Question: I see a large number of cavus feet in my facility. While it's common to see them in pronation control or other inappropriate shoes, I've seen a "trend" in running shoes that has me scratching my head. Biomechanics 101 tells us that people with cavus, supinators, under-pronators, locked-foot (pick your favorite term) tend to strike harder on the lateral heel. They also tend to overload the lateral column. I have observed high-end "neutral cushion" shoes from several manufacturers that have gel or visco polymer pods or wedges in the outsole under the lateral heel.
Many of these shoes also have a thicker and higher durometer outsole on the medial aspect. This would be called a varus wedge if my memory is correct. I suspect the engineers' belief is that hi-tech gels and polymers are better at attenuating impact? I've seen many patients crushing the lateral aspect of these shoes when they are almost new with very little to no mileage on them. This can cause peroneal tendinitis, illiotibial band syndrome, medial compartment overload of the knee as well as lateral ankle instability.
I thought the theory behind neutral cushion was that the shoe is neutral AND cushioned? Some of these shoes are listed as such and they cost well over $120 retail, yet they clearly are nothing more than a giant varus wedge. Fitting them on a patient with cavus or high degree of tibial varum is like putting reading glasses on someone that is near sighted...not the proper correction. Perhaps someone on the design/engineering side of the industry can shed some light on this, because I just don't get it.
Maybe during controlled testing in labs, these material and design combos perform admirably. In the real world, when you factor in center of gravity, tibial angle, ground reaction, body weight, terrain...not to mention foot type...many of these shoes fail miserably. This is what I call "over-shoeing" a patient, and it has become more common in the last few years. This also raises the question of why some runners seem to do well in minimalist or neutered shoes as I like to call them. Personally, I am not a fan of running in what amounts to training flats or less. I have read several accounts where runners say they got rid of their big running shoes and orthotics in favor of stripped down shoes and they actually did better.
It may not be that they were simply running the way God made them. Perhaps what really happened is they were no longer "over-shod" and their under-pronating foot was allowed to function a bit more normally? As always your thoughts and comments are most welcome. Please be gentle with your replies.

Question: Does anyone have recommendations for shoes and/or sandals (not flip-flop style) with good arch support for someone who suffers from plantar fasciitis but prefers to buy shoes that aren't made with animal materials such as leather? In shoes, she does use high quality over-the-counter orthotics to help support the arch of the foot.

Response: We are pleased with the Chaco brand sandals for providing excellent arch support. Also, Sole sandals (they are flip-flops) have an excellent foot bed with great support. Earth has several vegan choices in shoes that would be good.

Original Question: Am I missing something? How are other practitioners fabricating their diabetic fo's? The descriptor for A5513 is "Diabetic multiple density insert, custom molded from model of patient foot," and the descriptor for L3020 is "Foot insert, removable, molded to patient model, longitudinal/metatarsal support, each." To me, there really is no difference here, with the exception of the "longitudinal/metatarsal support." Obviously, the A5513 has the same features, and if there are labs fabricating "custom molded" diabetic fo's by heating up a prefab and "forming" it over a patient's mold, they are not a true custom fo, as they do not take on the shape of the patient's feet. I fabricate my A5513 fo's with trilam (with a pzote or pcell topcover), with either an eva or cork archfill if needed for heavier or more active patients. If I were to fabricate an L3020 for a diabetic patient, it would be built pretty much the same way. Doesn't a diabetic need to have a soft conforming top cover to accommodate prominent bones and high pressure areas? Wouldn't you use the same type of top cover for an L3020 that you are giving to a diabetic? So, what is the difference between the A5513 and L3020? To me, losing a patient and knowing that they might go elsewhere and receive substandard care is not something that I am OK with. An accommodative orthotic is an accommodative orthotic. Whether it is a diabetic orthotic or an accommodative orthotic for diabetes or arthritis, it's pretty much fabricated the same way (with the exception of some different materials). I think it all boils down to reimbursement. Don't get me wrong, I am not trying to reinvent the wheel or stir the pot, but to hopefully gain a better understanding as to why, in our small profession, we do things so differently. Let's try to share our ideas and constructive criticism in a positive manner and come up with positive solutions - not just for pedorthists, but also for our patients. - Joe Eads, C. Ped., COF
Previous Response: Joe, you MAY be missing a key point. I do not mean this disrespectfully, but it is a key nuance. I will oversimplify to make the point. "Every" accommodative diabetic custom foot orthosis (with guts/control) may be an L3020, but not every L3020 is an A5513. You cannot simply look at the description of A5513; you have to look to the elaborations in the LCD. They key point is this: "The base layer of the device must be at least 3/16 inch of 35 Shore A or higher material. [...] The specified thickness of the lateral portions of the base layer must extend from the heel through the distal metatarsals and may be absent at the toes." So, if your device has a base layer of cork (i.e. a cork shell) and that shell stops PROXIMAL to the mets rather than extending DISTAL to the mets (and being 3/16 inch at the mets), then your L3020 is NOT an A5513. You are committing fraud if you bill it as an A5513 and it does NOT HAVE THE 3/16" BASE LAYER EXTENDED UNDER THE METS. We use the A5513 all the time in custom molded shoes. However, the required design of the A5513 is rarely the best for OTS (off the shelf) shoes. Rarely is it a plus to make the device that thick at such a critical pressure area (the mets). Now, on some we may need to in order to unload one or more of the mets, however, rarely are such unloads fabricated from the base layer (i.e. extensions of the cork shell). We are usually adding our unloads from a different, firmer material, such as Nickleplast.
If you use a diabetic trilam and cork device, the cork is the base layer, not the royal blue pelite. If all you have under the mets is the royal blue pelite, your base layer does not extend to the distal mets. We may all fabricate our devices different ways and attempt to support and unload the foot in different ways. However, if your BASE layer does not extend TO THE DISTAL METS and isn't 3/16 INCH UNDER THE METS, then it is NOT an A5513 and should not be billed that way.

