Question: Flat feet--some are born with the condition, and it often causes no problems. But flat feet in children are often overlooked. Arches that disappear when standing and ankles turning inward are signs. We believe flat feet should be checked out to prevent problems later. What are some strategies to increase awareness and provide help for flat feet?

Response: These are some observations from my 48 years in the shoe and footcare professions. Not in every case, but commonly, a flat foot is a normal foot for many people. In many parts of the world, flat feet are the norm, rather than the exception. Since virtually all last designs were created for a population in Europe, where flat feet are the exception, people with flat feet often develop pain while trying to wear footwear built on these lasts. Feet have been evolving to regional environments for millions of years. Those of us whose ancestors evolved on the flat open areas in central Africa, India, and Australia will usually have flat feet at birth and throughout their life. The Masai are frequently cited in publications as representative of this condition. The relatively recent transport and migration of large segments of these populations tends to mask the origins of the evolution of foot types. Flat feet are ideal for walking barefoot over flat dirt surfaces. Many of the people who still live in these areas of our world continue to walk comfortably barefoot. Who do you think is more comfortable in the 1890 photograph of an African safari? Is it the guy in the boots and pith helmet in the front or the guy in the background who carried his luggage, for hundreds of miles, while barefoot? In a small survey we did for Munro Shoes, in the 1990s, on African American women's feet, we found all subjects we evaluated had flat feet with a weight-bearing styloid process of the fifth metatarsal, a posterior-projecting calcaneus, and a straight or slightly lateral deviation of the forefoot on the rear foot. It was obvious to us that most shoes, designed on European based lasts (adducted last, moderate to high heel-to-shank transition, and shallow back-curve on the counter), were not going to fit these feet and would be uncomfortable. It did explain why the Daniel Green mule was the best selling shoe among African-American women for decades.
The "take home message" here would be - we need to design lasts and stylish footwear for people whose feet are normal and ideal for the environment of their ancestors. In the U.S. they need to be able to purchase footwear built from lasts made for the contours of their foot type to be used in a society covered with flat concrete and blacktop walking surfaces. In other cases, the flat foot has progressed from a high forefoot varus resulting in a foot positioned in moderate to severe pronation while standing. Many orthopedists refer to this as "too many toes" when viewed from the rear. This is the type that needs early orthotic or surgical intervention. This can occur in people of any ancestry and is relatively rare in our general population. With this form, the leg will gradually shift more medial over the foot. This shifts the body's weight over the medial arch, everts the calcaneus, and shifts the talar head to a weaker part of the spring ligament resulting in a collapse of the arch. Orthotic management needs to address the forefoot varus with the goal of keeping the leg centered over the foot in walking and in stance. Surgically osteotomies can be done to reposition the forefoot relative to the rearfoot. The educational goals for the pedorthist would be to recognize the difference between the progressive flatfoot deformity (a frontal plane malalignment) from the normal, inherited flatfoot (a sagittal plane alignment). - William Coleman, DPM
Response: During my years of working with my mentor and co-founder, Dr. Palmer, his practice consisted of a high percentage of orthopedic pediatrics. With his Master's Degree in genetics, he donated many years to the study of club feet and reconstructive procedures. He often referred his patient's to my pedorthic clinic for functional feet orthoses, Whitman, Whitman Roberts Orthoses; all casted in Rectus position (non weightbearing, subtalar neutral while reducing the forefoot upon the neutral, aligned hindfoot). We also made gait plates to induce intoe our out-toe, dispensed torque heels attached to the soles of shoes, along with medial heel and forefoot wedges, etc. However, in your question for strategies to increase awareness and provide help for flat feet, we developed a brochure using a Krames brochure as our guideline and giving Krames credit for contributing to the brochure. We had hoped that in time, they might produce the brochure for this campaign. It is titled "What your physician wants you to know - Your Feet - Your Child's Feet". Our goal with this brochure was to get the brochure into the hands of pedorthists on a national basis, once they had achieved a specific level of understanding of the milestones of the growing child's foot and the timeframe by which we can actually attempt to correct these deformities, before ontogeny occurs. Learning how to properly cast these feet (similar to the Ponsetti technique principles; molding the foot with a cast versus us using a thermoplastic to remodel the foot). But before we can implement a national campaign, we need to ensure that the pedorthic community has education focusing on pediatric prevention, biomechanics, conservative and advanced techniques and posting. It is detrimental that we not reposition a foot incorrectly. Carl Riecken and I were working on this before his passing using the slogan for this national campaign as "These little feet grow like "Popcorn", let's mold them while their Hot!" (hince - for you RMPI former students - we always make freshly popped popcorn during our class!...fond memories - right?) The intent is to have pedorthist's in the country to place their logo on the brochures and desiminate them in pediatrician's offices, orthopedic offices, day care facilities, laundromats, retail shoe stores, etc. However, in our opinion, our present curriculum does not adequately educate a pedorthist on pediatric pedorthics to a competency level. We would recommend that any person desiring to market their pedorthic services to the pediatric marketplace, should first receive a certificate of completion of a pediatric prevention and biomechanics course. I am sure that many of the current pedorthic schools would be eager to work together on creating a well-rounded, multi-discliplinary approach to launch this national campaign! Question: There was an article in the recent LER magazine's special educational series supplement that caught my attention. It was regarding diabetic orthotics. Without making this long and drawn out, I will get to the point. Some of the healthcare practitioners that were interviewed stated that custom diabetic orthotics (A5513) are only needed occasionally. A quote from Bill Meanwell states, "The A5513 is needed occasionally." He goes on to say "The problem that we have in our industry is that we get paid more for the custom device than the OTS (off the shelf). You are thinking with your wallet and not with your head." There were also other healthcare practitioners quoted who stated that OTS or A5512 were appropriate for the majority of their patients. I know that in my practice, I see alot of diabetics, and the majority of them have at least moderate nueropathy, boney prominances, high pressuure areas with callusing, wide feet, etc. The majority of these patients will require more than an OTS heat moldable orthotic. In my experience, the OTS orthotics that are available are cheap, mostly only dual lam. They generally wear out quickly. The other issue I have is that these devices never contour 100% to the patient's foot when heat molded. You also will have to add a scaphoid pad to the medial/longitudinal arch to make a total contact orthotic. What happens to the lateral longitudinal arch? It will just flatten. Then, adding a metatarsal pad takes time to get in the correct location. Adding any further modifications like heel lift, adding a reverse Mortons extension as proposed by Marybeth Crane, DPM, takes even more time. The big question then is: is this the correct orthotic for someone requiring all these modifications? The other question is: how do you get paid to do all of this work? A custom diabetic orthotic can be manufactured with almost all of the features listed above. If a practitioner is going to put that much work into the orthotic, they need to be paid for the work. I don't have time to try to make a Cadillac out of a Pinto and get reimbursed poorly for it. The time spent "customizing" the orthotic could be spent seeing other patients or charting. It concerns me that one of the Pedorthic profession's educators is being quoted in a highly reviewed journal as saying that pedorthists are "thinking with their wallet and not with their head." This is not the impression we want other allied professionals to have of us. I don't want my local referral sources thinking that I am fabricating custom orthotics for their patients simply to make more money. Does anyone else have an opinion on this? - C. Ped.
Response: With respects to Bill Meanwell, if I'm thinking with my wallet every patient gets an A5512, as the difference in compensation doesn't really justify the difference in cost to manufacture customs. As a percentage to cost the A5512 inserts are far more profitable than the customs. But should this even be an argument? Isn't it up to the doctor to determine what the patient needs? Medicare requires a doctor's order (prescription) for each patient. Every time we see a prescription that does not specify custom inserts we either fit the patient with OTC inserts or we contact the doctors office with a recommendation for customs and request a prescription to match the treatment. During a Medicare audit we were turned down on customs for a patient because the doctors order did not specify the need for custom made inserts, so we simply don't bill for customs unless we have a prescription for them in hand. As to modifying the OTC inserts, we avoid having to augment the inserts largely by using a casting platform when we mold the insert, this holds the arch up and allows for a weight bearing molding. If we need to modify the inserts much beyond trimming them to fit the shoes we would request a script for customs. I would agree with Bill that the vast majority of our patients don't really need customs even though I also believe most would benefit from them. If your OTC inserts are requiring extensive mods in order for them to function correctly maybe you should consider a higher quality insert?

