Question: Am I missing something?  How are other practitioners fabricating their diabetic fo's?  The descriptor for A5513 is "Diabetic multiple density insert, custom molded from model of patient foot," and the descriptor for L3020 is "Foot insert, removable, molded to patient model, longitudinal/metatarsal support, each."  To me, there really is no difference here, with the exception of the "longitudinal/metatarsal support." Obviously, the A5513 has the same features, and if there are labs fabricating "custom molded" diabetic fo's by heating up a prefab and "forming" it over a patient's mold, they are not a true custom fo, as they do not take on the shape of the patient's feet.  I fabricate my A5513 fo's with trilam (with a pzote or pcell topcover), with either an eva or cork archfill if needed for heavier or more active patients.  If I were to fabricate an L3020 for a diabetic patient, it would be built pretty much the same way.  Doesn't a diabetic need to have a soft conforming top cover to accommodate prominent bones and high pressure areas?  Wouldn't you use the same type of top cover for an L3020 that you are giving to a diabetic?  So, what is the  difference between the A5513 and L3020?
 
To me, losing a patient and knowing that they might go elsewhere and receive substandard care is not something that I am OK with.  An accommodative orthotic is an accommodative orthotic.  Whether it is a diabetic orthotic or an accommodative orthotic for diabetes or arthritis, it's pretty much fabricated the same way (with the exception of some different materials).  I think it all boils down to reimbursement.
 
Don't get me wrong, I am not trying to reinvent the wheel or stir the pot, but to hopefully gain a better understanding as to why, in our small profession, we do things so differently.  Let's try to share our ideas and constructive criticism in a positive manner and come up with positive solutions - not just for pedorthists, but also for our patients.

Question: We've been having a theoretical debate in our office here, and I thought I would bring it to the PNW. If I have a patient that requires both a lift and a forefoot rocker, I could actually create the rocker in the same piece of material I added for the lift. Here's the question: if I used the same material for both, what do I bill for? Can I actually bill for both a lift and rocker sole, or does it need to be for one or the other? If I can only bill for one, do I just choose the one with better reimbursement?

A second aspect to this question would be this: what constitutes the actual rocker sole? For instance, if I took a pair of shoes with a thick enough crepe sole to grind a rocker into then outsole them with rubber, can I bill for a rocker sole? If not, what would this modification be considered? Before we get responses flaming us for bad billing, let me say we have not been doing it this way, again this is a theoretical debate. I just decided to play devil's advocate and see what the community had to say. Muwahaha...

Response: The question is an excellent one.  I would encourage all C. Peds to read the chapters on rocker soles and elevations in the PCC's guide to coding.  I offer the following excerpt:

Differentiating between Elevations and Rocker Soles

Since elevations are usually tapered at the toe, it is sometimes difficult to determine when to use the rocker solecode and when to use the elevation code.The PCC has developed the following ME-CE rule:

1. An elevation that is not tapered to the distal end of the toe is always billed as an elevation.

2. Assuming a shoe modification that is distally tapered/rockered:

a. If the total net material added is one-quarter of an inch or less, it is billed as a Rocker Sole.

b. If the total net material added is greater than one-quarter of an inch, the first one-quarter of an inch isbilled as a Rocker Sole. Any other material added is incrementally billed as an elevation.

This is a bright-line rule to help everyone (payor and practitioner) have clear guidance.  This is not the rule of any one payor per se, it is the rule proposed by the PCC for all payors.  The HCPCS system is an incremental coding system.  For a complete discussion of all of these issues, please see the PCC guide to coding which was offered free to all PFA members for several months, and is now available for purchase from PFA.

Response: I love a good theoretical debate!  You raise a great point that most other professions have long since addressed and concluded that; Yes, you bill for each legally billable component.  Example: SCAFO has 3 L-Codes, L-1940 The orthosis (including casting and prep), L-2330 is the leather covering, L-2820 is the padding. 

