Comment: Our industry is very disorganized, and if it is to survive, we will have to rethink how we fit into healthcare. We are still basically an unrecognized healthcare provider. PFA has not been able to create a strong long-term vision for our profession as it relates to CMS. Our industry is at the mercy of organized practitioners who do not care about the interests of the pedorthic community.I am referring to DPMs, the Pharmacists, and the O and P community.

I believe that we should try to work with the O and P community and join their associations, both nationally and on a state level. The O and P community is better organized and have much better funding than any pedorthic-based group. We should approach ABC and seek their endorsement.ABC is in a position to look at our industry with more depth. They credential C. Peds as well as fitters for shoes. It will be difficult to convince many C. Peds of this idea, but I don't see where we have a choice. PFA has done a poor job and our industry is in peril.

I also think that the shoe industry should work together. My understanding is that they helped create PFA and slowly their input was weakened. The shoe industry has as much to lose as we do. I know that my shoe sales are down, and that can't be good for my vendors.The vendors should put their competitive nature aside and work collectively towards re-energizing the pedorthic community.
This statement was not made with malice, but with frustration. We have to get some positive traction soon. If we wait another year to start reorganizing, we will be in a deeper hole than the one we are in now, and in a more vulnerable position.

Comment:  I could not agree with "PNW Reader" more [comment from last week's issue] about despensing shoes to patients who are driving the most expensive cars and the living extravagantly to boot, and have the nerve to ask about better style shoes. I have no problems with Medicare putting the screws to us and tightening the rules; it will drive the customers who are in it for "FREE SHOES" out as, they are the ones who don't understand the real reason the program was instituted to begin with, and the unscupilous practitioneers who do not follow Medicare guidelines and also make huge profits.

Do it because you enjoy working in this chosen field and the rewards will come. Hooray for Medicare.

Question: I wonder if anyone else who files to Medicare with paper claims is having the amount of mini audits we are getting? Every one I talk to that files electronically has not had the first mini audit from Medicare. We just got two more in the mail today, which takes the total to 27 audits we have received. Out of those, eight have been denied.

The denials coming back are due to the family physician not documenting the conditions in the patient notes. We now get the notes up front, and if the conditions are not documented in their notes, back they go to the primary physician. Then we have to contact the podiatrist to have them write a new RX, because it now has to be dated after the Certifying Physician Form.

So, what does Medicare have in store for us next? And will we be in business long enough to see it?

Response: For Reggie who had 8 of 27 TSD claims denied, that number works out close to a 30% denial rate.  Factor in the extra personnel time and management focus on responding to audit requests, and any potential for making any profit off of the claims that payment was received vaporized.  Of course I am not telling you anything that you do not know already.  Actually I am impressed that only 30% of claims failed auditing.  Over 75% of our PCPs' chart notes failed our internal auditing process, so we terminated our supplier number at that point rather than risk that level of denials or recoupments.
 
I strongly believe that all C. Peds who are NOT working at an O&P facility or for a DPM that is billing Medicare TSD for these pedorthists to get out of the TSD business and cancel their supplier number immediately.  Leave this risky business to O&P businesses, DPMs, and those big corporations like Wellness Life Systems.  Then Medicare auditors will have to focus on those suppliers.   When these suppliers wake up to the fact that the audits are causing them to lose money being in the TSD business, they will find ways to get out of being TSD suppliers while still supplying custom AFOs or other types of covered benefits that are not targeted by CMS for audits.  Let the other TSD suppliers spend all their time and energy responding to audits.  But at least pedorthists with their own businesses will not be taken down by the total chaos of TSD supply.
 
Once a pedorthist with his/her own business makes the decision to stop being a Medicare supplier, all the drivel and histrionics about PFA's recommendations to Medicare, or CMS picking on pedorthists, or ABC favoring orthotists over pedorthists evaporates as well.  Pedorthists can focus on where our profession should focus, and not on government rules and regulations that change only for the worse.
 
We terminated our number in August, took a 20% hit on revenues, and are slowly rebuilding our business model based on high levels of service to RA patients, peds, foot pain referrals, and our other core customers. People that appreciated pain relief and that we can feel good helping, and that have a life expectancy measured in decades and that cannot easily find solutions to their problems online. Now that's a business model for the future of pedorthists.

