NEW QUESTIONS
Question: Rather than have patients fill out their information questionnaires when they come to the office, has anyone offered them to download them from your webpage and either email or fax them to your office or bring them with them when they come. Having all of the patients information entered before their visit (including medical history) would seem to offer some significant advantages in time and patient management. Has anyone done this, and if so, how, and what have been the results?

PREVIOUS QUESTIONS AND COMMENTS WITH RESPONSES
Comment: It happens a few times per year: a patient who wants to "return" their orthotics after using them for a given time. I recently had a patient that had the proper script filled based on their diagnosis. We filled the script, and got her into a running shoe. We have a comprehensive break-in instruction sheet that states they can return for adjustments at no charge. We follow that with thorough verbal intructions, as well.
This person walked in without an appointment and wanted to return the FO's because "they didn't work and her physical therapist told her they weren't right." She never excercised the re-check offer, and she never called to ask about adjustments. She based her decision solely on the advice of her therapist (who never gave me a courtesy call). Like a pharmacist, we simply filled a script based on the Doctor's diagnosis.
If someone gets medication from a pharmacy based on their doctor's script, does the patient go back for a refund if it doesn't cure their respiratory infection? If they break out in hives, vomit or their toothache pain doesnt subside, are they entitled to a refund from the pharmacy? I am thinking of adding a disclaimer on our sign-in sheets explaining that we make every effort to ensure success, but we can't offer refunds. Our time money and materials are buried into the price.
I realize it is easy to say "no refunds," but we have all had those special patients who think FO's and shoes should be like returning a knife set at Walmart...no questions asked. Unfortunately, those are the ones who won't take no for an answer, and who will sometimes attempt to create ill-will with the referral source and anyone else who will listen. If anyone has a well working paragraph or two, please share. Having the patient initial it may cut down on this problem. If not, it will at least provide some backup in dealing with the ones who think orthotics are try-before-you-buy products.

Response: Brian, I feel your pain. I like the ones who ask what kind of "success guaranty" (in writing!) I can give them. I also provide the pharmacy/medication sample to those people. Fortunately, these people are few, but unfortunately, they can cause some headaches, especially if they come without an appointment and you have a few customers in the store. These people usually get very aggravated, and that is not a nice picture for your other customers.
I would suggest to have them sign and read !!! - I have 5 "No Refund" signs in the store and everybody that comes in asking for a refund states that they did not see the signs - a form at dispense so that they understand there is no "success" guaranty, and that there is no refund. Make sure you give them a copy and keep one for you on file. Good luck.

Response: As long as you have an adjustment policy and you communicate it in writing and verbally, you're meeting your obligation in making a custom product as good as it can be for that individual. As to her badmouthing you to the referral doctor and physical therapist, she's either done or will do similar things to them, and if they don't already view her skeptically, they will. She's not the first patient like this, and won't be their last. Keep your head up and know that you are serving many others who are appreciative.

Response: I tend to approach this issue from a slightly different perspective. I feel that patients come to me to solve their problem, not sell them a product. So from a practical standpoint, I am in a service business, not unlike an auto mechanic who uses parts and labor to fix your car. If the mechanic attempts a repair but the problem does not go away, I expect him to fix it or refund my money. I believe our patients have the same right to expect that from us. So in those rare cases where the patient is not satisfied and you cannot achieve the result that you both expected, I think it makes good business sense to refund the patient's money. Doing so is simply a cost of doing business, not unlike money spent for rent, advertising and salaries. Hopefully if you are good at your profession, these occurances will happen infrequently. And I do realize that in the example Brian used the patient never gave him the opportunity to make adjustments or remake the orthotics. But is it really worth the possibility of impacting a referral relationship? I say apologize profusely for not solving the patient's problem and send them on their way with refund in hand!

Question: How many pedorthists out there use impression foam for fabricating functional orthotics, rather than slipper casting, and what are your outcomes?
Does anyone use a first ray cutout in their functional orthotics routinely, rather than just for a plantarflexed first ray?

Response: I use Bioputty. Its a mixture of microballons and vasoline. It works much like a.foam box but yields an impression like a slipper cast. Best of all its reusable. My casting boxes are ten years old.

Response: We primarily use impression foam, though not always. The decision is based on whether we can gain a good negative of the foot in a neutral position. Hypermobile feet are hard to control in foam. It is important to make sure the first ray is not dorsiflexed when taking the impression. We will often press the first met head into the foam. To help establish the effect of first ray cutout, we end the orthosis proximal to the first met head when fabricating the orthosis.

