Question: Many people come to our practice with ingrown toe nails. What role do you think pedorthists should play in educating patients and helping them manage their ingrown toenails?

Response: Ingrown toenails are an easy problem to address, and I hope that none of your regular customers have that issue!  The cause?  Shoes that are too short, too narrow, or the wrong shape for the foot.  Pulling the tongue of the shoe - particularly athletic shoes -  too hard shortens the shoe slightly as well, and wearing socks that are too short, or pulled too tight when putting on shoes also contribute to the problem.  Because 80%+ of the population is wearing shoes that are too short, it's a common issue!

Response: Patients with ingrown nails need medical attention, and should be referred to their podiatrist, family doctor, urgent care or to the emergency room for treatment.    

Often people with chronic illness don't recognize the extent of their inability to feel the fit of their shoes. Frequently, I'll trace their feet, then place the insert over the tracing to show the patient the comparison.  Properly fitted shoes with a wide toe box and antimicrobial socks may temporarily help to alleviate pain while waiting to be seen by their doctor.

Comment: It happens a few times per year: a patient who wants to "return" their orthotics after using them for a given time. I recently had a patient that had the proper script filled based on their diagnosis. We filled the script, and got her into a running shoe. We have a comprehensive break-in instruction sheet that states they can return for adjustments at no charge. We follow that with thorough verbal intructions, as well.

This person walked in without an appointment and wanted to return the FO's because "they didn't work and her physical therapist told her they weren't right." She never excercised the re-check offer, and she never called to ask about adjustments. She based her decision solely on the advice of her therapist (who never gave me a courtesy call). Like a pharmacist, we simply filled a script based on the Doctor's diagnosis.

If someone gets medication from a pharmacy based on their doctor's script, does the patient go back for a refund if it doesn't cure their respiratory infection? If they break out in hives, vomit or their toothache pain doesnt subside, are they entitled to a refund from the pharmacy? I am thinking of adding a disclaimer on our sign-in sheets explaining that we make every effort to ensure success, but we can't offer refunds. Our time money and materials are buried into the price.

I realize it is easy to say "no refunds," but we have all had those special patients who think FO's and shoes should be like returning a knife set at Walmart...no questions asked. Unfortunately, those are the ones who won't take no for an answer, and who will sometimes attempt to create ill-will with the referral source and anyone else who will listen. If anyone has a well working paragraph or two, please share. Having the patient initial it may cut down on this problem. If not, it will at least provide some backup in dealing with the ones who think orthotics are try-before-you-buy products.

Response: We are not dispensing medication. The orthotics we dispense are designed and fabricated by us or someone we contracted with to fabricate them for us. The selection of materials and designs is in our hands. We are in the business of making people happy. We have to make sure the footwear is going to be compatible with the device we create. Patient education is important. Use your best personality to encourage the patient to return so you can determine what is wrong. Ask the patient if you can contact the physical therapist to determine what their issues are. Your customer is not just the patient. It is also the prescriber and the physical therapist. Use this as an opportunity to demonstrate your professionalism, openness to constructive criticism and the greater goal of trust.

Response: I just wanted to say, in my practices we charge 299.00 for custom orthotics. So we break that into 2 parts. The first one is the custom orthotics themselves, which cost 250.00 (refundable), and then we have a 49.00 eval. fee (refundable). We have a 60-day 100% guarantee. I have not had anyone want a refund yet and I don't always get it right the first time, so sometimes we will remake the orthotic, and that usually get the pt happy and comfortable.

Question: Previously we had purchased sponge rubber (not pedic rubber) neuroma pads, and we are starting to run out of this item. But a search of all our catalogs from various pedorthic and orthotic material vendors does not show this item as currently available.  Does anyone have a lead on where to purchase sponge rubber neuroma pads?

Question: Rather than have patients fill out their information questionnaires when they come to the office, has anyone offered them to download them from your webpage and either email or fax them to your office or bring them with them when they come. Having all of the patients information entered before their visit (including medical history) would seem to offer some significant advantages in time and patient management. Has anyone done this, and if so, how, and what have been the results?

Response: We recently redid our website and included this feature.  In the past month we have received four.  Since web forms are sent via e-mail, we only require name and address.  All other personal information is up to them to provide, if they feel comfortable submitting it.

Blood vessels and supporting cells appear to be pivotal partners in repairing nerves ravaged by diabetic neuropathy, and nurturing their partnership with nerve cells might make the difference between success and failure in experimental efforts to regrow damaged nerves, Johns Hopkins researchers report in a new study.

About 20 percent of diabetics experience neuropathy, a painful tingling, burning or numbness in the hands and feet that reflects damage to nerves and sometimes leads to infections and amputation of the toes, fingers, hands and feet  over time. Current treatments for diabetic neuropathy focus on relieving symptoms, but don't address the root cause by repairing nerve damage. Previous research has shown that nerve cells' long extensions, known as axons, regenerate slowly in diabetics, scuttling various experiments to regrow healthy nerves, explains study leader Michael Polydefkis, M.D., M.H.S., associate professor of neurology at the Johns Hopkins University School of Medicine.
