Question: Does anyone have recommendations for shoes and/or sandals (not flip-flop style) with good arch support for someone who suffers from plantar fasciitis but prefers to buy shoes that aren't made with animal materials such as leather? In shoes, she does use high quality over-the-counter orthotics to help support the arch of the foot. - PNW Reader
PREVIOUS QUESTIONS AND COMMENTS WITH RESPONSES
Question: I was wondering if anybody knows of a good lab that will fabricate real "custom molded diabetic orthoses" that are actually shaped like the pt's feet. I have some frustration (as I am sure a lot of you do) with how terrible my orthoses come back looking. They usually do not contour the arch properly, or they are too narrow in the forefoot, or heel, or both. Because of the "low reimbursement" for these orthoses, I continue to get poor quality. I fabricate my own if the foot has deformities (which most do), or if the feet are really wide, as I have yet to find a lab that can fab to anything but an average foot. This then bogs me down with technical work. I have seen how many pt's end up getting sub par footwear and have their feet put at risk because their orthotics were made incorrectly to save a few bucks, and their practitioner has no problem sending them home with said footwear. How many practitioners are fabricating their own orthoses? How many are doing more heat molded than custom? How many care enough about your pt's to assure that the orthoses are made properly and fit well? If there are any representatives from any labs that read this who think they have an answer to these problems, please feel free to e-mail me. [Please also send your responses to Pedorthic Newswire for the benefit of other readers.] - Joe Eads, Joe@waoandp.com Previous Response: We make an excellent custom foot orthosis for diabetic feet, but it is not an A5513, it is an L3020. Just because it is custom and a diabetic wears it, it doesn't necessarily meet the definition of A5513. Our device has 5 layers, excellent support, and fits nicely into OTS shoes. We have used this design successfully for over 20 years. HOWEVER, IT LACKS A BASE LAYER OF 3/16" RUNNING DISTAL TO THE METS WHICH IS THE SINE QUA NON OF AN A5513. Believe me, it has plenty of TOPCOVER thickness under the mets - but that padding is not an extension of the BASE layer (which in our device is cork). The devices we have made for our custom shoes meet the definition of A5513 and are billed that way. The ones we make for OTS shoes do not. L3020 is not covered by Medicare, but is covered by many other insurances. When filing to Medicare, we bill the L3020 with the GY (noncovered) modifier. If the patient has no secondary or a secondary that does not cover L3020, they are told the price of the L3020 and that they must pay it. If they choose not to pay it, they are offered the A5512 and have the risks and benefits explained. They sign a document stating such, and we proceed. We also loop back with the doctor as to what happened. If they have a secondary insurance and the secondary insurance DOES cover L3020, the secondary will pick up the coverage on the device after Medicare denies. Most times, the reimbursement is far superior to the reimbursement for the A5513. This makes sense, because the L3020 device is far superior to an A5513. The L code that most closely resembles A5513 is the L3002. I think where practitioners get themselves into financial or quality troubles is making L3020s and billing them as A5513s. They make a Mercedes, and bill for a Chevy. Remember, even if you are ok with losing money, you are not allowed to bill a device as an A5513 unless the BASE layer is at least 3/16" DISTAL TO THE METS. - PNW Reader New Response: This is in response to a response regarding my question about custom molded diabetic orthotics by a "PNW reader." What kind of response do you get from the beneficiary when you tell them that the shoes will be paid for, but they are going to have to pay $250 - $400 for their custom orthotics? The TSD bill was set up so that these patients (most of whom have multiple health problems, and have little to no funds) could receive appropriate footwear to protect their feet. Even more, what do your referring physicians think of this? I'm not sure what your 5 levels of material are, but if they don't include some kind of soft conforming material like plastizote, p.cell, or ultra cloud that will mold to and protect those high pressure areas on insensate feet, I wouldn't call it an appropriate diabetic orthotic. I don't agree with the low reimbursement from Medicare, either, but we as pedorthists need to make sure we are fitting our patients with appropriate shoes and orthotics that actually fit the patients' feet and last for an appropriate time, and that are comfortable to wear. Medicare should and does pay for this. For those doing 3 pairs of A5512 at a time (in one visit), you should be heat molding each orthotic and most likely be adding a scaphoid pad to the plantar aspect of the medial longitudinal to ensure total contact of the foot while in the shoe. And if you are doing this, how much time is this taking? To me, it takes quite a while if done properly. - Joe Eads, C.Ped., COF
Comment: I have attached my letter to Brian Lagana and board members of the Pedorthic Footwear Association. Please run this in your next issue of Pedorthic Newswire, as I believe their efforts may have a very negative impact on our profession. Read the letter here. - Burt Humphries, ABC Board-Certified Pedorthist Response: Having read Burt's letter to PFA, I have to take issue with one part of his letter condemning Podiatrists, Orthotists and Prosthetists as poor fitters of shoes or somehow less qualified than C. Peds to fit diabetic shoes. I think we all (DPMs, COs, CPs and C. Peds alike) can name instances where someone from another profession(s) may have misfit someone. It happens. I will give you that perhaps those professions, generally speaking, might benefit from additional shoe fitting training, but I will also say that a lot of C. Peds can use it as well! Just because we, as C. Peds, are supposed to be experts in shoe fitting, this alone doesn't qualify us to be good fitters. It is experience and the sharing of that experience with other C. Peds and professions that will help make us all "more qualified." You included in your letter your experience, which seems considerable. Perhaps you might use that to your advantage and work with those other professions in your area to increase their skills or simply WORK to have them send to you their patients so that there will be increased positive outcomes. A win-win situation for everyone! Ask yourself this question: With the scope of practice that DPMs, COs and CPs have (compared to ours), do you really think they want to deal with shoes, with all the other modalities and procedures they have to perform? In most cases, they don't! Again, a perfect opportunity for you to be your own best advocate for your particular skill set. Furthermore, whether you like it or not, diabetic shoe fitting is part of their collective scopes, so you'll just have to deal with it. Also, you failed to mention those outfits such as pharmacies, medical supply shops, fly by nighters who use people fitting "under the supervision of a qualified practitioner" (no one seems to know who that is in these places). These "fitters" are the ones that actually have NO significant training or experience whatsoever and have no business fitting shoes period, let alone on the diabetic foot. These places have jumped on the TSD gravy train to increase their bottom line rather than provide proper patient care. Sure, they may have attended a weekend course on shoe fitting, but that absolutely DOES NOT qualify them to fit a diabetic shoe on a problematic foot! At least DPMs, COs and CPs have considerable experience dealing with diabetes on various levels! So, rather than continually throwing our allied partners under the bus with an us against them attitude, how about we focus our collective energies on eliminating those providers who are TRULY not qualified to fit diabetic shoes under the TSD? - Rob Patai, C. Ped./L.Ped, Victory Orthotics and Prosthetics Question: I have been listening to all the talk about changing the educational requirements to a two-year Associate's Degree. While I agree that the requirements need to be changed, I have one question for thought. Why would one want to go to two years of education to become a C. Ped. to work basically on the foot and ankle when in the same two years of education, you can become an Orthotist? This field opens the doors to more modalities that can be done, and the potential to making more money. Then I have another question. What will happen to the Pedorthic field? Response: I would say I have had the privilege of being in a great company as a Pedorthist for the last ten years now. I have been under the umbrella of a comprehensive orthotic facility as opposed to duking it out, so to speak, as a solely pedorthic store. Throughout my years here, I have been offered the chance to become an orthotist but have turned it down. I appreciate what I do and wouldn't want to change it, even if it means more money. I like working with the geriatric and veteran demographics, and dealing with my sub-malleolar scope of practice has offered me plenty of challenges. I appreciated Jim's question/comment in the PNW about the new education levels and the future of pedorthics.
If you asked me even fresh out of high school what I would want to do with my life, I would have never replied, "I want to work with feet!" The company needed a pedorthist and the requirements at the time seemed doable to me so I went for it. It's easy for me to say that the new level of education is worth it, I would even argue necessary looking back. I wondered myself if I would have become a pedorthist had I been required to go through not just the Associates but the 1k clinical hours, which seems daunting. I think because of this, those who become pedorthists will do so for the specific choice to be one, rather than a blasé "four weeks of school and I get a pay raise," and I believe they will be much more equipped than I was when I began. I think in the end this will be a very good thing for our profession, and I look forward to seeing the benefits. I could speculate all day, but only time will tell.