Previous Response: In regards to "PNW Reader's" comments: No disrespect taken. The base layer of my diabetic fo's runs through the met heads, so I am providing proper diabetic fo's. What I am trying to get across is that an A5513 and an accommodative L3020 are made the same as far as the positive cast is concerned in my practice. We are trying to accommodate and not biomechanically correct. The only difference, as we have both pointed out, is the material used, and every practitioner has their favorite material. The base material should taper as it goes under the met heads toward the toes. Also stated in the LCD is: Code A5513 describes a total contact, custom fabricated, multiple density, removable inlay that is molded to a model of the patient's foot so that it conforms to the plantar surface and makes total contact with the foot, including the arch. A custom fabricated device is made from materials that do not have predefined trim lines for heel cup height, arch height and length, or toe shape. I think that pretty much sums it up. So if there are labs heat molding prefabs to a positive model of a patient's foot, then they are not following these guidelines. Unfortunately I think this happens. I have used multiple labs for fabrication and the majority of orthotics come back with no arch, too wide of a heel cup, odd trimlines. - Joe Eads, C. Ped, COF
New Response: I am quite sure that it is the PNW reader that mis-interpreted the LCD. The base layer of the device is the bottom layer of the multiple density inlay or footbed. The base of the footbed itself must be at least 3/16 inch of 35 shore A or higher material for an A5513. It goes on to say that the base layer is allowed to be thinner in the custom-fabricated device than the A5512 because appropriate arch fill (longitudinal, transverse and lateral) or other additional material will be layed up to maintain shape and achieve total contact and accommodate each patient's individual needs. The arch fill is not just a cookie under the longitudinal arch-it is referring to the posting which supports all the arches under the foot.
You seem to think that because it is under the footbed that it becomes the base layer. This is not the case. The base layer they refer to is still the base layer of the footbed itself -- the bottom of the multiple-layered footbed, which may be thinner distal to the metatarsal heads so as to accommodate hammer-toes, cocked-up toes, etc.
If you go back and carefully read the entire LCD you cannot rationally interpret it any other way. I believe that PNW-Reader was out of line with his/her interpretation of the guidelines.

Question: This past week over 45,000 runners participated in the New York Marathon. Runners of all shapes, sizes, and stamina levels were challenged to their potential and beyond. What advice would you give to them to keep their feet in good shape, before, during and after participating in a strenuous 26.2 mile marathon? Is the RICE formula sufficient?
Rest your feet.
Ice helps keep inflammation and swelling down.
Compression with ACE wraps will also reduce swelling.
Elevation will help your feet rest up for the next big run.

Response: The RICE formula has good application when a runner has experienced a sprain of a joint or strain of a muscle/muscle group. This approach has been the standard 'first line of defense' for treating minor injuries for many years. Some runners will be able to ward off nagging injuries sufficiently to keep running.
However, the first principle for an aspiring or seasoned marathoner is adequate preparation. 'Guts' and 'iron will' are no substitute for training. There are numerous running clubs in large and even small metropolitan areas that provide coaching, training schedules and other advice that is very helpful. These groups can be found through local running specialty stores and on the Internet. Training injuries are often the result of 'too much, too fast.' This applies to beginners and seasoned runners as well. Running too far, too fast, sudden changes in terrain, and inadequate footwear are some common errors in training. We as pedorthists can play an important role in identifying alignment, as well as shoe issues that can be corrected, and thus alleviate a foundation cause for many injuries.
While there is much information on the Internet, there is a potential downside. Some individuals will be successful with 'self-treatment,' while others may cause serious injury (egs. stress fracture) when they don't seek professional advice.
There are products in the shoe industry promoted as 'recovery' shoes, but as yet, I am uncomfortable recommending any of these.