So in your example, why couldn't a pedorthist bill for the lift (including prep) L-3310, Forefoot rocker L-3400, and the outsole covering L-3540 = 3 codes?  You can, but the problem is that the L-Code descriptors are antiquated when it comes to footwear modifications.  They do not get as specific as they should given modern descriptions.  Take L-3400 Metatarsal Bar Wedge, Rocker; it's the only descriptor that mentions the word, Rocker.  We all know there are a host of rockers for purposes other than metatarsal relief.  How about, L-3595 orthopedic shoe addition, March Bar.  How many in the profession are even aware of a March Bar or its purpose? (Probably just us old-timers)

There are other codes that apply mainly to O&P, but if the definition of  "orthotic" includes feet, why can't we use them?  L-4200 Repair of an orthotic device or S-0395 Impression casting of a foot by a practitioner other than the manufacturer of the orthotic. I think our profession is behind the curve on proper coding.  Perhaps, if we put more emphasis on proper coding instead of the TSD, pedorthists would realize their full scope of practice, offer more billable services and get paid what they're worth.  That's why PFA offers at least two different resources on coding.

Original Question: I received an email from a person who simply said, "My legs get over heated if I continuously wear shoes and socks. Then if I remove the shoe and sit I will become alright.  I have diabetic, but it is only in the brim. Fasting sugar is between 112 to 118. After food it is around 114 to 118." 

That isn't much to go on, and I have no idea if the issue is circulatory or neurological in nature. How common is neuropathy of the legs? Would it be common in a person that seems to have his blood sugar levels under apparent control?   I have seen leg muscle strains that can produce a burning feeling, and sitting may reduce that sensation, but I don't see how removing his shoes would be a factor, even if they were poorly fitted.  Any suggestions?

Previous Response: I'm a little confused. I have always been under the impression that diagnosing was the purview of physicians, not pedorthists. I don't understand why Mr. Stanley and others would be trying to come up with a diagnosis. Has anyone considered asking this person to go see a doctor?

New Response: No worries - the question was from a very short email message I received.  I already replied to the person with my typical "see a doctor" reply, but also a request for more details.  My posting to PNW was merely an attempt to learn more myself...not to diagnose, but to understand. This person who sent me the email probably also sent it to 28 other people, possibly not only to those in the medical trade but his accountant, his psychic guide and his mechanic.  I'm just trying to have a little bit more understanding than his psychic advisor.

Question: Does anyone have recommendations for shoes and/or sandals (not flip-flop style) with good arch support for someone who suffers from plantar fasciitis but prefers to buy shoes that aren't made with animal materials such as leather?  In shoes, she does use high quality over-the-counter orthotics to help support the arch of the foot.

Comment: Went through my first prepayment review...and flunked!  Thanks to the certifying doctor who checked off "poor circulation" on the certifying form...and upon receiving the patient's notes from the doctor...no mention of poor circulation!   He did, however, note neuropathy, but did not check that off on the certifying form.  So no pay!

It's totally ridiculous that the Pedorthist needs to worry about the Rx that the Doctor signs. Are we not filling a prescription??  Do pharmacists need to get patient notes for all their prescriptions??? PFA must make this matter their number 1 priority. I am accredited by ABC; they have seen all my forms, etc. and I am golden with them. Then Medicare pulls a facility visit and checks everything including patient files...no problem there.   Because the certifying Doctor doesn't do his/her job, I am penalized.  Totally unfair.   Thanks for letting me vent.

Comment: I have been reading the PNW on and off in the past months.  I say it this way because there are times I can't read it, as I get so frustrated with the issues. I am disappointed in our Pedorthic Industry support system. PFA & ABC were supposed to be there to have the Pedorthic Industry best interests at "heart." I haven't seen much help. It doesn't appear to this Pedorthist, that they wish to help the industry survive.  I get very frustrated with the issues, medical supply shops and others who have no real training doing our job, the lack of reimbursement...but I digress, we all know the issues. I unfortunately see the demise of the industry in the near future. No one would want to come into this industry with the problems with reimbursement and other issues, especially if they do their homework first.

I agree that educational requirements need to change, but an associate's degree is very expensive to come into an industry where you lose money by taking insurance, if you do quality work. Very sad situation.  Eight years ago, when I decided to come into this industry. I thought it would continue to retirement.  Unfortunately, it has retired before I was ready.

