Query: Pain in Peroneus Brevis Tendon

I have a healthy 74 year old female patient with chronic pain at the peroneus brevis tendon insertion at the 5th met base from a prior inversion injury. Her pain is with direct palpation and weight-bearing. X-rays are unremarkable. I treated her conservatively over the past six months with NSAIDs, PT, Bracing, orthotics, and immobilization with a CAM walker, and even cast her.

A prior MRI displayed a small insertional tear. Following this, we cast her for several weeks, then placed her in a CAM walker for several more weeks.  She returned to normal shoe wear for a brief period but the pain returned, I repeated the MRI for a surgical evaluation for tendon repair, and the MRI showed her having a healed tendon compared to the prior MRI.  Am I correct in assuming that there probably is still an insertional tear? Any recommendations as to repair would be appreciated.

Response: From Robert S. Schwartz, C. Ped.: An important clue is "She returned to normal shoe wear for a brief period but the pain returned." Many shoes are designed so that the base of fifth is pressing against the lateral aspect of the shoe where the shoe is narrower and the seams, welts and other reinforcements join. This increases shear and pressure to base of fifth and styloid process. Inversion forces are exacerbated when the foot extends lateral of the base of the shoe. The lateral side of the shoe has to be wider to accommodate the deformity. Look for a straight last shoe with a "broad shank" and wide lateral base. Ask your local footwear retailer or pedorthist for specific brands and styles available in the marketplace. A shoe can be customized to broaden the shank on the lateral side both inside (relasting) and outside (lateral flare and offset). In flexible deformities a lateral wedge and flare can be added to the outside of the shoe to relieve pain and improve balance and gait.


Question: I am interested in the PNW's readership's thoughts on physical therapists providing foot orthoses - both OTC and custom.  In my area, I am starting to see this trend taking off.  Right now, it seems to be mainly OTC, but I know of a few groups who are molding, outsourcing the actual fabrication and then dispensing the custom devices.  Can they really bill for these?  I want to maintain good, symbiotic relationships with the PTs in my area, but I could see this encroaching into my territory more and more as time goes on.

Chiropractor Hits the Ground Running with Footwear Venture


Over the years of seeing patients at his chiropractic clinic, Dr. Daryl Sherman became passionate about healing a part of the body everyone takes for granted - the feet. Lotus Tissue Repair Inc. has launched as a spinout from Third Rock Ventures, which led the Cambridge biotech's $26 million Series A funding round. The funds will be used to develop the technology it licensed from the University of Southern California into a collagen-based therapy for a rare skin disorder....
 
The company licensed a proprietary recombinant collagen type VII (rC7) technology from USC, which will be used to develop a protein replacement therapy to treat "dystrophic epidermolysis bullosa (DEB), an orphan disease that causes devastating skin blisters, morbidity and early mortality." Beyond treating DEB - for which there is no approved treatment yet - Lotus Tissue says future applications could include helping to accelerate chronic wound healing from conditions such as diabetic foot ulcers, venous stasis ulcers and similar conditions. Why is Michael Vick shilling for Nike?

Michael Vick's rocky road to public redemption goes on. The quarterback, who served nearly two years in prison for his role in a dog fighting ring, has, since signing with the Philadelphia Eagles in 2009, grown into the kind of player that legendary games are made of and become an advocate for the Humane Society. But is America ready to accept him again as a sneaker pitchman? Nike, which dropped Vick's endorsement deal and halted the release of a signature Zoom Vick V shoe shortly after he pleaded guilty to federal felony charges in August 2007, has signed him back up. The deal is reported to be the first time a company has brought back a celebrity it previously dropped.

Though the details of the deal were not disclosed, Nike told CNN in a statement: "Michael acknowledges his past mistakes. We do not condone those actions, but we support the positive changes he has made to better himself off the field." Vick, for his part, says, "I am thrilled to be on the Nike roster." Yet not everyone is quite as psyched. A Facebook campaign to boycott Nike has already been launched, and high-profile opponents are voicing their disgust on Twitter. Full Text
