Question: Just wondering if I could get some advice on a patient of mine. She is a roller derby skater and has plantar fasciitis. The doctor has prescribed custom foot orthotics for her street shoes and her skates. The skates, at present moment, do not have even a sockliner as it has delaminated, exposing the hard base. From what she has told me, they always skate counter clolckwise around the track. She wears no socks most of the time because they tend to twist and become bunched up from the constant pushing of the feet within the skates. She will apply duct tape to certain areas to prevent blisters, and believe it or not does not present with any callusing. Does anyone have experience with a case like this?

Question: What are PNW reader's general rules of thumb regarding heel height and its effect on the foot and body? Some say that shoes with a heel that's two inches or less is appropriate. The higher you go, the more problematic it gets. What do you think? How do you compensate for high heel shoes to make them more comfortable?

Comment: I sympathize with J. Watkins from Georgia [who wrote in seeking an internship, issue #392]. I hope someone stepped up to help him. We in Ohio are a licensure state also. I have been a C. Ped./LPed for many years and have worked with several C. Peds taking the steps to becoming licensed in Ohio.


I just wanted to put it out there to people in southwest Ohio needing an internship that I will work with you. From clinical to lab, we do it all.


I hear from other C. Ped's (some I have worked with to get licensed) that they would not assist other's needing help with this last hurdle, that it's bad business. I think differently. I truly believe we need to stick together to grow our profession. I would rather be friends with my competition and know what they are up to. Trust me, there are plenty of feet to go around!

Question: Does anyone have recommendations for shoes and/or sandals (not flip-flop style) with good arch support for someone who suffers from plantar fasciitis but prefers to buy shoes that aren't made with animal materials such as leather?  In shoes, she does use high quality over-the-counter orthotics to help support the arch of the foot.

Response: My office has been selling Orthaheel Sandals, flip flop style, for this past summer.  Although we do not normally carry any sandals in our office, I was impressed with the arch profile and foot bed. I sampled a few pair for my family and friends and found they were exceptionally comfortable, so I began to stock a few styles and colors.  Wow!  They flew out of the office and began to generate their own sales by word of mouth!  If any patient came in for any type of orthotic we would mention the sandal as an add on, and 9 out of 10 times it was an additional sale.  The reason I am mentioning this product is it is now being made without the thong between the toes, although this never was an issue for fit.  The price point averages out to be around $60.00.  I would highly recommend you taking a look see at this brand based on my success with this product.

Comment: I don't know whether to laugh or get angry about this one. I had a new diabetic shoe patient come in with a signed Statement of Medical Necessity from her physician. The physician checked off "history of pre-ulcerative callus" and "foot deformity." The form was signed and dated.


But wait! On the right hand side of the form was written in large handwriting, "Patient has no history of any of these conditions. She has been wearing diabetic therapeutic shoes for the past 3-4 years. This has prevented any of these conditions from occurring".


I called the physician's office in order to clarify this. "That's right," the nurse said. "This patient doesn't have any of those conditions and there's nothing in her chart notes."


I think this emphasizes the fact it's the physicians who need to be audited, not the suppliers!! Talk about being put in a difficult position as the supplier...please doctors and staff...help us out, this is not rocket science!

Index Bodes Well for the Holidays

The economic malaise and an unemployment rate of about 10 percent in the last quarter of 2009 forced merchants to slash their prices to lure shoppers, but in many cases they still fell short of their sales goals.

 

This time, however, there may be more reason for optimism, based on results of the Thomson Reuters/University of Michigan preliminary sentiment index, which shows that the outlook of U.S. consumers improved in November to its highest level since June. Remember that old expression "walk a mile in my shoes?" That's exactly what Donald Myers has been thinking for more than two decades. Since the late 1980s, Myers, who runs New Shoes Professional Shoe Recrafters and Pedorthic Center in Tigard has been collecting gently worn shoes as part of his Shoes for the Shoeless program. Once collected, the shoes are donated to the Portland Rescue Mission to be given to Portland's homeless population. Full Text

Foot Pain (with Video)

 

Thanks in part to simple gravity, feet swell over the course of the day, but this can cause or irritate many conditions leading to foot pain. Dr. John Doolan, a podiatrist, visited Good Day to talk about how to identify and treat some of the most common kinds of foot pain. Feet should be snug in the shoe, but still have a bit of "breathing room."

 

The number one most common complaint is plantar fasciitis, which is incredibly painful and beyond being the most common complaint in Dr. Doolan's practice, it is incredibly common. It's synonymous with heel spurs and is the inflammation of the ligaments in the heel. Full Text

Shocking Variations in Diabetes Care Across UK


Almost a quarter of people with diabetes are not getting the foot checks they need, and diabetes results in about 70 amputations a week in England, 80% of which were potentially preventable, shows a Government report out today. The NHS atlas of variation in healthcare shows that the amputation rate varies twofold between different strategic health authorities (SHAs).


But in areas with integrated multidisciplinary specialist diabetes foot teams (MDT), the amputation rate has fallen dramatically. In Ipswich, it fell over an 11-year period to only a fifth of the rate before the MDT was established. Full Text

As a person living with Type 1 diabetes Angie Hashemi-Rad must prick her fingers and give herself every day to stay alive. But nothing irritates her more than having people mistakenly assume she has Type 2 diabetes - and then suggest she "cure herself" by eating less sugar and exercising more.

 

"I'm sorry, but I hate Type 2. I call it the wuss version," Hashemi-Rad, 34, wrote on diabetesdaily.com in response to an online article headlined: "Which is worse: Type 1 or Type 2 diabetes?" "NO TYPE 2 COULD EVER DO WHAT I HAVE DONE FOR THE PAST 28 YEARS," she wrote. "IT IS NOT THE SAME. NOT EVEN CLOSE. HOW DARE YOU."

 

As rates of Type 2 diabetes soar, tempers are flaring in the diabetes blogosphere, where many people with Type 1 diabetes are lobbying for a new, distinct name for their condition in hopes of clearing up misconceptions and securing more resources to put toward a cure. Full Text

I crashed on my bike on Oct. 3 and broke my collarbone, an experience so horrific that my first impulse was to say I would never ride on the road again. Turns out I am not alone.

To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute. The course includes the required tests for competency to sit for the ABC or BOC exams.

Experience clinical settings in Orthopaedic, Podiatric, Chiropractic and Physical Therapy Clinics as well as experience in a BAPFOL approved functional foot orthosis laboratory. We specialize in diabetic wound care as well as pediatric and adult biomechanics. Your internship hours can earn you an Associates Degree In Pedorthic Biomechanics or be used as continuing medical education hours. The Robert M. Palmer, M.D., Institute of Biomechanics, Inc. a not-for-profit pedorthic school: Advanced Manufacturing - 50 Hour Course - Approved For 49.5 CME's for Pedorthists by the ABC. This course is designed to teach BAPFOL standards in manufacturing functional feet orthoses and provide an introduction to advanced biomechanical posting. Class size is limited to two students.
