Question: I was wondering how many C. Peds accept insurances other than Medicare. Where can I find information on how to sign up with insurances? How difficult is it to become a provider. and what is the biggest hurdle ? - CPed2005
Question: Does anyone bill insurance companies and get paid for "computer analysis" or "gait analysis?" I have the tekscan system, which cost me money, and it does take time to run patients through it. Many orthotists do not have this system. Shouldn't we get paid for it? Blue Cross recently lowered our reimbursement for orthotics by 33%. No warning or nothing. And there is nothing we can do if we want to stay in network. At the same time, they raised my health insurance premiums by 30% last year. Any info will be helpful.

Comment: It happens a few times per year: a patient who wants to "return" their orthotics after using them for a given time. I recently had a patient that had the proper script filled based on their diagnosis. We filled the script, and got her into a running shoe. We have a comprehensive break-in instruction sheet that states they can return for adjustments at no charge. We follow that with thorough verbal intructions, as well.
This person walked in without an appointment and wanted to return the FO's because "they didn't work and her physical therapist told her they weren't right." She never excercised the re-check offer, and she never called to ask about adjustments. She based her decision solely on the advice of her therapist (who never gave me a courtesy call). Like a pharmacist, we simply filled a script based on the Doctor's diagnosis.
If someone gets medication from a pharmacy based on their doctor's script, does the patient go back for a refund if it doesn't cure their respiratory infection? If they break out in hives, vomit or their toothache pain doesnt subside, are they entitled to a refund from the pharmacy? I am thinking of adding a disclaimer on our sign-in sheets explaining that we make every effort to ensure success, but we can't offer refunds. Our time money and materials are buried into the price.
I realize it is easy to say "no refunds," but we have all had those special patients who think FO's and shoes should be like returning a knife set at Walmart...no questions asked. Unfortunately, those are the ones who won't take no for an answer, and who will sometimes attempt to create ill-will with the referral source and anyone else who will listen. If anyone has a well working paragraph or two, please share. Having the patient initial it may cut down on this problem. If not, it will at least provide some backup in dealing with the ones who think orthotics are try-before-you-buy products.

Response: It is my personal view that refunds do need to be given at times, even if you have a no refund policy. As stated in the original question, "It happens a few times a year." If it is only a few times, then what's the problem? You also stated, "Like a pharmacist, we simply filled a script based on the Doctor's diagnosis." It is very seldom that a pharmacist has to "custom" make a prescription. Usually all they have to do is put the correct pills into the bottle. Even if you are using an over-the-counter orthotic, you have to make the decision as to what size, what arch height, with or without a metatarsal raise, etc. If you get a RX for "diabetic shoes," you have to pick the correct last, size, width, depth, etc. As pedorthists we have more chances to make an error than a pharmacist does.
The other thing is to make sure you do the math! What I mean by that is, what will it cost you to give a refund vs what will it cost you if don't? To give a refund might cost you a couple of hundred dollars. But by not giving a refund, the decision could cost you thousands! What if this patient is a golfing buddy of the doctor that sent him to you? What if she is his mother-in-law? What if it is just an ordinary patient who goes back to the doctor and says that the C. Ped. you sent me to stuck me for a $300.00 piece of plastic that I can't wear?
We all have had patients come into our facilities and say those words about others. But on the other hand, if that same golfer, mother-in-law or patient goes back to the doctor and says, "I went to the pedorthist for my shoes and orthotics and they didn't work out...BUT...they gave me my money back even after trying to make me feel better. WOW...You have a doctor for life.
I found a C.Ped's website today that is taking this a step further by offering a "Money Back Guarantee." Check it out http://www.comfortshoe.com/.
Then answer this question...If you were a doctor in the Saint Louis area, who would you be sending your patients to? The C. Ped. with a NO REFUND policy, or the company with a YES refund policy? I am assuming that the quality of work and results are equal.

Question: Rather than have patients fill out their information questionnaires when they come to the office, has anyone offered them to download them from your webpage and either email or fax them to your office or bring them with them when they come. Having all of the patients information entered before their visit (including medical history) would seem to offer some significant advantages in time and patient management. Has anyone done this, and if so, how, and what have been the results?

| Experts say Flip-Flops Pose Dangers for Feet, Back

Summer and flip-flops tend to go hand in hand (insert foot-in-foot joke here). However, local foot specialists caution buyers to beware when looking at summer footwear, especially flip-flops.
Foot and ankle specialist Dr. Ritu Mathur has been in the business for 17 years, and as the popularity of flip-flops has grown in recent years, she said, so has the number of patients she sees with foot, leg and back pain.
But flip-flop wearers say sometimes wearing anything is better than nothing.
"They're comfortable," said Andrew Wallor, a Holland vacationer from Chicago.
Laura Karasick of Grand Blanc said she likes wearing flip-flops because they're convenient. "I have back problems, so I usually wear shoes like those," she said, pointing to tennis shoes, "but once in a while I like to wear flip-flops because they're easy on, easy off."
"They used to be more like beachwear, but recently, everyone's wearing them, especially teenagers. They just live in flip-flops," Mathur said. She estimates about 50 percent of foot pain can be traced back to wearing improper footwear, and flip-flops often fall into that category.
"In general, flip-flops are bad for people because most of them are just completely flat pieces of foam with no support and no stability," said Andy Kuiper, a certified pedorthist for Lokers Shoes in Holland. Full Text

Genesco Acquires UK Footwear Retailer In $240M Deal
Genesco Inc. (GCO) acquired specialty footwear retailer Schuh Group Ltd. in a deal worth up to GBP150 million ($239.9 million), a move that gives the hat and footwear company an established retail presence in the U.K. Schuh, a casual and athletic footwear retailer, operates 59 stores in the U.K. and Ireland, 16 locations in Republic apparel stores and an online business. For the year ended March 27, Schuh reported net sales of about GPB164 million.
Under the deal, Genesco, which operates Journeys and Lids Sports Group, paid GBP100 million up front and will pay up to GBP25 million in milestone payments and another GBP25 million in management bonuses in 2015 for reaching performance-related targets. Genesco also benefits from GBP29.5 million remaining under Schuh's credit facilities. Full Text

At a time every politician's top priority seems to be creating jobs, North Carolina lawmakers hope to impose new barriers to entrepreneurs, proposing nearly a dozen new occupational licensing laws this session. Dog Chews Off Toe of Diabetic Woman As She Sleeps: Warning Note for Diabetics
By David W. Freeman An Iowa woman found out the hard way that a peckish pooch and a foot made numb by diabetes-related nerve damage make for a very bad combination. Her Jack Russell terrier chewed off part of her infected big toe as she slept. Talk about bad dogs. "She didn't feel it at all," Dr. Lee C. Rogers, a podiatrist who treated the Des Moines woman, told CBS News. "When she woke up, there was blood everywhere. The dog had been tracking it all over the place." Rogers, who now directs the amputation prevention center at Valley Presbyterian Hospital in Van Nuys, Calif., said he eventually had to amputate the woman's leg - leaving her a double amputee. The three-year-old case, which Rogers described in the May-June 2011 issue of the Journal of the American Podiatric Medical Association, is an unusual one. Adidas is going after a small California footwear company in the latest defense of its distinctive three stripes.
The German sportswear giant and its Portland-based subsidiary, Adidas America, have accused Radii Footwear of infringing on its trademark in a lawsuit filed in federal court in Oregon. Adidas wants to block the Chino, Calif.-based company from selling two makes of its lifestyle shoes and is strait jacket.JPGThe Radii Footwear's "Straight Jacket." seeking damages.
