Comment: Reading the multitude of "issues" when dealing with billing of therapeutic shoes is making my head hurt. I imagine it would be like filling out mortgage paperwork while getting a colonoscopy. I cant imagine spending that much of my day dealing with such nonsense for such little in return. I realize many of us have built our business model around therapeutic shoes and billing. I saw the writing on the wall over 10 years ago and I made the decision to reject that aspect of pedorthics. We are strictly patient pay, but we do provide them with a detailed invoice with the proper codes etc. I encourage them to submit it themselves. If they don't have coverage they can try to apply it to their deductible, or their HSA if they have one.
The only form of billing we do is for auto or work-comp claims. On average, we have approximately 4 to 5 invoices that are past 30 days. Every other patient pays in full before they walk out the door. It appears to me that making therapeutic shoes and billing for them is a PITA of Olympic proportions. I would also predict that the reimbursements will go even lower, while the costs to service this part of the business will go up. No thanks to the charlatans and bottom feeders that have jumped on the bandwagon, the net effect will cause many legitimate providers to evolve or close.
I would encourage other practitioners to cultivate other aspects of pedorthics. Different regions of the country have unique markets one can cater to. I know several C.Peds making solid incomes servicing ski resorts. Many of these resorts switch from ski/snowboard to golf or hiking. Typically these are sports that can be improved instantly with a proper orthotic. Not to mention, those participants have discretionary income. Heavy industrial and manufacturing areas have a great potential for ergonomics, work boots and orthotics.
Many college-based sports programs can be a great source of business. Often times, athletes are fit by trainers that don't have as much experience or time to deal with custom orthotic issues. Try offering them a package where you will do all of their evals, casting and fitting. Most programs have a shoe deal of some sort. It's very common even at the D1 level to have athletes in the wrong size and type of shoe. In most cases, they have not been measured since middle school. Sadly, at many programs it's not much better. The equipment manager will "ask" the players what size they are. The shoe company will then send them the shoe "they" want them to wear in the size they "think" they are. Even a marginal device in a properly fit shoe will make a big difference.
Again, I realize that many of us cant just shift gears in our business model. That said, I would not count on any board or insurance plan to bail you out, and I certainly wouldn't count on ANY government program either. How many other businesses can you think of where you provide a product/service, but you don't get paid for months or sometimes never? To add insult, you sometimes have to ask several times for payment. Can you tell your auto insurance or cable company that you didn't get the invoice they mailed you? Imagine telling them you are not sure their bill is legit so you are going to take some extra time and review it. How about telling your mortgage company that they didn't put "Mr." in front of your name on the envelope so you are going to hold payment until they correct that error.

Question: What compensation method works the best for a C. Ped. employee at a pedorthic retail shoe store? We have typically used a straight salary, with an occasional spiff program to get our staff focused on bringing out specific shoes to show customers. We have been reluctant to use an incentive-heavy compensation plan, as we worry that it could put the C. Ped. in a position of having to weigh their financial benefit when making a recommendation to the customer. We would like our staff to generate additional revenues when appropriate, but not come across like some of our competitors that seem to push the most expensive option only, when a customer is asking for our professional advice to address their need.

Response: There are many ways to compensate employees without relying on incentive-heavy compensation plans. Many pedorthic retail sales associates gain great satisfaction from making your customers feel better. There is immense satisfaction derived from substantially reducing or eliminating their customers' pain issues. If you challenge your staff to keep learning, and share your knowledge to help them improve outcomes, they will become personally invested in the success of your pedorthic retail business. A straight salary or commensurate hourly wage can work well, but incentives are always fun. Spiffs are a great way to introduce new brands and to close out old inventory. Games can be fun too. We periodically award a $20 bill for the sales associate that rings the greatest number of units per transaction, and a $20 bill for the single largest transaction rung that day. It can foster friendly competition ("smack talk"), and it gets sales associates thinking about bringing multiple shoes on the floor, and adding socks and shoe care items to the purchase. Your sales associates can also be bonused if monthly revenue goals are achieved or surpassed. If they know what the scorecard is, and they have a roadmap, they can work together to achieve the goal, and collect the bonus. This a good topic. Does anyone else have ideas on this subject?

