Question: The relationship between pedorthists and podiatrists has been controversial since I joined Eneslow 37 years ago. Recently we had an education event and 40 DPMs showed up to learn more about ready made and custom rockers. As a result, a number of attendees started prescribing while others made it clear they would not send their patients to a pedorthist. We continue to build our relationships with the DPMs in our area. What are other practitioners experiencing?

Question: A few years back, I was asked to sit in on a meeting of education professionals (as a substitute for my coworker at the OSU O & P dept) in Chicago that was to determine the best future route for O & P education to take. Today's Masters Degree requirements are the ultimate result of that early meeting. Since then, I have wondered what effect increasing the educational requirements would have on new and existing schools, and the profession itself. The answer came in a recently published article. So, my question is, has anyone besides myself looked at the latest article in the O & P Edge (Sustainable O&P Education: Are O&P Academic Programs at Risk? http://www.oandp.com/articles/2010-12_01.asp  Dec. 2010) and is this a parallel predictor for the future of our pedorthic schools/profession as ever looming new requirements are put into place in the coming year?

Response: It is my belief that the increase in educational requirements was not really thought out by the members sitting on the Board.  The people who thought up these requirements had grand ideas that were not based in reality. By increasing the requirements for education, several unexpected (frankly, stupid that they were unexpected, or at least naive) results occurred. First by increasing the education it was made more expensive, and as a result less people could really afford the education.  Second, by the increase it made the ideas of degrees more daunting to people.  Third, it made it harder for schools to set up reasonable programs.  Because of these results, what should have been growing professions instead are in real danger of severely shrinking. 

 

So what does the Board do?  They try to mitigate the problem by creating the Certified Orthotic Fitter or the Certified Therapeutic Shoe Fitter.  What this does is create a situation where you need to decide to become a certified Orth/Pros/Ped at an early age.  Go into a lot of debt and get an advanced degree that is not, has not and will never be, needed.  Why do you have to have a Masters Degree to be an Orthotist?  Why do you really need to have a college degree in the first place?  I have a B.S. in Business.  Does an Orthotist need two years of undergrad work to learn to make braces? 

 

I have been a C. Ped. for almost two decades.  I can tell you now that you do not need 25 weeks of internship plus the 120 hours of education courses to learn this profession. 

 

That being said, it is impossible to un-ring a bell.  Now it is too late.  The only option we have is to figure out a way to make getting the education easier and cheaper.  A great majority of the courses should be available over the Internet.  And shop program can be run on 40-hour, 1-week intensive programs.  But you have to convince both the schools and the Boards to go in this direction.

Question: I was wondering how the C. Ped. community is reacting to the Medicare TSD. It seems to me that CMS is being penny wise and pound foolish by making the TSD almost impossible to implement safely. I recently read what PFA had suggested, and I don't agree with most of PFA's suggestions. I am wondering about the reaction from the Pedorthic community on the following:


1. Is TSD going the way of the dinosaur?

2. Do you see PFA providing leadership?

3. Have you thought of the possibility of entering into the O and P market?

4. Will you seek to diversify your practice into more retail?


These are just some questions that may provoke discussion.

Co-Founder of the RMPI Pedorthic Pre-Certification School, passes away December 26, 2010


In honor of Robert M. Palmer, M.D., who passed away on December 26, 2010, the faculty, adjunct faculty, staff and volunteers would like share the accomplishments of this remarkable physician. He was credited in the world with finding the genetic aspects of clubfeet and upon his retirement embarked on a mission to enhance the educational opportunities to aspiring pedorthists and allied health professionals who shared his level of commitment and passion for treating foot and ankle musculoskeletal alignments. He was the founding orthopedic volunteer during the conception of the prestigious Indiana University School Of Medicine Indianapolis location - IUPUI and was the first orthopedic surgeon under orthopedic employment at IUPUI. In 1996, during his retirement, he served as a volunteer to the development of the first pedorthic biomechancic school within the USA. In 2002, Dr. Palmer was approached and agreed to offer his name and career accomplishments as a token of his support for the pedorthic field of medicine; as the co-founder of the The Robert M. Retail Sales Rebound, Beating Forecasts

Shoppers spent more money this holiday season than even before the recession, according to preliminary retail data.  After a 6 percent free fall in 2008 and a 4 percent uptick last year, retail spending rose 5.5 percent in the 50 days before Christmas, exceeding even the more optimistic forecasts, according to MasterCard Advisors SpendingPulse, which tracks retail spending.  Full Text

Flat, Casual Footwear is Gaining Traction


The shoeless craze captivated the marathon crowd first, with books promoting barefoot running and even "shoes" that fit the foot - toes and all - like a glove. And now, the trend is stepping into the land of the rest of us - the walking crowd.


Pedestrians are shunning things with heels and curvaceous inner soles and stiff leather sides. Instead, they want something to protect the foot and keep it warm, and that's about it. "As we continue to see the casualization of America grow and grow and grow, it has reached the footwear business to the degree of, 'How more casual can you get than running shoes?,'" said Marshal Cohen, chief industry analyst with the NPD Group, a market research firm. Crocs No Longer Saying Shoes Are Antimicrobial


Regulators say Crocs Inc. has agreed to stop saying its brightly colored, lightweight clogs are antimicrobial and to pay $230,000 to resolve cases involving those claims.


Federal law considers products that kill or repel bacteria or germs to be pesticides, and those products have to be registered with the U.S. Environmental Protection Agency before a company can claim that they control germs. Full Text

 Avera Medical Minute: Diabetic Woman with Charcot Foot


If you have diabetes, but don't keep it under control there are a number of negative side effects, even deadly consequences that can result from it. Nancy Naeve Brown went to Yankton and met a woman from Springfield who credits her foot doctor at Avera Sacred Heart for saving her foot, her leg, and possibly her life.

 

Marilyn Aase from Springfield went to Avera Sacred Heart Hospital to find out why her left leg hurt so badly. Doctors diagnosed her with arthritis and consulted Dr. Scott Torness with the Avera Foot and Ankle Clinic about an infected ulcer on the other leg. Marilyn is diabetic and has severe neuropathy in that foot.

 Marilyn says, "I can't feel anything in the better part of the top of my foot. I was walking around thinking I'm fine."
