Question: How are other pedorthic retail stores dealing with the growth in Internet sales of shoes?  Has your business started doing e-commerce?  And if so, is it adding profitable revenues to your business?  Our business is feeling price pressure from customers who buy online for other items.  While we can justify pricing based on professional services rendered on the initial purchase, we wonder how to handle the customer that then goes online and when repurchasing the same shoe, asks us to match the online price.  We do offer a repeat customer discount but even with this discount, the Internet sites offer lower prices that do not reflect our overhead and inventory carrying costs.

Response: Web-enabled smart phones and shopping apps make it very easy for customers to compare our prices against our nearest competitor or online discounters. While in-store customer service is still valued by many, others will find a better online deal irresistible, and will opt for savings, free shipping/returns and no sales tax. Once they buy from an online discounter, they are exposed to weekly "deals" on other known, respected brands. It's a serious threat to the independent, brick-and-mortar pedorthic retailer. We either lower our margins to match the discounter, or we can potentially lose that customer.

 

The best defense to this growing trend is to embrace new, emerging "unsaturated" brands that have strong pedorthic benefits, and add them to your assortment. These brands are very hard to shop online for a cheaper price, and have strong word-of-mouth appeal. These are the brands that drive profitability in our store, and we will continue to pursue worthy unsaturated brands in all of our footwear categories. Response: Here's what I do, as we DO offer the same price from the Internet, WITH PROOF that they have the Shoe in stock for shipping. We OFFER the same thing the Internet does...Take an order and Ship/Hand out the shoe!!

 

EXAMPLE:

What size do you want? And then hand them the shoe! We state we WILL Give you the same thing the Internet does...the shoe IN HAND, and that's it! We then explain that what we provide is KNOWLEDGE about the product, KNOWLEDGE about how to FIT the Human Foot and that a "SIZE" is NOT a FIT and a Warm Blooded Human to "listen" about ALL his/her problems. We also explain that EVERY business (Brick and Mortar) must make a small profit to STOCK ALL THE SIZES and pay the rent and heat bill! It's also NICE to have a paycheck too!

Question: As a general rule, when designing foot orthoses for flexible foot conditions we put the rear foot into a neutral position and accommodate the position of the forefoot after the rear foot is corrected. In other words, for a flexible pes plano valgus foot, we post the rear foot with a varus (inner) post.  If the forefoot is then put into a varus position, we put an accommodative varus post to keep the foot in plantagrade.   After managing an orthotics laboratory and doing many trade shows, I have talked to all types of practitioners in the field.  I find that more orthotics with forefoot posts are designed by people in the western part of the country than in the east.  Obviously we are all looking to relieve the discomforts and pain experienced by our patients.  How many practitioners have success when using forefoot posts and how many rarely use them at all?

Response:
I desire to respond to Wade Nield, C. Ped. and also Jerome S. Klein, C. Ped. within the same email with respect. I first read Jerome's question and continued reading the PNW and saw Wade's comment. My mind reflected on my years in working with podiatrists, dating back to the 1970s. Wade did an excellent job of representing the opportunities available in working with podiatrists as part of their network to resolve cases for which they would like to utilize a pedorthist. However, as I reflect on Jerome's comment - here lies an interesting perspective to consider if you are desiring to expand your referral source as a pedorthist to include podiatric physicians. (Orthopedic physicians have little training in the topics presented below, and will usually not require much in-depth theory discussion in trying to secure their business).

 

First to answer Jerome's questions vaguely - Yes, we have success and expect success because we use forefoot posting in every case we see in regards to custom foot orthoses, and also in custom accommodative orthoses as well as applying forefoot posts to OTC products. However, to accurately answer your question, and when marketing to and receiving referrals from the podiatric physician, we must begin to speak in a language that they understand, and as experienced or knowledgeable pedorthists, we must ask questions in a way that will get responses that provide the most insight.  An example of this is by answering Jerome's question in another way which clearly identifies to the podiatrist, who is biomechanically oriented: how, specifically, we are posting the forefoot by using posting techniques within the podiatric biomechanical arena.  If we do not begin to increase our pedorthic knowledge of the history of posting and how to communicate what specifically our products and services do, biomechanically, then we may lose the account with our lack of skill in how to present or communicate the following theories in an effort to achieve optimal anatomical alignment for their patient, when range of motion is available in our attempt to improve patient outcomes. I encourage pedorthists to visit their local or a national podiatric symposium, which will provide the pedorthist (referring physician or allied footcare provider) with an opportunity to explore the various posting techniques provided by the hundreds of nationally known, reputable podiatric laboratories, enhancing the pedorthist's ability to market themselves and improve their own patient outcomes. Question:
We are getting an increasing number of GPs who are signing both the Diabetic Rx and the Statement of Certifying Physician (SCP) for Therapeutic Shoes for Diabetics (TSD).  Up to now, we have been sending two separate documents and requiring two separate signatures.  Of course, there is a lot of duplicate writing involved (name, address, diagnosis, etc).

 

Does anyone have experience with combining these two documents into one demarcated document to improve efficiency for GPs (i.e. on one page but with a line down the middle to show that there is both an Rx and an SCP on the one page)?  By experience, I mean have gone through an audit or a pre-payment review and either been successful with such documentation or specifically failed with such documentation?  Since logic seems to have little to do with the TSD, I am really intent on hearing from people who have been tested through an audit.  If you want to respond directly, please e-mail me at brett.richey@richeyco.com.

Question:
I was wondering how the C. Ped. community is reacting to the Medicare TSD. It seems to me that CMS is being penny wise and pound foolish by making the TSD almost impossible to implement safely. I recently read what PFA had suggested, and I don't agree with most of PFA's suggestions. I am wondering about the reaction from the Pedorthic community on the following:

 

1. Is TSD going the way of the dinosaur?

2. Do you see PFA providing leadership?

3. Have you thought of the possibility of entering into the O and P market?

4. Will you seek to diversify your practice into more retail?

 

These are just some questions that may provoke discussion.

Ruskin Cobbler Collects, Repairs Shoes for the Needy

You could say that shoes run in the Corniello family.  Starting in southern Italy with great-grandfather Nicola, four generations of cobblers have worked the once-dying trade coming back into fashion, given the economy."People are starting to consider shoe repair now, instead of going out and buying a new pair of shoes," said Jeffrey Corniello, owner of The Shoe Lab.

 

The licensed pedorthist has taken the business well beyond what his predecessors may have envisioned. He and his staff of three experienced cobblers not only do traditional repairs, they make new shoes, from scratch, to help folks with medical challenges associated with the feet. XBOX Kinect Injuries Afoot? Doc Says 'Wear Shoes'

Oh dear. It seems the Kinect-related injuries continue unabated as gamers learn the hard way that full-body motion controls do need to be approached with some care. Today brings us the report of a woman who snapped her foot while playing the long jump game in "Kinect Sports." As  KKTV in Colorado reports, 35-year-old Nikki Harding was jumping in her living room (as the game asks you to do) when she landed on her foot and broke it.