Question: I wonder if anyone else who files to Medicare with paper claims is having the amount of mini audits we are getting? Every one I talk to that files electronically has not had the first mini audit from Medicare. We just got two more in the mail today, which takes the total to 27 audits we have received. Out of those, eight have been denied.
The denials coming back are due to the family physician not documenting the conditions in the patient notes. We now get the notes up front, and if the conditions are not documented in their notes, back they go to the primary physician. Then we have to contact the podiatrist to have them write a new RX, because it now has to be dated after the Certifying Physician Form.
So, what does Medicare have in store for us next? And will we be in business long enough to see it?

Previous Response: Reading, hearing and experiencing all the hardships that Medicare, "our" Associations and Boards are putting in the C. Peds' way, wants me to ask them to finally just come out with the truth and just admit it: You don't like the C. Peds or the therapeutic footwear bill, or anything else regarding C. Peds, and because you have not found a way yet to just shut us completely down, you give us the slow death by enforcing new restrictions, fees, bonds, education requirements, accreditation', new certifications and requirements and whatever else that has hit the C. Ped. in the last few years. No other group has been hit that hard. Just get it over with and give us the final thrust. - A "tired of it" C. Ped.

New Response: Like most of the folks here, I have noticed all the changes to the Medicare TSB. Simply stated, the government has finally realized that the more hoops they make, the fewer recipients will suffer through the entire process, thereby creating a decrease in funds paid out. The ration czar that was put in place by the President is doing his job. So what does that mean? Us C. Peds have to disconnect ourselves from the government teat, which we have been suckling on for many years now. Let's not mince words here. For the most part, a healthy percentage of the folks that "qualify" for the TSB really don't NEED the overpriced shoes, and the diabetic inserts, (especially pre-fab). And Honestly, do you really feel right with dispensing the three pair of heat gun special inserts? Really? That those pre-fab heat molded inserts are somehow going to stave off the ulcer that will cause an amputation. Yeah, right. Another thing. I'm as conservative politically as they come. But these days, even I am having a hard time biting my tongue dispensing TSB shoes and inserts to someone who drives up in a Jaguar, or BMW 700 series, dressed to the nines, and living in one of his/her million dollar homes. As I drive my 1993 Ford Escort Wagon with 203,000 miles. Don't get me wrong, I don't want to get into class warfare, and I am not suffering from envy, but seriously. Seriously! Should all of us taxpayers be forced to pay for overpriced shoes, and cheap heat gun inserts for folks that "qualify" but are not really, truly, honestly, at risk? Especially given the economic state the government is in? So, face it, we are part of the problem. Not part of the solution. You want to be part of the solution? Go out and market your services to people who find enough value in them to pay out of their own pocket. Do what you love. I have never viewed Pedorthics as a career in which I would become wealthy. God told me to do Pedorthics. I put my faith in Him who gives me all that I have and all that I need to survive according to His will. In conclusion, the state of the TSB is what it is. New Response: Depending on Medicare is a business model that our company has not been interested in. When the accreditation requirements came down, we bowed out. We are interested in doing what is best for our patients/clients. Thankfully, many customers are also interested in having the solutions that are not dictated by Medicare regulations with very low margins and allowances. We are grateful that it was never 20% of our business, since that probably would equate to 80% of the headaches. Excellence is always in demand and a lot more fun!

Shoe Service Institute of America, CFS Allied Health Education Partner on Summer C. Ped. Course
The Shoe Service Institute of America, in partnership with CFS Allied Health Education, would like to announce a C.Ped. course to be presented prior to this summers SSIA convention in St Petersburg, FL. The American Board for Certification has approved a total of 14 CEUs that can be earned for the low cost of $395 during this two-day course.