Question: In a recent discussion between DME students working nursing homes and Retail Students, two different perspectives came into debate. One perspective was that the "DME" company could "drive" the prescription by completing the CMN form for the physician using the patient's nursing home facesheet as the means for obtaining the primary and secondary diagnoses, and that the physician was responsible for verifying the information on the CMN form completed by the DME provider. The second perspective was that the "DME" company was performing an illegal action by completing any aspect of the CMN form, other than the beneficiary's (patient's) name, date of birth and Medicare number. Typically, in a retail setting the attending physician will usually complete the forms where the nursing home attending physician usually leaves them blank, with only his signature. Thus, the DME perspective is to assist the physician with completing them, and the DME student felt this was legal, while the Retail student said this was illegal. In my clinical experience and as an educator, I agree with the Retail Student.
Another consideration: Can a DME company have the attending physician sign a document stating that he (the physician) gives permission/allows the pedorthist who is assessing, fitting and dispensing the shoes, to assist with completing the form on his behalf; as long as he has agreed and approves of the primary and secondary diagnoses on the form completed by the pedorthist and then followed up with his (the physician's) signature on the CMN?
If anyone knows where there might be a formal document from Medicare defining any of these topics, I would appreciate receiving a copy. If anyone feels there is a person of authority that we could ask this question of and obtain some form of written validity, I would appreciate receiving their contact information.

