Question: Many people come to our practice with ingrown toe nails. What role do you think pedorthists should play in educating patients and helping them manage their ingrown toenails?

PREVIOUS QUESTIONS AND COMMENTS WITH RESPONSES
Comment: It happens a few times per year: a patient who wants to "return" their orthotics after using them for a given time. I recently had a patient that had the proper script filled based on their diagnosis. We filled the script, and got her into a running shoe. We have a comprehensive break-in instruction sheet that states they can return for adjustments at no charge. We follow that with thorough verbal intructions, as well.
This person walked in without an appointment and wanted to return the FO's because "they didn't work and her physical therapist told her they weren't right." She never excercised the re-check offer, and she never called to ask about adjustments. She based her decision solely on the advice of her therapist (who never gave me a courtesy call). Like a pharmacist, we simply filled a script based on the Doctor's diagnosis.
If someone gets medication from a pharmacy based on their doctor's script, does the patient go back for a refund if it doesn't cure their respiratory infection? If they break out in hives, vomit or their toothache pain doesnt subside, are they entitled to a refund from the pharmacy? I am thinking of adding a disclaimer on our sign-in sheets explaining that we make every effort to ensure success, but we can't offer refunds. Our time money and materials are buried into the price.
I realize it is easy to say "no refunds," but we have all had those special patients who think FO's and shoes should be like returning a knife set at Walmart...no questions asked. Unfortunately, those are the ones who won't take no for an answer, and who will sometimes attempt to create ill-will with the referral source and anyone else who will listen. If anyone has a well working paragraph or two, please share. Having the patient initial it may cut down on this problem. If not, it will at least provide some backup in dealing with the ones who think orthotics are try-before-you-buy products.

Response: I think it is a bad precedent to do refunds on any Custom work. First, it is very important when that customer comes in, before any monies change hands, to communicate with said patient and get a feel not only for their medical issue, but also for their expectations. Depending on the problem, you then can sufficiently explain how you are both going to approach said issue, the kind of results that can be expected, and a time frame. Also, specify that there might be adjustments needed, and always schedule a 2 week follow-up visit. Communicate with your patients that it is a team effort, and you have to work together to accomplish great results. Be very careful to be realistic with your services, products and footwear. The customer has the same responsibility.
Lastly it is important to explain that you will be providing a custom device that is made specifically for that individual, and is not returnable. You are investing your knowledge, time and materials. You will not be able to sell them to anyone else. Ensure that you also have this on your receipts and clearly visible in your facility. - Althea Powell C. Ped.
Question: How many pedorthists out there use impression foam for fabricating functional orthotics, rather than slipper casting, and what are your outcomes?
Does anyone use a first ray cutout in their functional orthotics routinely, rather than just for a plantarflexed first ray?

Response: I use Bioputty. It is a mixture of microballons and vasoline. It works much like a foam box, but yields an impression like a slipper cast. Best of all it is reusable. My casting boxes are ten years old.

PREVIOUS QUESTIONS WITHOUT RESPONSES
Question: Previously we had purchased sponge rubber (not pedic rubber) neuroma pads, and we are starting to run out of this item. But a search of all our catalogs from various pedorthic and othotic material vendors does not show this item as currently available. Does anyone have a lead on where to purchase sponge rubber neuroma pads? - John Mishasek, BOCPD, C. Ped.
Question: Rather than have patients fill out their information questionnaires when they come to the office, has anyone offered them to download them from your webpage and either email or fax them to your office or bring them with them when they come. Having all of the patients information entered before their visit (including medical history) would seem to offer some significant advantages in time and patient management. Has anyone done this, and if so, how, and what have been the results?

Diabetics Might Be at Greater Risk for Second Stroke

After suffering a first stroke or transient ischemic attack (TIA), people with diabetes are at greater risk of having another stroke or a heart attack, a new study confirms. But, aggressively lowering cholesterol can help reduce that risk, the researchers report.
"Patients who had a stroke or TIA and who have diabetes are at higher risk of having another stroke as compared to patients who have no diabetes or those who have metabolic syndrome," said study researcher Dr. Larry B. Goldstein, director of the Duke Stroke Center at Duke University Medical Center in Durham, N.C. Full Text

| Stress Fractures of the Foot

Stress fractures of the foot are more common than you might think. Most stress fractures occur in the weight-bearing bones of the foot and lower leg when muscles become fatigued or overloaded and cannot absorb the stress, shock and repeated impact. Fatigued muscles transfer that stress to the nearby bone and the result is a small crack or fracture, in the bone. Symptoms include gradual soreness, stiffness, and a pinpoint pain on the affected bone. Early diagnosis is critical because the fracture can propagate and eventually become a complete fracture of the bone. Full Text

The Shoe Service Institute of America, in partnership with CFS Allied Health Education, would like to announce a C.Ped. course to be presented prior to this summers SSIA convention in St Petersburg, FL. The American Board for Certification has approved a total of 14 CEUs that can be earned for the low cost of $395 during this two-day course.