AAOP President Announces Gold Award in MarCom Contest
Scott Cummings, PT, CPO, FAAOP is pleased to announce that the American Academy of Orthotists and Prosthetists' O&P Career Awareness campaign won the Gold Award in the MarCom Award Contest in the category of External Communication Program.The Academy's O&P Career Awareness campaign was developed to increase awareness about the career opportunities available in the Orthotic and Prosthetic profession. Castro Honored with Dawn Janisse Award
The Pedorthic Foundation honored Ernesto Castro, C.Ped., as the fifth recipient of the Dawn Janisse Award during the recent North American Footwear Congress, held Nov. 18-20 in Orlando.
Castro received the Award from Dennis Janisse, C.Ped., who introduced the late Dawn Janisse to the pedorthic community first as an employee of his company, National Pedorthic Services; their work together grew into a deeper appreciation of each other, and they became husband and wife. The Dawn Janisse Award was created to recognize her work in fund-raising for the Foundation and in promoting greater public understanding of the contributions pedorthics makes to health. Shoppers Won't Use Credit Cards As Much This Holiday Season

The buy now, pay later mentality may be taking a backseat to debit cards and cash this holiday season. With budgeting a big concern this year, Americans are expected to rely less on credit cards for their holiday purchases, according to a survey expected to be released by the National Retail Federation.
The survey, which polled 8,778 consumers this month, found that 43% of holiday shoppers will rely on debit cards as their primary form of payment and 25.7% will mainly use cash, both slight increases over last year. Full Text

Canadian Footwear Wants Staff to Balance Work, Life

Scharfstein and his wife, Pam Cipryk, are partners in the second-generation family business, which has become renowned not only for carrying Canada's largest footwear inventory, but for its expertise as a foot health specialist (Scharfstein is a certified pedorthist). Canadian Footwear has three locations in Winnipeg, two in Calgary and an e-commerce store that sells online to customers across North America. 40 Years of Happy Feet
When he apprenticed under a master cobbler as a young teenager in his native Italy, Elio DiFelice learned a basic thing about shoes that is a foundation of his family's business decades later: no two feet are alike, and fit is the essence of a good shoe.
DiFelice, who moved to Canada as a young man and ended up marrying Lucy, a girl from his Italian home town of Castropignano, worked at the former Sunbeam shoe factory in Port Colborne before striking out on his own with his own shoe store on Albert Street in Thorold. A couple of years later he moved to a new storefront at 9 Front St. South in the city, making custom shoes and doing repairs, and also eventually moving into retail sales of footwear. Full Text

Diabetes and Foot Care

November was established as National Diabetes Awareness Month to raise awareness about diabetes, which has been ruled a major public health threat of epidemic proportions by the Centers for Disease Control and Prevention and continues to increase substantially each year[1]. Today, there are nearly 25 million children and adults living with diabetes in America, and every hour more than 166 adults are diagnosed and approximately eight die from the disease. For diabetes sufferers, National Diabetes Awareness Month should serve as a reminder of the importance of foot care. Diabetes can impair blood flow to the feet and cause nerve damage or an infection, which may cause an unavoidable amputation. Over 60% of nontraumatic lower-limb amputations occur in people with diabetes. Full Text

Type 1 Diabetes May Be Preventable
Finnish and Canadian researchers suggest an infant diet change may help prevent type1 diabetes from being initialized in children at risk for the disease. Hans Akerblom of the University of Helsinki led researchers in Finland and Canada in a pilot study of the diet intervention -- using an hydrolyzed casein-based formula when weaning infants -- rather than a regular cow's milk formula. German athletic clothing company Adidas is apparently pretty serious about defending its three-stripe logo. Adidas and its U.S. subsidiary, Adidas America Inc., have filed a trademark infringement lawsuit in federal court in Los Angeles, accusing a Gardena company of selling soccer clothing with "confusingly similar imitations of Adidas' three-stripe mark."
The lawsuit, filed Oct. 21, seeks monetary damages and a court order prohibiting Conrad Asher Licensing Group Inc. from marketing clothing that infringes on Adidas' trademark. Full Text

Pedorthic Pre-Certification Training Course January 24th - February 7th, 2011
In cooperation with the ABC and BOC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute. The course includes the required tests for competency to sit for the ABC or BOC exams.
Upon completing the course and successfully passing the competency tests, and fulfilling the 1,000 hour "Clinical practice Requirement" students are eligible to sit for the ABC or BOC exam to earn their Pedorthic Certification (CPED or BOC Pedorthist).
