Question: I was wondering how the C. Ped. community is reacting to the Medicare TSD. It seems to me that CMS is being penny wise and pound foolish by making the TSD almost impossible to implement safely. I recently read what PFA had suggested, and I don't agree with most of PFA's suggestions. I am wondering about the reaction from the Pedorthic community on the following:


1. Is TSD going the way of the dinosaur?

2. Do you see PFA providing leadership?

3. Have you thought of the possibility of entering into the O and P market?

4. Will you seek to diversify your practice into more retail?


These are just some questions that may provoke discussion.

Question: A few years back, I was asked to sit in on a meeting of education professionals (as a substitute for my coworker at the OSU O&P dept) in Chicago that was to determine the best future route for O&P education to take. Today's Masters Degree requirements are the ultimate result of that early meeting. Since then, I have wondered what effect increasing the educational requirements would have on new and existing schools, and the profession itself. The answer came in a recently published article. So, my question is, has anyone besides myself looked at the latest article in the O&P Edge (Sustainable O&P Education: Are O&P Academic Programs at Risk? http://www.oandp.com/articles/2010-12_01.asp  Dec. 2010) and is this a parallel predictor for the future of our pedorthic schools/profession as ever looming new requirements are put into place in the coming year?

Question: How are other pedorthic retail business owners handling special orders of shoes that they do not normally carry? Do you require a full payment up front with no refund? Or establish before ordering that special order shoes can be exchanged at no additional cost if there is a fitting issue? When a new customer comes in looking for a specific style from a brand we carry, we want to be customer friendly, but know there is an inherent risk in ordering in shoes that we have no fitting inventory or history fitting. What policy works to be fair to store and customer on special orders?

Response: We have strict terms of sale and return policies. If not, our inventory would be OOC. We collect for all special orders in full up front, but occasionally make exceptions for good cause. If we carry the model and are just out of the size, we bring in as a "trial fit" with no restocking or exchange fee. We don't mind filling in sizes that may be missing. BUT if we do not carry the model, then we will order ONLY with the stipulation that they are responsible for shipping costs (to and from) and the shoes are again paid for in full before the order is placed.

PREVIOUS QUESTIONS AND COMMENTS WITHOUT RESPONSES




Question: I recently posted a query regarding the increase in neutral cushion for runners i've seen that have softer lateral outsoles and firmer medial outsoles. I got a response outlining how to make inserts for that foot type. I was hoping that someone in the know would respond and address that issue. I've seen it in some Asics's, New Balance and to a lesser degree some of the other major brands. Surely someone that is a member of this newswire is tied in to one of the companies? It's a bit tough seeing a patient with even subtle cavus crushing the lateral outsole of a very expensive shoe with little to no miles. Perhaps there is something I don't grasp, but having a shoe that encourages varus tipping of the foot and increases varus thrust of the knee doesn't seem like sound biomechanical theory.

Consumer Spending is Up: Are Americans Enjoying a Post-Recession Holiday?

 

Consumers may have been in a post-recession spending mood this Christmas season. Spending is up in all retail categories compared with last year, according to MasterCard Advisors' SpendingPulse, which tracks spending on all transactions including cash. Analysts say the increase in personal savings and a decline in consumer debt are giving consumers more confidence than they had in recent years to spend on others and even themselves.

New Medicare Rules on Medical Supplies Aim to Save

A federal crackdown on Medicare fraud and overspending will force South Florida seniors to think twice about where they buy walkers, motorized wheelchairs, mail-order diabetes supplies and other equipment.

 

Starting Jan. 1, Medicare will pay only for certain equipment that comes from a vendor who has been screened and selected through competitive bidding. Affected items include feeding-tube supplies, mail-order diabetic supplies, oxygen equipment, respiratory devices, mattresses, hospital beds and motorized scooters. Full Text

As the house boot-fitter at Great Northern Cycles in Whitefish, Duie Millette knows more about feet than the average non-podiatrist should. He can rattle off scientific names of the dozens of muscles and bones in a foot as if he's reading off an anatomy chart - but that's precisely what makes him one of the best in the business.

 

"He's probably just a step down from being a doctor," local ski instructor Lori Rust joked.

 

Rust recently had Millette customize the fit of her ski boots, and she says there's "no doubt" it's made a difference in her abilities and comfort on the slopes....

In cooperation with the ABC and BOC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute. The course includes the required tests for competency to sit for the ABC or BOC exams.

 

Upon completing the course and successfully passing the competency tests, and fulfilling the 1,000 hour "Clinical practice Requirement" students are eligible to sit for the ABC or BOC exam to earn their Pedorthic Certification (CPED or BOC Pedorthist).