Comment: In the June edition of The Costco Connection, there is an article titled "When Little Things Turn Big." In this article by Susan Glariron under the heading Feet First, the story of a Costco member losing his leg to complications of diabetes is given. The article goes on to say, "improperly fitting shoes are the biggest cause of amputations in diabetic patients." And lo and behold, the article then quotes Dr. Michael Pinzur, professor of orthopedic surgery at Loyola Health System, that "to avoid the problem, people with diabetes should wear only diabetic shoes fitted by a certified pedorthist, a specialist who designs, fits and/or modifies shoes and foot orthoses to alleviate foot problems." The article says: "to locate a nearby pedorthist, visit www.pedorthics.org." The article also states that if a person with diabetes gets a foot blister, they should immediately seek treatment from an orthopedic surgeon or podiatrist. It would be very helpful if pedorthists as a group could get this type of exposure more routinely. Perhaps pedorthists and PFA could work together to identify those orthopedic and podiatric physicians that include pedorthists as part of their clinical team to get better clinical outcomes for their patients, and determine if these physicians are willing to generate articles in their local papers about how they deliver care to their patients, in combination with pedorthic footwear specialists who have an inventory composed of a wide range of lasts, sizes, and brands on the shelf to do proper fitting and problem solving. - John Mishasek, BOCPD, C. The American Academy of Orthotists and Prosthetists Launches Online Book Reviews
The American Academy of Orthotists and Prosthetists (AAOP) is proud to introduce formal book reviews as part of the Recommended Reading section of its website at www.oandp.org/reading.asp.
Phil Stevens, MEd, CPO, FAAOP, chair of the Academy's Publications Committee, has teamed up with other experienced clinicians to provide a summary of content and relevance of a variety of texts to orthotic and prosthetic students, clinicians, and researchers.
"Textbooks aren't cheap, and we want to assist professionals in identifying the ones that will be most beneficial for their individual needs," said Stevens. "Orthotic and prosthetic texts vary tremendously in their scope and depth. Most are good, but appeal to different audiences-some to new students, others to professionals in related fields, some to experienced O&P clinicians, and some to patients."
"The reviews from online resellers are rarely written by clinicians in the field. The reviews that the Academy has put together, on the other hand, come from those who can speak about texts from an insider's perspective," continued Stevens.
There are currently six book reviews posted to the site. The Publications Committee plans to launch as many as 10 additional reviews by the end of the year.

| Eliminating Shoe Tax Could Save Companies, Consumers

U.S. consumers could see a sharp drop in the price of imported shoes - nearly all the footwear sold in the country - and retailers could see a bonanza if lawmakers dump taxes on certain imports.
The move to scale back the taxes essentially signals surrender on one front of a protectionist trade war. There are virtually no American-made shoes to protect from foreign competition anymore. Instead, lawmakers now see the tax as antiquated and unfair. Full Text

Arthritis Increase Blamed on High Heels

An "arthritis crisis" could be looming with certain shoes to blame, the Society of Chiropodists and Podiatrists said recently.
The BBC reported that the condition, more frequent in women, can be aggravated by wearing high heels. The footwear alters the body's posture and increases pressure on the foot, ankle and knee joints, experts said, leading to an increased risk of osteoarthritis. A quarter of women reportedly wear high heels either frequently or daily, a Society poll of 2,000 people reported. Full Text

To see the Kobi Levi's "shoe creatures" might make you wonder how it's possible to teeter on high heels inspired by chewing gum, chopsticks or toucans. But the Israeli designer says they're intended to be worn -- just ask Lady Gaga, who wore a pair of his shoes in her video, "Born this Way."
Levi says he's not necessarily out to make money off his whimsical creations. He has a job designing and developing a line of women's fashion shoes in collaboration with an Israeli shoe store chain. Fewer amputations in diabetic patients with dual isotope SPECT/CT
Research introduced at SNM's 58th Annual Meeting reveals the extent to which an ongoing study can help save life and limb for patients with diabetes-related foot infections. Diabetes can cause nerve damage and reduced blood flow to the bones and tissues of the feet, leaving diabetics vulnerable to infection. This study shows that combining two imaging agents with molecular imaging techniques provides diabetic patients an excellent infection screening method that has already spared a number of patients from aggressive amputation of infected feet.
"As a result of this study we have concluded that dual isotope SPECT/CT is an important method for detecting infection in diabetic patients who might otherwise receive unnecessarily aggressive treatment such as amputation," says Sherif Heiba, MD, associate professor of radiology at Mount Sinai School of Medicine, New York, N.Y. "We want to avoid amputation whenever possible, because it has been shown to be detrimental to patients in terms of both quality of life and survival." Full Text

Derma Sciences Foot-wound Drug Shows Promise

Derma Sciences Inc said foot ulcers in 85 percent of diabetic patients healed completely after being treated with its experimental drug at the end of a 24-week trial, improving its chances of licensing the drug to potential partners.
Shares of the company, which specializes in wound-care products, jumped 25 percent to a more than three-month high of $11.65 in afternoon trade on Wednesday on Nasdaq. The data will help the company in outlicensing ex-U.S. rights of the drug, codenamed DSC127, to a potential partner, Chief Executive Edward Quilty said in an interview. Full Text

Richard Caesar headed out the door Wednesday afternoon of Arc Bridges on a mission. Caesar, a client of the not-for-profit organization for developmentally challenged adults, made a beeline for Noelle Thomas-Desso's car. Caesar was eager to pick up the large box in her trunk.
It was full of new athletic shoes, donated by Schererville-based Fleet Feet Sports to Caesar and 24 other clients who will head June 2-4 to Terre Haute to compete in track and field events at the 2011 Indiana Special Olympics Summer Games. Full Text