Response: I am constantly seeing questions regarding Medicare guidelines on the TSB and who can bill, what documentation is required and what doctors can order the shoes. Does anyone read the Medicare manual and lcds? It lays out plainly who can order shoes, what documentation should be in your hands (if you are dispensing shoes) and who can dispense shoes. There is even a timeline now that became effective January 1, 2011 which states that the primary care physician treating diabetic patients must see and examine patient's feet within six months of the shoe dispensed. The ordering doctor may or may not be the PCP but if they are not, the PCP must review the chart notes on the foot exam and sign off on that exam. The shoes can then be ordered and the PCP has to complete the Medicare certification statement in full (the supplier is no longer able to fill in the blanks or prompt the doctor in any way.) The list goes on to explain the full timeline. LCD states "The Supplier is the person or entity that actually furnishes the shoe, modifies, and/or insert to the beneficiary and that bills Medicare. The supplier may be a podiatrist, pedorthist, orthotist, prosthetist or other qualified individual. The Prescribing Physician may be the supplier. The Certifying Physician may only be the supplier if the certifying physician is practicing in a defined rural area or a defined health professional shortage area". In the past, a pharmacist could dispense shoes, which is why you often find many rural area pharmacies still dispensing them. As of last July, a face to face encounter between the patient and the supplier is a requirement. An initial visit, including documenting all measurements, problems relating to the foot diagnosis and whether there is stock of the shoes is a must. Any supplier that did not have a part in seeing the diabetic patient beforehand, a certification statement, and a prescription per item and/or chart notes would be making a huge mistake. The next steps are to fit the patient with the proper footwear, heat mold the inserts (if dispensing A5512) and document the date. You cannot be a supplier and not know your LCD and Medicare guidelines. Not knowing this info will cause you to lose money, the product and possibly your ability to dispense footwear.

Question: What is the best slip resistant shoe for culinary field customers who have plantar fasciitis? Typically the prescription from the podiatrist will request the supply of a running/walking shoe to be used with the orthotic that we are to supply, but none of the running/walking shoe brands we carry have proved to be really slip resistent on wet, greasy kitchen floors. We carry Sanita clogs that seem to work well after their plantar fasciitis has gone away, and we just picked up Klogs that are labeled slip resistant, but neither meet the requirements of the podiatrist's Rx for the initial treatment phase. Response: New Balance 512 is OSHA approved for slip resistant. It comes in multiple widths, and comes with a removable insole to accommodate orthotic devices.

PREVIOUS QUESTIONS WITHOUT RESPONSES Question: I have some concerns of my own, but I'd be very interested in knowing what the rest of the Pedorthic Newswire readership feels is lacking or could be improved as far as the pedorthic industry's educational offerings - both pre-cert and post-cert.

Question: I do not take assignment for therapeutic shoes and inserts from medicare. However, patients still want me to file a claim on their behalf. The reason I do not take assignment is because of all the paper work and documentation. When I file a claim, non assigned for the patient, for therapeutic shoes, I can not use the KX modifier because I do not have all the documentation. But without the KX modifier the claim will be denied. So it is a waste of time to send claim in without KX modifier. Can the patient gather the documentation on his/her own and keep it on file and then fight it out with Medicare? Can I use the KX modifier on behalf of patient and not be held liable by Medicare for repayment if they are not sending me any money in the first place? Or should I just refuse to file claim and let patient do it all? - Scott Torppey, CO, C.Ped.
Question: Does anyone know if custom foot orthotics are excluded in the new healthcare plan or if they are covered? I think if they are excluded it would help the pedorthists that don't bill orthotics, and also would help our businesses so we don't have to compete with other MD, DPM, and PT that bill for foot orthotics. But if they are included in the healthcare plan, then how do we get on the provider list so we can do foot orthotics? Does anyone have any ideas on this? - Rodney Johnson, C. Ped.

