Question: Had the 4th patient this month presenting with a prescription for diabetic shoes and inserts. Evaluated, fitted, shoes on order, requested physician statement. Patient has diabetes and foot evaluation shows 2nd toe amputation on the right. Hallux is deformed, healing ulceration mid-foot left.

Certifying physician statement comes back from patient's primary MD stating that "None of the above conditions apply."

I call the patient and (even I had seen it with my own eyes) tell her that according to the statement, she does not qualify. I get an earful of the patient's opinion and am also told that I just lost a customer. The other 3 patients were similar cases. Each patient also stated that their primary had never looked at their feet.

Can anybody explain to me why it is that only a MD or DO can fill out and sign the Physician Statement? I understand why the MD or DO may have to certify that the patient has diabetes and is treated under a comprehensive plan of care for his/her diabetes (which is already not true if this physician is not evaluating the patient's feet).

Why can a DPM, a.k.a. "Foot Doctor" not be the one signing the certification form? Who are those people at CMS that come up with all these ridicules rules and criteria? What can we do to change this?

Comment: It is my opinion, based on my experience in the Pedorthic industry, that one major factor threatening our survival as a profession is the number of good, available Pedorthic jobs. I think this has to do with a crucial point wisely discussed at the 2010 North American Pedorthic Congress in the session 'Don't Just Move a Stone, Build a Cathedral,' where older generation C. Ped's were urged to consider what will happen when and if they can no longer work, decide to retire, get sick or die.
 
As a younger generation C. Ped., I urge the older generation C. Peds to open the doors to us. I cannot tell you how hard it is to find work as a Pedorthist. On behalf of the younger generation pedorthists, I urge you to mentor us, teach us, appreciate our skills and experience, and take us on as employees. Consider it a wise investment...insurance. We want your legacy to continue, we want Pedorthics to continue. We want your store to continue to be successful. We want to learn and add to our skill set.
 
Do you have a back-up plan? Do you have a plan for your future and the future of your business which you have poured your life into? There will come a time when you need to hand over the reins. Start investing in us now. You might be glad you did.

Comment: Our industry is very disorganized, and if it is to survive, we will have to rethink how we fit into healthcare. We are still basically an unrecognized healthcare provider. PFA has not been able to create a strong long-term vision for our profession as it relates to CMS. Our industry is at the mercy of organized practitioners who do not care about the interests of the pedorthic community. I am referring to DPMs, the Pharmacists, and the O&P community.

I believe that we should try to work with the O&P community and join their associations, both nationally and on a state level. The O&P community is better organized and have much better funding than any pedorthic-based group. We should approach ABC and seek their endorsement. ABC is in a position to look at our industry with more depth. They credential C. Peds as well as fitters for shoes. It will be difficult to convince many C. Peds of this idea, but I don't see where we have a choice. PFA has done a poor job and our industry is in peril.

I also think that the shoe industry should work together. My understanding is that they helped create PFA and slowly their input was weakened. The shoe industry has as much to lose as we do. I know that my shoe sales are down, and that can't be good for my vendors. The vendors should put their competitive nature aside and work collectively towards re-energizing the pedorthic community. This statement was not made with malice, but with frustration. We have to get some positive traction soon. If we wait another year to start reorganizing, we will be in a deeper hole than the one we are in now, and in a more vulnerable position.

Comment: Reading the multitude of "issues" when dealing with billing of therapeutic shoes is making my head hurt. I imagine it would be like filling out mortgage paperwork while getting a colonoscopy. I cant imagine spending that much of my day dealing with such nonsense for such little in return. I realize many of us have built our business model around therapeutic shoes and billing. I saw the writing on the wall over 10 years ago and I made the decision to reject that aspect of pedorthics. We are strictly patient pay, but we do provide them with a detailed invoice with the proper codes etc. I encourage them to submit it themselves. If they don't have coverage they can try to apply it to their deductible, or their HSA if they have one.

The only form of billing we do is for auto or work-comp claims. On average, we have approximately 4 to 5 invoices that are past 30 days. Every other patient pays in full before they walk out the door. It appears to me that making therapeutic shoes and billing for them is a PITA of Olympic proportions. I would also predict that the reimbursements will go even lower, while the costs to service this part of the business will go up. No thanks to the charlatans and bottom feeders that have jumped on the bandwagon, the net effect will cause many legitimate providers to evolve or close.

I would encourage other practitioners to cultivate other aspects of pedorthics. Different regions of the country have unique markets one can cater to. I know several C.Peds making solid incomes servicing ski resorts. Many of these resorts switch from ski/snowboard to golf or hiking. Typically these are sports that can be improved instantly with a proper orthotic. Not to mention, those participants have discretionary income. Heavy industrial and manufacturing areas have a great potential for ergonomics, work boots and orthotics.