Question: I received an email from a person who simply said, "My legs get over heated if I continuously wear shoes and socks. Then if I remove the shoe and sit I will become alright. I have diabetic, but it is only in the brim. Fasting sugar is between 112 to 118. After food it is around 114 to 118." That isn't much to go on, and I have no idea if the issue is circulatory or neurological in nature. How common is neuropathy of the legs? Would it be common in a person that seems to have his blood sugar levels under apparent control? I have seen leg muscle strains that can produce a burning feeling, and sitting may reduce that sensation, but I don't see how removing his shoes would be a factor, even if they were poorly fitted. Any suggestions? - Chris Stanley, C. Ped. Response: Is this a brain buster? With what little information you have, it sounds like it could be Erythromelalgia. Stopping and cooling down is how it is managed. - Carole Romig, C. Ped. Response: Find out if this person has been diagnosed with neuropathy; that would explain the symptoms being described, so start there. If it is neuropathy, you can have it anywhere on the body. As you know, there is no cure for neuropathy. Some patients have reported success in certain medications, however the patterns seem to indicate that this will vary considerably. Unfortunately, most of the time we hear nothing works. Recommend the person to on endocrinologist. - Althea Powell C .Ped. Question: Every once in a while a client returns a full-length carbon fiber foot plate because it is fractured. What are the conditions you have found to cause this to happen? What are the preventions? - Robert S. Schwartz, C. Ped., Eneslow Pedorthic Enterprises, Inc.
Response: We used to have problems with some returns on the carbon fiber footplates in the past as well. One thing that helped us was making a switch to a company that manufactured both a flat and a contoured version of the footplates. The bridging that occurs when placing a flat footplate into a shoe with a raised heel or exaggerated forefoot curvature increases the chance of breakage, and is an ill fit overall. I'm not saying this is the case with your patients, but for those who didn't know, I thought I would comment. As Chris Bonsall said, no matter what you do repetitive stress will eventually break the footplate.