One Shoe Can Change Your Life

BABY boomers might be surprised to learn that many of their generation are wearing the wrong size shoes. Overtime, feet can widen or flatten, and fat padding on the sole of the feet can wear down. Weight gain or loss, activity and lifestyle changes, and foot problems can also contribute to changes in shoe fit. Acupuncture Treats Plantar Fasciitis -- New Research
New research proves that acupuncture relieves the pain of plantar fasciitis. Plantar fasciitis is a painful inflammatory condition of the foot. Plantar fasciitis pain is usually perceived on the underside foot/heel and is often most painful with the first footsteps of the day. Also, plantar fasciitis may cause pain upon dorsiflexion (raising the ankle so that the foot is lifted towards the shin).
This randomized, controlled study is interesting in that only one acupuncture point was tested for efficacy. In most studies, a combination of points are used to determine whether or not acupuncture is effective for reducing pain. The study tested point P7 (Daling, "Big Tomb"). P7 is located in the middle of the transverse crease of the wrist between the tendons of the m. palmaris longus and m. flexor carpi radialis on the palmer side of the body. P7 is a Shu-Stream point and Yuan (Primary) acupuncture point that is known for the treatment of Heart and Spirit related issues including myocarditis, palpitations, insomnia, mental illness, irritability, and cardiac pain. P7 is also used for Stomach related conditions, however, nearby point P6 is a more common point for the treatment of Stomach issues such as stomachache, nausea, and vomiting. Full Text

A 12-year-old California boy who pulled on a pair of socks to ease cold feet suddenly got to thinking about the homeless-and socklessness. Parker Ward set up a nonprofit called "Parker's Pairs" and he papered his San Diego County neighborhood with fliers and mounted a website campaign to gather 500 pairs of socks for the homeless. Copper May be the Key to Defeating MRSA

| Diabetes and the FREMS Treatment

Diabetes appears to be nearing epidemic proportions. Because this particular disease produces a constellation of problems, there will be a tremendous strain on our health care system in the years to come. Of course, these financial expenditures are secondary to the suffering and disability that will result.
One of the most common problems associated with diabetes is an opening in the skin, which is called an ulcer. Because the skin is the body's best defense against the invasion of bacteria (which can easily lead to an infection), any opening in the skin is a cause for concern. This is especially true since diabetics have a reduced ability to fight bacteria. Even a tiny crack in the skin can lead to an infection; after all, germs are extremely small and don't need much space to get in. Full Text

| The Sports Doctor: Art to Sports Medicine

In dealing with sports-related injuries, whether it's with a world champion, a high school or college player, a young child athlete or a baby boomer, there is definitely an art to sports medicine. Dealing with never-ending decisions, outlooks and expectations is part of the art. How much time off? When can I return to play? How aggressive can I be?
Doctors, physical therapists, athletic trainers, coaches and parents are some of the group of people making these decisions, and it's far from an exact science. There is often plenty of pressure, especially at the professional and highest levels of sports. We remember Tiger Woods making the decision against his doctor wishes to play the 2008 U.S. Open on his soon-to-be-surgically-reconstructed knee. He gutted it out and even won the tournament, but there were plenty of opinions that questioned the decision....

Mary from Aurora: My 15-year-old son runs cross country and track. His team runs daily over the summer. My concern is they run a lot on sidewalks and concrete. His shins have been hurting. Would softer surfaces make a difference? I've been told his feet have mild pronation by the shoe people, and he wears motion control shoes. It's the third episode of shin problems this past year.
Dr. Weil: Great question! Concrete is the worst surface to run on. Asphalt is much softer for running - of course, prairie paths and grass would be best, but are not always available. Every day I see the kids' teams on the concrete sidewalks running, and your question pops into my mind. I know that traffic and routes available are always a point, but concrete is the most non-forgiving running surface and should be the last choice. The fact your son has excessive pronation (rolling in) and he's had the same problem before, sets off concern. Often orthotic control and strengthening is the best long-term answer to persistent "shin-splint" problems. Full Text

In cooperation with ABC requirements, Eneslow Pedorthic Institute (EPI) is offering the three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, interactive CD's and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at Eneslow Pedorthic Institute for 2 weeks during August 15th, 2011 to August 28th, 2011. This class completes required education prerequisite to sit for national certification exam. It does not satisfy the requirement of 1000 documented hours of field experience.