Unfortunate, as in my town there is really no quality place for the people to go, and I don't just mean the diabetic. Most places that do the orthotics do poor quality work, providing more risk than value. The quality places are either stopping altogether or, like myself, only seeing cash pay patients.  This is so unfortunate for the public.

It is very sad to see where the industry is going, as it is so important.

MEPs Back Country of Origin Labels

Clothing and footwear imported from outside the European Union could be bound by law to state its origin following a landslide vote in European Parliament. Some 525 MEPs voted in favour of the proposed legislation compared to just 49 votes against. There were 44 abstentions.

The detail of the legislation still needs to be thrashed out before it is formally passed, but the vote is being seen as a major boon for British and Italian manufacturers, whose product is considered to be higher quality than their counterparts in many other Eastern and Southern European countries. U.S. Luxury, Dept. Stores Set for Xmas Bounce - NRF

Luxury and department stores will be the biggest beneficiary of expected growth in the U.S. retail market this holiday season, according to the head of the National Retail Federation. Matt Shay, speaking at the World Retail Congress in Berlin, said consumers were still looking for value, but that they were now basing this on quality and service, rather than just price, as they had done for the past two years.

Shay added that consumers were also looking for discretionary purchases, rather than just practical gifts. "Department and luxury stores are clearly going to benefit more this year. We're seeing jewelry and apparel as items people want on their holiday shopping list." Full Text

In Canada: Meeting a Local Need

After 20 years as a custom boot fitter for skis and skates, Brantford-born and raised Jesse Cornell has gone from fitting boots to fitting limbs. Cornell returned to his hometown about three months ago to open Biodesign Prosthetics and Orthotics, located at 207 Brant Ave., with the help of his wife, Kari Osborne.

While his move to prosthetics seemed a natural transition for the self-professed "perfectionist," it took a push from a London physician to steer Cornell in that direction. "I had been fitting ski boots for about 20 years," Cornell said. "I worked with a physician outside of London who was working with a competitive ski team that I was fitting and he kept pushing me to become a pedorthist."

Cornell's pursuit of becoming a certified pedorthist ignited an interest in prosthetics and orthotics. Full Text

Cuban Treatment for Diabetic Foot Ulcers Heads for Europe

Cuba's biotechnology industry is hoping to conquer the European Unionmarket with Heberprot-P, a therapeutic drug used to prevent foot amputations in patients with diabetes. Heberprot-P, developed by the Genetic Engineering and Biotechnology Centre (CIGB), stimulates tissue healing in deep, hard-to-heal foot ulcers that frequently occur as a complication of diabetes. It was approved in Cuba in 2006 and has been in clinical use on the island since 2007.

The active principle is recombinant human epidermal growth factor (rHu EGF), a genetically engineered protein that aids skin cell repair and regeneration. It is injected directly into the wound, and its makers stress that it is the only effective therapy available for patients with chronic ulcers. Full Text

Majority of Americans with Diabetic Foot Ulcer Suffer from PAD

A new analysis published by THE SAGE GROUP, concludes that peripheral artery disease (PAD) afflicts 2.0-3.7 million U.S. citizens with diabetic foot ulcers (DFU). Approximately 1 million diabetic foot ulcer patients suffer from the most severe and deadly form of PAD, critical limb ischemia.

Distance Runners Are a Paradox for Insurers

Jennifer Frighetto is not a marathoner, but it is not for lack of trying. Had she crossed the finish line at this year's race in Chicago, it would have been her first successful attempt at the 26.2-mile distance. But just as at the 2008 and 2009 Chicago Marathons, Frighetto was unable to finish because of injury.

Frighetto, a self-described former couch potato, said that since she first decided to run a marathon in late 2006, she has seen doctors for a stress fracture in her foot, plantar fasciitis and iliotibial band syndrome. The activity that promised to make her healthier was actually increasing the frequency of her doctor visits, a fact that makes amateur athletes like her a problematic group of people for health insurance companies to insure. In cooperation with the ABC and BOC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS.