Comment: Reading the multitude of "issues" when dealing with billing of therapeutic shoes is making my head hurt. I imagine it would be like filling out mortgage paperwork while getting a colonoscopy. I cant imagine spending that much of my day dealing with such nonsense for such little in return. I realize many of us have built our business model around therapeutic shoes and billing. I saw the writing on the wall over 10 years ago and I made the decision to reject that aspect of pedorthics. We are strictly patient pay, but we do provide them with a detailed invoice with the proper codes etc. I encourage them to submit it themselves. If they don't have coverage they can try to apply it to their deductible, or their HSA if they have one.

The only form of billing we do is for auto or work-comp claims. On average, we have approximately 4 to 5 invoices that are past 30 days. Every other patient pays in full before they walk out the door. It appears to me that making therapeutic shoes and billing for them is a PITA of Olympic proportions. I would also predict that the reimbursements will go even lower, while the costs to service this part of the business will go up. No thanks to the charlatans and bottom feeders that have jumped on the bandwagon, the net effect will cause many legitimate providers to evolve or close.

I would encourage other practitioners to cultivate other aspects of pedorthics. Different regions of the country have unique markets one can cater to. I know several C.Peds making solid incomes servicing ski resorts. Many of these resorts switch from ski/snowboard to golf or hiking. Typically these are sports that can be improved instantly with a proper orthotic. Not to mention, those participants have discretionary income. Heavy industrial and manufacturing areas have a great potential for ergonomics, work boots and orthotics.

Many college-based sports programs can be a great source of business. Often times, athletes are fit by trainers that don't have as much experience or time to deal with custom orthotic issues. Try offering them a package where you will do all of their evals, casting and fitting. Most programs have a shoe deal of some sort. It's very common even at the D1 level to have athletes in the wrong size and type of shoe. In most cases, they have not been measured since middle school. Sadly, at many programs it's not much better.  The equipment manager will "ask" the players what size they are. The shoe company will then send them the shoe "they" want them to wear in the size they "think" they are. Even a marginal device in a properly fit shoe will make a big difference.

Again, I realize that many of us cant just shift gears in our business model. That said, I would not count on any board or insurance plan to bail you out, and I certainly wouldn't count on ANY government program either. How many other businesses can you think of where you provide a product/service, but you don't get paid for months or sometimes never? To add insult, you sometimes have to ask several times for payment. Can you tell your auto insurance or cable company that you didn't get the invoice they mailed you? Imagine telling them you are not sure their bill is legit so you are going to take some extra time and review it. How about telling your mortgage company that they didn't put "Mr." in front of your name on the envelope so you are going to hold payment until they correct that error.

Derma Sciences Says Foot-wound Drug Meets Trial Goal

Derma Sciences Inc said high dose of its experimental diabetic foot-wound drug met the main goal of a mid-stage trial. The Princeton, New Jersey-based company is developing the drug, DSC127, for the treatment of diabetic foot ulcers. Derma Sciences, which specializes in wound-care products, was testing the drug in two dose strengths and one placebo-controlled arm.

The company said the high dose of the drug exceeded the trial's main goal of improving complete healing of wounds within the 12-week duration by at least 15 percentage points, against a dummy drug. Full Text

Scientists develop low cost, nanometer-sized drug to treat chronic wounds

A low cost, nanometer-sized drug to treat chronic wounds, such as diabetic foot ulcers or burns, has been developed by a group of scientists from the Hebrew University of Jerusalem, Harvard Medical School and others in the U.S. and Japan. Diabetes is a rapidly growing medical problem affecting close to 3 percent of the world's population. Poor blood circulation arising from diabetes often results in skin wounds which do not heal, causing pain, infection and at times amputation of limbs.

Several proteins, called growth factors, have been found to speed up the healing process, however purifying these growth factor proteins is very expensive, and they do not last long on the injured site. Full Text

Dear Colin: I'm a 38-year-old runner and have been running since I was 16. I've never really had any serious injuries, but over the past few months I've been getting more and more heel pain in my right foot after running for only 10-15 minutes. My preferred care provider referred me to a podiatrist who diagnosed me with plantar fasciitis, which he says is best treated using a cortisone injection, orthotics, ice, rest and lowering my running speed. Is there anything else I can do to help this? I really don't like running slower ... boring!
