Question: We are getting an increasing number of GPs who are signing both the Diabetic Rx and the Statement of Certifying Physician (SCP) for Therapeutic Shoes for Diabetics (TSD).  Up to now, we have been sending two separate documents and requiring two separate signatures.  Of course, there is a lot of duplicate writing involved (name, address, diagnosis, etc).

 

Does anyone have experience with combining these two documents into one demarcated document to improve efficiency for GPs (i.e. on one page but with a line down the middle to show that there is both an Rx and an SCP on the one page)?  By experience, I mean have gone through an audit or a pre-payment review and either been successful with such documentation or specifically failed with such documentation?  Since logic seems to have little to do with the TSD, I am really intent on hearing from people who have been tested through an audit.  If you want to respond directly, please e-mail me at brett.richey@richeyco.com.

Question: How are other pedorthic retail stores dealing with the growth in Internet sales of shoes?  Has your business started doing e-commerce?  And if so, is it adding profitable revenues to your business?  Our business is feeling price pressure from customers who buy online for other items.  While we can justify pricing based on professional services rendered on the initial purchase, we wonder how to handle the customer that then goes online and when repurchasing the same shoe, asks us to match the online price.  We do offer a repeat customer discount but even with this discount, the Internet sites offer lower prices that do not reflect our overhead and inventory carrying costs.

Question: As a general rule, when designing foot orthoses for flexible foot conditions we put the rear foot into a neutral position and accommodate the position of the forefoot after the rear foot is corrected. In other words, for a flexible pes plano valgus foot, we post the rear foot with a varus (inner) post.  If the forefoot is then put into a varus position, we put an accommodative varus post to keep the foot in plantagrade.   After managing an orthotics laboratory and doing many trade shows, I have talked to all types of practitioners in the field.  I find that more orthotics with forefoot posts are designed by people in the western part of the country than in the east.  Obviously we are all looking to relieve the discomforts and pain experienced by our patients.  How many practitioners have success when using forefoot posts and how many rarely use them at all?

Response: At Comfort Shoe Specialsits we do forefoot posts on a lot of our patients.  For pes planto valgus we post the rear foot, then if the forefoot is still in varus we do a forefoot post and forefoot varus wedge.  We have great success with this formula.

Question: The relationship between pedorthists and podiatrists has been controversial since I joined Eneslow 37 years ago. Recently we had an education event and 40 DPMs showed up to learn more about ready made and custom rockers. As a result, a number of attendees started prescribing while others made it clear they would not send their patients to a pedorthist. We continue to build our relationships with the DPMs in our area. What are other practitioners experiencing?

Response: First of all, I've always scratched my head about some of the negative comments about Podiatrists I've read in this forum over the years. Candidly, if it were not for the excellent referral relationships I have with Podiatrists, I would not be in business. We are now in our 9th year of practice and almost 100% of my business is referred (or repeat business of patients who were originally referred).  And the range of patients referred covers the entire comprehensive Pedorthic modalities, not just diabetic shoes. Before I actually opened my business I personally visited nearly every Podiatrist in my market area. One of the very first doctors I called on gave me some very sage advice. He told me "don't try to compete with him and to simply be a resource for him and his patients." I took that advice to heart and have not looked back. I frequently receive telephone calls from Podiatrists describing a particular problem one of their patient's is having and asking for advice. More than once I've been visiting a practice and been pulled into an exam room to look at someone's feet and shoes. I never look at a Podiatrist as a competitor, even those who dispense diabetic shoes. They will inevitably find patients who are too difficult to fit, have extreme issues beyond their comfort level or require custom footwear that is too time consuming. If this cooperative approach is not your particular mindset, my advice would be to get up every morning, look in the mirror and recite "Podiatrists are my friends"...then go out there and start building those relationships, you'll be surprised at what will develop!

Question: I was wondering how the C. Ped. community is reacting to the Medicare TSD. It seems to me that CMS is being penny wise and pound foolish by making the TSD almost impossible to implement safely. I recently read what PFA had suggested, and I don't agree with most of PFA's suggestions. I am wondering about the reaction from the Pedorthic community on the following:

 

1. Is TSD going the way of the dinosaur?

2. Do you see PFA providing leadership?

3. Have you thought of the possibility of entering into the O and P market?

4. Will you seek to diversify your practice into more retail?

 

These are just some questions that may provoke discussion.

Biolyst wants $1m Plus to Franchise Laser Treatment for Nerve Disease

Biolyst LLC is seeking $1.25 million from the sale of equity and warrants, according to documents filed with the Securities and Exchange Commission. The company, based in Plymouth, Minnesota, wants to franchise a laser system that treats peripheral neuropathy, a painful nerve disease. Biolyst, which launches the system this quarter, is targeting 30 chiropractor and podiatry clinics a year, or about 200 clinics in five years, CEO Phil Walter said in a telephone interview.

 

Peripheral neuropathy is caused by nerve damage resulting from diabetes, trauma, and infections, according to Mayo Clinic. Full Text

Diabetes Takes Toll on Health Costs

And the obesity-related disease is on the rise, increasing by almost 15 percent from 2000 to 2004 and taking a greater toll on older people in Sonoma County and across the state, according to a report by California's Office of Statewide Health Planning and Development.
