Comment: It happens a few times per year: a patient who wants to "return" their orthotics after using them for a given time. I recently had a patient that had the proper script filled based on their diagnosis. We filled the script, and got her into a running shoe. We have a comprehensive break-in instruction sheet that states they can return for adjustments at no charge. We follow that with thorough verbal intructions, as well.
This person walked in without an appointment and wanted to return the FO's because "they didn't work and her physical therapist told her they weren't right." She never excercised the re-check offer, and she never called to ask about adjustments. She based her decision solely on the advice of her therapist (who never gave me a courtesy call). Like a pharmacist, we simply filled a script based on the Doctor's diagnosis.
If someone gets medication from a pharmacy based on their doctor's script, does the patient go back for a refund if it doesn't cure their respiratory infection? If they break out in hives, vomit or their toothache pain doesnt subside, are they entitled to a refund from the pharmacy? I am thinking of adding a disclaimer on our sign-in sheets explaining that we make every effort to ensure success, but we can't offer refunds. Our time money and materials are buried into the price.
I realize it is easy to say "no refunds," but we have all had those special patients who think FO's and shoes should be like returning a knife set at Walmart...no questions asked. Unfortunately, those are the ones who won't take no for an answer, and who will sometimes attempt to create ill-will with the referral source and anyone else who will listen. If anyone has a well working paragraph or two, please share. Having the patient initial it may cut down on this problem. If not, it will at least provide some backup in dealing with the ones who think orthotics are try-before-you-buy products.

PREVIOUS QUESTIONS AND COMMENTS WITH RESPONSES
Question: I am seeing a lot of patients who are runners, wearing shoes like the Nike Free and New Balance Minumus. For many of these people, these shoes are completely inappropriate and actually causing problems, yet they insist on wearing them. Is anyone else encountering this and how are you handling it?

Response: In summarizing the overall comments on running from last week, there was a good cross section from the opinionated to the educated. I agree that the minimalist footwear movement has traction for a variety of reasons, and although it's a tiny segment of the market, it's a subject C. Peds should educate themselves about.
Here's some information from the lab. Humans utilize three different footfall patterns: rearfoot, midfoot and forefoot. The patterns are distinguished by the portion of the foot making initial contact with the ground. When you look at Ground Reaction Force (GRF) graphs of these patterns, the overall force magnitude of the curve is the same. However, the mid and forefoot impact is absorbed by the soft tissues of the foot rather than the skeletal system of the body when you rearfoot strike; which explains the Achilles injuries occurring in barefoot runners. The collision forces still occur; it's just a matter of which anatomy is forced to absorb it.
· Discomfort forces the body to change its habitual state of running
· Forefoot running is not the natural or habitual way to run for most individuals
· Enabling the body to move in its habitual state can lead to a reduction of injuries and increase efficiency of running
· Runners will default to their natural pattern when they get tired no matter their initial forced footfall pattern.
The majority of runners run for pleasure and exercise. Regardless of the validity of the info, they spend more time gathering info on their chosen past-time than any other segment of the footwear market. Runners are increasingly seeking companies who build their footwear based on science rather than hype. Pedorthists are here to treat runners when called upon, and maybe to add to their education as an extension of our practice; not change them.

Response: After receiving the June 1, 2011 Pedorthic Newswire and reading in the discussion section the question from Erick Janisse, and the following response from S. Johnson, C.B.R.T., Certified Barefoot Running Trainer, I find myself a bit mystified about S. Johnson's certification. After a half-hour of Google searching, I've found no information on "Certified Barefoot Running Trainer," anyone credentialed with this certification, or any training program for it. Could someone, S. Johnson, perhaps, please provide information on this certification as to its validity, who is the certifying body, and how one can pursue becoming certified? Quite honestly, I'm left wondering if this is a recognized credential.

Question: Previously we had purchased sponge rubber (not pedic rubber) neuroma pads, and we are starting to run out of this item. But a search of all our catalogs from various pedorthic and othotic material vendors does not show this item as currently available. Does anyone have a lead on where to purchase sponge rubber neuroma pads?

Comment: In the June edition of The Costco Connection, there is an article titled "When Little Things Turn Big." In this article by Susan Glariron under the heading Feet First, the story of a Costco member losing his leg to complications of diabetes is given. The article goes on to say, "improperly fitting shoes are the biggest cause of amputations in diabetic patients." And lo and behold, the article then quotes Dr. Michael Pinzur, professor of orthopedic surgery at Loyola Health System, that "to avoid the problem, people with diabetes should wear only diabetic shoes fitted by a certified pedorthist, a specialist who designs, fits and/or modifies shoes and foot orthoses to alleviate foot problems." The article says: "to locate a nearby pedorthist, visit www.pedorthics.org." The article also states that if a person with diabetes gets a foot blister, they should immediately seek treatment from an orthopedic surgeon or podiatrist.
It would be very helpful if pedorthists as a group could get this type of exposure more routinely. Perhaps pedorthists and PFA could work together to identify those orthopedic and podiatric physicians that include pedorthists as part of their clinical team to get better clinical outcomes for their patients, and determine if these physicians are willing to generate articles in their local papers about how they deliver care to their patients, in combination with pedorthic footwear specialists who have an inventory composed of a wide range of lasts, sizes, and brands on the shelf to do proper fitting and problem solving.

Question: How many pedorthists out there use impression foam for fabricating functional orthotics, rather than slipper casting, and what are your outcomes?
Does anyone use a first ray cutout in their functional orthotics routinely, rather than just for a plantarflexed first ray?

Herpes Infection Led to Toe Pain
Q. Recently, my left big toe swelled up, formed something like small blisters and itched and burned. I knew this wasn't normal, so I went to the doctor.
The physician's assistant diagnosed the problem as "herpetic whitlow." She prescribed acyclovir to speed recovery. I couldn't afford it, and when I learned it wouldn't cure the problem, I went without. I took diphenhydramine to help me sleep through the itch. It seemed to reduce the swelling a bit as well.
