 House and Senate pass HR 4994,

the Medicare and Medicaid Extenders Act of 2010


 


In a rare show of bipartisanship, both the House and Senate passed HR 4994, the Medicare and Medicaid Extenders Act of 2010, legislation that extends current Medicare reimbursement payment to physicians through the end of 2011, and averts a 25-percent pay cut scheduled for January 1.

The bill, approved by voice vote in the Senate on December 8 and by a vote of 409-2 (
Roll No. 626) in the House just after 1 p.m. today, is expected to be signed by President Obama. See the rest of this alert here.

Question: How are other pedorthic retail business owners handling special orders of shoes that they do not normally carry?   Do you require a full payment up front with no refund?  Or establish before ordering that special order shoes can be exchanged at no additional cost if there is a fitting issue?  When a new customer comes in looking for a specific style from a brand we carry, we want to be customer friendly, but know there is an inherent risk in ordering in shoes that we have no fitting inventory or history fitting.  What policy works to be fair to store and customer on special orders?

Question: I recently posted a query regarding the increase in neutral cushion for runners i've seen that have softer lateral outsoles and firmer medial outsoles. I got a response outlining how to make inserts for that foot type. I was hoping that someone in the know would respond and address that issue. I've seen it in some Asic's, New Balance and to a lesser degree some of the other major brands. Surely someone that is a member of this newswire is tied in to one of the companies? It's a bit tough seeing a patient with even subtle cavus crushing the lateral outsole of a very expensive shoe with little to no miles. Perhaps there is something I don't grasp, but having a shoe that encourages varus tipping of the foot and increases varus thrust of the knee doesn't seem like sound biomechanical theory.

Question: Does anyone have recommendations for shoes and/or sandals (not flip-flop style) with good arch support for someone who suffers from plantar fasciitis but prefers to buy shoes that aren't made with animal materials such as leather? In shoes, she does use high quality over-the-counter orthotics to help support the arch of the foot.

Question: What are PNW reader's general rules of thumb regarding heel height and its effect on the foot and body? Some say that shoes with a heel that's two inches or less is appropriate. The higher you go, the more problematic it gets. What do you think? How do you compensate for high heel shoes to make them more comfortable?

Response: Generally speaking, the higher the heel height, the more unstable the mid-foot becomes, specifically the subtalar joint. Moving up the kinetic chain, the pre-patellar area of the knee receives greater stress, in particular the patellar tendon. The pelvis is tipped forward, increasing the lumbar lordosis. Some bodies can tolerate these stresses and not have biomechanical pain. Others cannot handle these stresses and tissues will become inflamed at each of the sites mentioned. Sprained ankles, metatarsalgia, increased pronation and mid-foot pain will occur. Patellar tendonitis will commonly occur at the knee.  If the customer has underlying knee problems such as anterior cruciate laxity or meniscal degeneration, these issues can be aggravated. Those with underlying back problems such as spinal stenosis or facet degeneration could find these conditions worsening. The challenge is that customers' anatomy varies greatly and a 1 1/2" heel height or even lower could aggravate or cause all of these conditions. Some customers will share, without being asked, that they have been having XYZ problem, and that can alert you to be observant.  Others won't know, and you won't know until they return with complaints. Careful customer observation may be the most useful tool if a history is not forthcoming from the customer.

Response: Nobody needs more than 2 inches in heel height...but women want it. To reduce forefoot forces, we raise the forefoot as much as possible, sculpting aesthetically.  We call it heels to rocker sole comfort: cushions and protects metatarsal heads while keeping the heel height. We used to lower the heels, but frankly this is faster and easier and women prefer keeping those heels! When the shoes are on the ground, the modification goes away. But, hey, platforms are in. Why? For the comfort! Vibram has some nice pattern soling that works beautifully, cushioning and slip resistant.

Comment: I sympathize with J. Watkins from Georgia [who wrote in seeking an internship, issue #392]. I hope someone stepped up to help him. We in Ohio are a licensure state also. I have been a C. Ped./LPed for many years and have worked with several C. Peds taking the steps to becoming licensed in Ohio.


I just wanted to put it out there to people in southwest Ohio needing an internship that I will work with you. From clinical to lab, we do it all.


I hear from other C. Ped's (some I have worked with to get licensed) that they would not assist other's needing help with this last hurdle, that it's bad business. I think differently. I truly believe we need to stick together to grow our profession. I would rather be friends with my competition and know what they are up to. Trust me, there are plenty of feet to go around!

Question: Just wondering if I could get some advice on a patient of mine. She is a roller derby skater and has plantar fasciitis. The doctor has prescribed custom foot orthotics for her street shoes and her skates. The skates, at present moment, do not have even a sockliner as it has delaminated, exposing the hard base. From what she has told me, they always skate counter clolckwise around the track. She wears no socks most of the time because they tend to twist and become bunched up from the constant pushing of the feet within the skates. She will apply duct tape to certain areas to prevent blisters, and believe it or not does not present with any callusing. Does anyone have experience with a case like this?

Response: I've worked with speed skaters, but usually there is a very thin RI.  If not, I glue additional support into the boots.  IF she has forefoot varus, then scaphoid with extension lifting the first met head will reduce pull on the plantar fascia as she pushes off.  Glue well, since there will be tremendous force to dislodge.

The shoeless craze captivated the marathon crowd first, with books promoting barefoot running and even "shoes" that fit the foot - toes and all - like a glove.

And now, the trend is stepping into the land of the rest of us - the walking crowd.


Pedestrians are shunning things with heels and curvaceous inner soles and stiff leather sides. Instead, they want something to protect the foot and keep it warm, and that's about it.

Winter Boots Cause Bother For Motorists, Warns Swinton

With the snow set to continue falling across the UK, Swinton Insurance is warning motorists to ensure they are wearing the correct footwear behind the wheel to avoid an accident.

 

The UK's leading high street retailer of car insurance issued the warning after an online survey of 1,200 customers found that 15% had admitted to momentarily losing control of the car as a result of wearing thick soled winter boots. Wearing thick-soled winter boots can cause the driver to step on both the accelerator and brake together and prevents the driver from feeling the correct amount of pressure being applied to the pedal. Researchers Discover Molecular Function that Increases Type 2 Diabetes Risk

 

In a study that could have major implications for the treatment of metabolic disease like type 2 diabetes, a team of researchers from the Harvard School of Public Health has discovered a molecular switch that can turn off cellular mechanisms that protect against aging and metabolic dysfunctions.

The team reported in the journal Cell Metabolism that the SMRT protein accumulates more on older cells. This increases the effects of oxidative stress on the cells' mitochondria, which are responsible for the energy production and metabolic functions of cells. This damage increases the likelihood of developing type 2 diabetes.

Additionally, SMRT turns off the production of other proteins that protect against oxidative stress and promote the burning of fat. Full Text

Pedorthic Pre-Certification Training Course January 24th - February 7th, 2011

 

In cooperation with the ABC and BOC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute. The course includes the required tests for competency to sit for the ABC or BOC exams.

 

Upon completing the course and successfully passing the competency tests, and fulfilling the 1,000 hour "Clinical practice Requirement" students are eligible to sit for the ABC or BOC exam to earn their Pedorthic Certification (CPED or BOC Pedorthist).
