribe such products as well.
2. It is my opinion that, at best, the argument that the general public is not well-served in pedorthics by the combined expertise of physicians (including podiatrists), C. Peds and orthotists is weak. In some isolated, rural areas this might be the case, but to argue that PTs are needed in order to provide access to care is akin to a rural area pharmacist (why do I always pick on them?) arguing that he/she be allowed to perform surgery because there is no medically-qualified surgeon nearby. 3. PTs have the same opportunity as anybody else to obtain C. Ped. credentials. A PT/C. Ped. combo certification/license) is a great professional combo.
4. Our profession/trade associations have done an ineffective job in past years of preventing territorial encroachment by the untrained and unqaulified. We could (and should) have done better. It's not too late, by the way. PFA should lead the charge in this area, in my humble opinion. I've repeatedly expressed this opinion to anyone who would listen over the years.
5. It seems to me that our concern on this issue would be shared by our podiatrist friends. I'm looking forward to hearing from them on Erik's post.

Response: All healthcare professionals who deal with feet are involved with foot orthoses, as providers or prescribers. The quality of foot orthoses dispensed is in direct proportion to their understanding of footwear, biomechanics and gait. There is overlap between what we do and what others do. The best solution is to market to physical therapists as providers of footwear, shoe modifications and foot orthoses, ready made and custom made. We have to realize that we are not entitled to their referrals and are happy when they send us any of their patients. Our best marketing is word of mouth advertising by satisfied customers, prescribers and consumers alike.

Response: Physical Therapists, Chiropractors, Orthotists, Pharmacists, Doctors and Athletic Trainers have been doing this for years. This is not a new phenomenon. The terms "encroaching" and "territory" indicate an attitude of ownership or right to some geographical market area. In today's market culture, the individual who provides the best product/service in a timely, easily accessible and cost effective fashion gets the consumer's money. The days of "territory" have gone with the advent of access to transportation and willingness to travel for a better product/greater value. Thus, to compete with these other providers, Pedorthists must be proactive rather than reactive. Better access, processes, materials, products, customer service, technical infrastructure, marketing and consumer value are the keys to not just staying afloat, but overall growth. Those who have been riding the wave for years will quickly find themselves far behind those who are furiously, and consistently paddling their boat to stay ahead of it.

Response: Physical therapists in my area that do not cast for, or dispense, custom feet orthoses or OTC products are a rarity. The super majority of them are very educated on the matter and have a much more thorough understanding of biomechanics, gait and orthoses' overall effect on the human body than most C. Peds I know. Their training gives them insight into topics that we as C. Peds are not even exposed to. At the least, they have a four year degree, and most continue to a higher level of education towards a master's. As PTs, they have many biomechanical courses that they can attend. I have attended a few. They are great courses. In fact many C. Peds would have a hard time following the course content.
Are they competition? Absolutely! With the changes in health care on the horizon, most people are scrambling towards any income stream they can get their hands on. Most PTs my company works with are 'phasing out' of the insurance networks as they do not reimburse well. What I have found opens the door to this market is giving them an in-service and educating them on what we as C. Peds can do and why we do it. The most difficult part of this task is getting past the 'Iron Curtain' called the front desk. Do your research and determine if they are dispensing custom orthoses. If they are, they might see you as competition and not meet with you. On the other hand, they might be open to what you have to offer. We as C. Peds must continue to educate ourselves and strive to be the experts. Most of the patients referred to my company by PTs are tough cases, but at least it's a referral and referrals mean income! So to summarize, get used to competing for your market share not just with PTs, but chiropractors, athletic trainers, non pedorthic shoe stores and even some orthopedic practices. Good luck!

Query: Pain in Peroneus Brevis Tendon
I have a healthy 74 year old female patient with chronic pain at the peroneus brevis tendon insertion at the 5th met base from a prior inversion injury. Her pain is with direct palpation and weight-bearing. X-rays are unremarkable. I treated her conservatively over the past six months with NSAIDs, PT, Bracing, orthotics, and immobilization with a CAM walker, and even cast her.
A prior MRI displayed a small insertional tear. Following this, we cast her for several weeks, then placed her in a CAM walker for several more weeks. She returned to normal shoe wear for a brief period but the pain returned, I repeated the MRI for a surgical evaluation for tendon repair, and the MRI showed her having a healed tendon compared to the prior MRI. Am I correct in assuming that there probably is still an insertional tear? Any recommendations as to repair would be appreciated.

Response: From Robert S. Schwartz, C. Ped.: An important clue is "She returned to normal shoe wear for a brief period but the pain returned." Many shoes are designed so that the base of fifth is pressing against the lateral aspect of the shoe where the shoe is narrower and the seams, welts and other reinforcements join. This increases shear and pressure to base of fifth and styloid process. Inversion forces are exacerbated when the foot extends lateral of the base of the shoe. The lateral side of the shoe has to be wider to accommodate the deformity. Look for a straight last shoe with a "broad shank" and wide lateral base. Ask your local footwear retailer or pedorthist for specific brands and styles available in the marketplace. A shoe can be customized to broaden the shank on the lateral side both inside (relasting) and outside (lateral flare and offset). In flexible deformities a lateral wedge and flare can be added to the outside of the shoe to relieve pain and improve balance and gait.
Response: I would be curious as to whether the orthotic included a met support to offload the 5th met. Sometimes it is as simple as that.

Response to Discussion in Past Issues on Refunds: All this sounds nice and dandy from a customers' standpoint. I have had people come in with all the reasons someone can come up with why the inserts are not working. There are some great excuses made up from time to time. Then you look at the inserts and they show no wear. You interview the patient a little longer, and there are discrepancies in their story. You find out that they are a little short on cash and need to find a way to get back the $250.00 or more that they had payed for the inserts, that they had not followed your recommondation on how and how long to wear the inserts,etc., etc.
Unfortunately, we live in this Walmart-thinking society were everybody is used to bringing anything back they don't like, need or just don't want anymore after they had it for a while, and get a refund. (Have you ever listened to their excuses?)
Fortunately, we are not Walmart! We are providing a customized, personal service to our customers, made in the USA ! Many of us have a small business where the C. Ped. is the owner, lab-technician and basically a one-man show. We are putting all our time, knowledge, skill and money into making these inserts. Will they always fit and work? No, they won't. Every patient is different (and that's a good thing, it would be very boring if we were all the same). The left foot is different from the right. Patients have to understand this, and also be made aware that there is no guarantee that these inserts will work. We then have the option to modify inserts according to "what does not work" with the present inserts. A refund should not be the first option. Many of us small businesses can't afford it.

Nuts Benefit Type 2 Diabetics
Eating roughly 2 ounces of nuts daily in place of carbohydrates may benefit type 2 diabetics by lowering LDL cholesterol levels and improving blood sugar control, according to a new study published in the journal.
Researchers at St. Michael's Hospital conducted a three-month study involving 117 people with type 2 diabetes who were randomly put into three different groups, one that consumed approximately 2 ounces of mixed nuts each day, one that ate a healthy muffin, while the third group had a half nuts and half muffin diet. The nuts consisted of mixed unsalted almonds, pistachios, pecans, hazelnuts, walnuts, peanuts, cashews, and macadamias. Full Text