Stride Rite's Children's Study Sheds New Light on Shoes
Well-fitting, flexible shoes could help children to stumble and fall less when learning to walk, according to a study commissioned by footwear brand Stride Rite. According to the two-year study, The Effects of Shoe Design on Children Learning to Walk, the first step a baby takes could determine their gait of the rest of their lives.

Robofoot Project Aims to Implement Robotic Solutions in Footwear Industry
Basque Tekniker-IK4 technological center announced the launch of a new project that aims to implement robotics into the footwear industry. The Robofoot project, initiated by Tekniker-IK4 consortium, will help in overcoming various manufacturing complications in the footwear sector.
The project will introduce a host of robotized solutions in the upcoming two and a half years to a range of operations that are being undertaken manually. The European Commission data for 2007 stated that the European footwear industry comprises 26,100 companies, which provide employment to 368,000 people. EU's two-third of footwear production takes place in Portugal, Spain, and Italy. Full Text

Facebook Credits a Winning Incentive for Shoebuy

What do espadrilles and FarmVille have in common? Plenty, as far as Shoebuy.com is concerned. The online footwear retailer in November began giving away 50 Facebook Credits with each purchase, making it the first merchant to use them as a sales incentive.
We've been positively surprised at how much attention the program has gotten in the marketplace," says Shoebuy chief marketing officer James Keller, "and we're seeing the volume grow basically with no advertising, so at this point, it's definitely a win."
Shoebuy has promoted the incentive via advertising on Facebook and other Websites; the ads mentioning the Facebook Credits had twice the clickthrough rates as ads that did not.
Facebook Credits are an online currency used for more than 200 Facebook games and apps, including the megapopular FarmVille game. Hosted solution Ifeelgoods manages the promotion for Shoebuy. Full Text

| New Delivery Method for Pressure Wounds and Diabetic Foot Ulcers Improves Healing
Researchers have developed a new growth factor delivery system that heals wounds such as pressure sores and diabetic foot ulcers faster than previous treatments. The scientists found that fabricating nanospheres containing keratinocyte growth factor-the protein that promotes healing-improved healing of deep skin wounds in diabetic laboratory mice when the compounds were suspended in fibrin gel. India: Diabetics with Wounds Spend More
For diabetics, wounds can make a bigger hole in their wallets. Experts at the MV Hospital for Diabetes who did a quick study of medical bills of patients in the hospital say diabetics who have wounded themselves spend at least five times more annually than those who haven't. According to the hospital data, a diabetic spends an average of Rs 30,000 on medication annually while for those with wounds the spending increases to Rs 1.5 lakh. What I Do: Charley Henckler Helps You Have Happy Feet while Skating
Figure skates and hockey skates need to fit correctly, otherwise the wearer ends up with painful pressure points, blisters and heel movement, which are distracting and take focus off of the sport. I played hockey throughout high school in my native New Jersey. I came to Madison to attend the UW and have worked at Middleton Sports & Fitness for almost four years. I was trained as a skate technician by David Patton, who has been my supervisor for three years, and by Jay Hyma.
I replace rivets, eyelets and cracked housing on skates. I also replace blades, which can break from the impact of a hockey puck. To ensure the boot is comfortable, I use a boot stretcher and punch, which stretches the leather in specific places. I also repair hockey sticks. Full Text

Our store hours are 10am to 6pm weekdays and 10am to 5pm Saturdays. This position will require you to be career oriented, a self starter, highly motivated, customer service driven, and strong listening skills. The position requires someone who truly enjoys helping people. You will also possess strong follow-up skills, be goal driven and have a positive attitude. You will be a good problem solver, and have a strong attention to detail. You will have a minimum of 3+ years of face-to-face sales experience and a minimum HS Diploma or GED is required, Bachelors is preferred. Our ideal candidate will have experience in Pedorthics/Cpeds, or a background in Health Management is highly preferred. Competitive Salary commensurate with experience.