Pedorthic Foundation to Launch "Feet for Life" & Honor Richard Schwartz at North American Pedorthic Congress The Pedorthic Foundation will host a major reception on November 17 in Orlando, Florida, the evening before the opening of the first North American Pedorthic Congress, a three-day event being co-sponsored by the Pedorthic Footwear Association and the Pedorthic Association of Canada. The reception includes two distinct components: one, the inauguration of the largest single charitable undertaking in the pedorthic community; and two, recognition of a long-time industry leader, Richard B. Schwartz, who has been a pedorthic leader from more than 40 years, by the establishment of an award in pedorthics named after him. Guests will enjoy food and drinks during an evening of fellowship and fun. The Foundation's Board of Directors expects the reception to become an annual Pedorthic Foundation event; the inaugural celebration is being sponsored by Aetrex Worldwide, Inc., and will be held at the Walt Disney World Resort. The Office for National Statistics said the annual rate of clothing and footwear inflation was 0.9 per cent in September, the first rise since 1992.
Prices rose by 6.4 per cent between August and September, the largest increase for that period since records began, as shopkeepers put up the prices of their autumn clothing ranges and started to prepare for next year's increase in VAT. Full Text

York's small business owners feel even more pessimistic about the economic outlook for their companies than the rest of the nation. Only 19% of entrepreneurs in the state intend to hire over the next six months and just 39% have a positive outlook on the economy and business prospects, according to an American Express survey released Monday. By comparison, last spring the survey showed 36% of companies planned to hire, and 53% had a positive view on the economy and business prospects.
It's not just forecasting future problems, either-61% of New York's small businesses have cash-flow issues, compared to the national average of 53%, according to the study. Additionally, a lower percentage of entrepreneurs here have capital investment plans than others across the country. Some 30% of New York small business owners describe their businesses as "a sinking ship," while 53% claim they are "staying afloat." Full Text

A stem cell scientist at UC Irvine hopes in the next few years to be able to test a new type of transplant for Type-I diabetes patients.
You control Type II diabetes with diet, exercise and medication. You control Type I the same way, plus frequent insulin injections. You can't live without those injections. That's because with Type I diabetes, your pancreas doesn't make insulin.
UC Irvine scientist Jonathan Lakey is trying to fix that. He's working on a type of islet cell transplant that uses a patch-like device to put the cells on the pancreas. Full Text

It hurts and defies logic, really, to shell out $100 or more for new shoes when your current pair still looks good. You can probably get away with holding onto dress shoes a little longer but clinging to an old pair of athletic shoes too long can cost you plenty. The American Academy of Podiatric Sports Medicine reports that if runners don't replace their shoes every 350 to 500 miles, they face a potential litany of injuries, including stress fractures, plantar fasciitis, shin splints and heel spurs. Exactly how long shoes will continue to provide the needed level of protection depends on individual size and weight. Average runners pound their shoes on the ground around 700 times per mile so it doesn't require an active imagination to see how shoes lose their shock absorption rather quickly. And once that occurs, it can change your stride and alignment, meaning pain and possible injury are just down the road. Pedorthic Pre-Certification Training Course January 24th-February 7th, 2011
In cooperation with the ABC and BOC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute. The course includes the required tests for competency to sit for the ABC or BOC exams.
Upon completing the course and successfully passing the competency tests, and fulfilling the 1,000 hour "Clinical practice Requirement" students are eligible to sit for the ABC or BOC exam to earn their Pedorthic Certification (CPED or BOC Pedorthist).