Many college-based sports programs can be a great source of business. Often times, athletes are fit by trainers that don't have as much experience or time to deal with custom orthotic issues. Try offering them a package where you will do all of their evals, casting and fitting. Most programs have a shoe deal of some sort. It's very common even at the D1 level to have athletes in the wrong size and type of shoe. In most cases, they have not been measured since middle school. Sadly, at many programs it's not much better.  The equipment manager will "ask" the players what size they are. The shoe company will then send them the shoe "they" want them to wear in the size they "think" they are. Even a marginal device in a properly fit shoe will make a big difference.

Again, I realize that many of us can't just shift gears in our business model. That said, I would not count on any board or insurance plan to bail you out, and I certainly wouldn't count on ANY government program either. How many other businesses can you think of where you provide a product/service, but you don't get paid for months or sometimes never? To add insult, you sometimes have to ask several times for payment. Can you tell your auto insurance or cable company that you didn't get the invoice they mailed you? Imagine telling them you are not sure their bill is legit so you are going to take some extra time and review it. How about telling your mortgage company that they didn't put "Mr." in front of your name on the envelope so you are going to hold payment until they correct that error.

Mobile Shoppers Shaking Up Retail Industry

Benjamin DeClue doesn't step foot inside a store without his BlackBerry. As he browses the aisles, he multitasks by keeping one eye on the shelves and one on his smart phone, which he uses to scan bar codes, compare prices, read product reviews and even make purchases. When he spots a book he wants at Borders, he immediately looks up the title on his phone. If he finds a cheaper price on Amazon or another website, he will buy it through his phone while still standing inside the bookstore.

Question: Why is it important for diabetics to have special shoes?

Answer: The statistics speak for themselves. There are 24 million cases of diabetes in the United States. Sixty percent to 70 percent of people with diabetes have some degree of nerve damage. The chances of developing a foot ulcer are as high as 25 percent in these folks. Not only are foot ulcers hard to heal, but they often lead to amputation. There are 60,000 amputations in this country every year, but it has been proven that prevention programs could reduce amputations by as much as 85 percent.

While these shoes are not a miracle cure, we know that improper shoes are the trigger for numerous foot problems. Many times people develop foot complications from wearing shoes that do not fit correctly regardless of the quality of the shoes. CardioVascular BioTherapeutics Submits FDA Application for 'Fast Track' Status for CVBT-141B, CVBT's Biological Treatment for Diabetes Patients

CardioVascular BioTherapeutics, Inc. has announced it has submitted an application to the U.S. FDA to obtain "fast track" designation for CVBT-141B, its biological therapy to treat ischemic diabetic wounds based on the active ingredient fibroblast growth factor-1 (FGF-1).  The FDA grants "fast track" designation to accelerate the development of promising new therapies that treat a serious medical condition and for which there are no available therapies that adequately address the problem.

CVBT believes CVBT-141B meets the FDA's criteria, as ischemic diabetic wounds, if not successfully treated, can lead to infection, sepsis, amputation, and death.  Even with currently-available treatments, there are more than 80,000 foot amputations per year in the diabetic patient population.  CVBT-141B is targeted at reducing the suffering in the diabetes community by closing ischemic diabetic wounds much more rapidly, thereby halting the all-too-common progression to infection, gangrene, amputation, and death. The Affect of Plantar Fasciitis on Athletes

Affecting as many as two million people in the United States every year, plantar fasciitis is a painful swelling and irritation of the thick connective tissue on the bottom of the feet, usually felt in the heel, and worse in the morning when first getting out of bed, that may become degenerative.

Symptoms:

Commonly occurring after long stretches of weight bearing, and high body mass indexes, the first few steps of the day are typically the most painful for sufferers of plantar fasciitis, who also may have problems bending their foot because of decreased ankle dorsiflexions, between the superior surface of the foot and the leg, that bring the toes closer to the shin, restrict passive movements of the skin, and allow transference of shear forces to the skeleton. Plantar fasciitis may also lead to the development of heel spurs, bone califications on the insert of the Achilles tendon and calcaneus heel bone, and inflamed flexor digitorum brevis muscles in the middle of the soles of the feet. Pedorthic Pre-Certification Training Course August 15th - August 28th, 2011

In cooperation with ABC requirements, Eneslow Pedorthic Institute (EPI) is offering the three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, interactive CD's and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at Eneslow Pedorthic Institute for 2 weeks during August 15th, 2011 to August 28th, 2011.
