Comment: Cheers to "-C. Ped." from issue #411. I've wanted to be able to explain this issue to people for some time now but not been able to state it quite so well. Reimbursement rates from "Medicare's Therapeutic Shoes for Persons with Diabetes Benefit" are ridiculously low, and have brought many companies to one of their toughest decisions. Some seem to have made up for the low reimbursement by increasing patient load, some provide shoes and make up for the disparity by providing and billing for all six orthotic units at one time, and some have gone to the unfortunate extreme by not accepting assignment altogether.
All options have their ups and downs. By increasing patient load, one may also increase stress, create a greater opportunity for clerical error and possibly decreases the actual patient care. I've even seen attempts at 15 min. appointments for new patient evaluations, which was just ridiculous. When it comes to the TSD, my fellow practitioners and I understood it to allow one pair of shoes and either orthotics or additions as needed per patient, to a total of one pair of shoes and six billable orthotic units (again, as needed) throughout the course of a year. A lot of our local competitors have decided that it means a pair of shoes and three pair of heat molded (if even that) FOs for the year, to be done in either one visit or through the mail. While this offers an obvious monetary improvement to the provider, it also decreases patient care both immediately and over time. While a diabetic patient deserves attention multiple times throughout the year in regards to both their general health but more specifically their feet, there unfortunately isn't reimbursement for this in the bill. Finally, not accepting assignment allows for better reimbursement from the patient directly, but it risks a heavy loss in patient load. While those being seen should be getting the best care possible, many are not able to take part due to economic hardships.
Like many government programs, the TSD is clearly flawed, and I find it ironic that one of the reasons the reimbursement isn't enough is the overhead necessary to be able to use the system in the first place. I know I'm preaching to the choir here, we've all known this for quite some time. I just thought I would take the opportunity to attempt a formulation of my thoughts and welcome any discussion. - Mark C. Scheibelhut C.Ped., O.S.T.
PREVIOUS QUESTIONS AND COMMENTS WITH RESPONSES
Comment: The recent posts on pediatric flatfoot are helpful and interesting. However, I would like to offer a different opinion on orthotic treatment. Many children that over-pronate do not need a forefoot varus post. In many cases, the problem is caused by tight heel cords, lack of dorsiflexion and or weak post tib tendon. This causes the hindfoot to evert and the midfoot to collapse as the foot tries to get "motion" where it can as the knee passes over the foot at midstance.
Many of these pt's have what you could refer to as a flexible deformity. I rely on a deep heel cup and the proper amount of heel lift with varus wedge, extended distal to under the navicular. Placed in a proper shoe, one can gain an amazing amount of control, while avoiding the use of a forefoot varus wedge. I realize this runs counter to some popular teaching but I would prefer that the 1st ray be allowed to come down to a more natural position. Putting high degree of varus posting in developing feet, could encourage the development of a true forefoot varus later in life.
I also feel the family/pediatric physicians need to be detailed more. I cant recall how many times I have seen an angry parent of a 9 to 16 y/o with foot, shin or knee pain that was told "don't worry, they will grow out of it" or "it's just growing pains". Having lower extremity pain every sport season isn't "growing pains" when they experience it for several years in a row when they ramp up their intensity. Even a decent pre-fab that has the proper modification can work wonders and keep costs down.

Response: I would like to point out to the PNW readers who may be new to the profession, that Brian is describing extrinsic forefoot varus posting, which dorsiflexes the first ray versus intrinsic varus posting, which allows the first ray to maximally plantarflex at propulsion. We agree that using intrinsic or extrinsic hindfoot posting in realigning these feet and preventing further degeneration of this pathology is a good treatment modality, however, these feet may require that their fixed forefoot varus deformity be addressed with some form of intrinsic forefoot varus posting. This can be accomplished using Rectus casting, manipulating the foot during the non weightbearing casting technique, or it can be done in the positive model by applying a force at varying locations such as techniques described by Root, or Modified Root, Functional, Modified Functional or using more advanced techniques. Either way, in most biomechanical theories, when there is range of motion available, intrinsic forefoot varus posting is a viable and biomechanically sound way of attempting to restore the mechanics to these feet. Furthermore, to clarify, when attempting to use intrinsic forefoot posting, it is widely used when range of motion is available in the first ray joint axis, midtarsal joints, first MPJ and LisFranc joint complex. Hope this helps to clarify or encourage further study of this topic.

Question: Where can bulk casting foam be purchased? We like to use individual casting trays with bulk foam, which cuts waste and is cost effective when making supports in house. Our supplier just informed us that they were no longer distributing bulk foam.

Response: We get our bulk biofoam from Acor. (1-800-237-2267) I just called, and they have no intention to stop selling this product in bulk. It is the pink biofoam, which I think is much easier to work with than the blue. It is softer, makes a better impression, and cleans off better after plaster is poured. No, I do not work for Acor, I just know what I like!

Question: I have some concerns of my own, but I'd be very interested in knowing what the rest of the Pedorthic Newswire readership feels is lacking or could be improved as far as the pedorthic industry's educational offerings - both pre-cert and post-cert. Response: I was brought into pedorthics back in 2002, when all that was required was four weeks' worth of class and a certification exam held in Chicago. I will admit that the brevity of the education was one factor that I appreciated when weighing the decision to become a pedorthist. I was young, and didn't enjoy the idea of going through a lengthy education. Yet it was immediately clear to me that, while what I learned in those classes applied, it barely touched on what I had to deal with. I was "pushed into the deep end," so to speak, by my employer, and was expected to see patients directly out of school. I felt inadequate at best.
It has been nine years, and I can say that I have learned quite a bit, both on my own and with my fellow practitioners. But now I realize even more what I am lacking. It surprised my employer to know that I still get nervous when dealing with patients. I believe this is a twofold problem: one, being that I am just socially anxious anyway, and two, that I feel undereducated even now.
When talks began about an associate degree in pedorthics, I actually got excited. I am now at a point in my life when I believe better education would be rewarding. I would love the chance to spend a while studying biomechanics, anatomy and pathology to better my knowledge and improve as a practitioner. I must say that working in an O&P facility, I have seen many more AOPAs than PFA events, and the continuing education always seems to be either a company selling its product or a rehash of one of the classes I have already been through. I suppose what I am getting to is that, while I appreciate my pedorthic education, which in itself was quite well done, I would welcome a longer, broader and more in-depth education.

PREVIOUS QUESTIONS WITHOUT RESPONSES
Question: I do not take assignment for therapeutic shoes and inserts from medicare. However, patients still want me to file a claim on their behalf. The reason I do not take assignment is because of all the paper work and documentation. When I file a claim, non assigned for the patient, for therapeutic shoes, I can not use the KX modifier because I do not have all the documentation. But without the KX modifier the claim will be denied. So it is a waste of time to send claim in without KX modifier. Can the patient gather the documentation on his/her own and keep it on file and then fight it out with Medicare? Can I use the KX modifier on behalf of patient and not be held liable by Medicare for repayment if they are not sending me any money in the first place? Or should I just refuse to file claim and let patient do it all? - Scott Torppey, CO, C.Ped.
