Question: I was wondering if anybody knows of a good lab that will fabricate real "custom molded diabetic orthoses" that are actually shaped like the pt's feet. I have some frustration (as I am sure a lot of you do) with how terrible my orthoses come back looking. They usually do not contour the arch properly, or they are too narrow in the forefoot, or heel, or both. Because of the "low reimbursement" for these orthoses, I continue to get poor quality. I fabricate my own if the foot has deformities (which most do), or if the feet are really wide, as I have yet to find a lab that can fab to anything but an average foot. This then bogs me down with technical work. I have seen how many pt's end up getting sub par footwear and have their feet put at risk because their orthotics were made incorrectly to save a few bucks, and their practitioner has no problem sending them home with said footwear.  

 

How many practitioners are fabricating their own orthoses? How many are doing more heat molded than custom? How many care enough about your pt's to assure that the orthoses are made properly and fit well? Question: If I am not a participating provider with Medicare, and I do not take assignment on a particular claim (i.e. diabetic shoes), am I required to send a claim in for patient if he/she asks?  If the answer is yes, is it because I bill Medicare for other services and get paid?

 

Obviously, some shoe stores sell Diabetic type shoes, but never bill Medicare for any services, so I am sure Medicare cannot require them to file a claim on behalf of patient.

Comment: To PFA Board, Members, and other Pedorthists:

Regarding the recent publication from the PFA (PFA's Pushes for Common Sense Changes to Improve the Administration of Medicare's Therapeutic Shoes for Person's with Diabetes Benefit), I have serious concerns about the organization that is representing us as Pedorthists.   I don't know when this publication was written, but I just received it yesterday.  In it, there are six areas the PFA recommends improving.  The first is to establish a qualified supplier definition.  This is excellent, and should control 90% of all fraud, waste, and abuse by having only credentialed professionals dispensing the footwear. However, has this not already been implemented?  I thought with all the facility and individual accreditation, we had already covered all of this.

 

My main concern is with the second and fifth recommendations.  The second wants to restrict the number of inserts that can be dispensed to only one pair per visit.  While this may be a highly noble stance to take in lowering Medicare payouts, it will hamper the quality of the products the patient receives, as well as make every practitioner consider the viability of continuing this program.  Where will the time come from to do these additional fittings in the future?  How can we continue to provide good service while lowering our revenues?  I am a one-man operation.  I see 8 - 10 patients a day five days a week and spend about 45 minutes with each.  I spend any extra time and my entire day Saturday in the lab fabricating the custom jobs.  I have always dispensed 3 pairs at one time. Whether it is the A5512 or A5513, the nature of soft plastazote or similar inserts is that they will bottom out.  I have always advised patients to rotate their inserts once a week.  I also don't know of any of my patients who would want to come back thee times a year to get what they're getting now in one visit.  It should also be noted that the vast majority of my patients do have to pay a co-pay.  The point that a patient's condition may change during the year is valid, but as practitioners, it is our responsibility to make sure their inserts are refurbished or replaced as necessary at no charge during that year.  [Read the rest of this letter here.]

Comment: I read this publication thoughtfully and often. I am delighted to see some light being shed onto the subject of diabetic socks. Having something to do with the creation of this product category way back in 1994, I have had more than just a small stake in its development and growth over these years. There is creditable research on socks, albeit out of date, and there is more than enough anecdotal evidence to support the benefits of well-made diabetic socks. I have been in the premium sports, medical and functional sock business for over 25 years, the last 10 spent as a consultant to interested and vested companies. In 2004, I was asked by the USA's largest direct-to-consumer Internet/Catalog diabetic sock seller to draft a document for their personnel as guidance in the development and selection of diabetic socks. Comment: I am responding to the comment from Rob Zellmer in the September 29, 2010 issue of Pedorthic Newswire and your response [Thank you for pointing out the inaccuracies in the article we linked to, and for furthering the cause of educating our readers about the appropriate treatment, footwear, and socks for diabetic patients.  After reading your letter I did some research to try to find a comprehensive, more reputable published article about diabetic socks, and I was surprised at the differing recommendations in the media in the very few articles that I could find.  The American Diabetes Association also had limited information on their website.]  You are correct that there are not many articles on the subject of diabetic socks, nor is there much peer-reviewed, published clinical research on the topic. However, there is an authoritative article that was published in Diabetes Spectrum, a peer-review journal of the ADA, in 2001 entitled "Sockwear Recommendations for People with Diabetes" by Carol B. Feldman and Ellen D. Davis [see this link: http://spectrum.diabetesjournals.org/content/14/2/59.full].  The key concluding statement in this article reads, "Patients who are at risk for ulcer development because of decreased pressure sensation should be advised to wear densely padded socks.  In the studies cited here, the padded socks used were the Thorlos brand, which are made of 100% acrylic fiber with nylon and spandex for elasticity."

 

Additionally, for the most part, Mr. Zellmer's information is correct; however, his assertion that "hand linked seamless toe closures eliminate the rough toe seam that is present in 'ordinary' socks and, although very more expensive to manufacture, are critical to a patient suffering from neuropathy who may not feel a rough spot in his footwear" is not substantiated by anypublishedresearch that we can find.  There was a study done by Stacpoole-Shea, et.al. in the 90's that measured pressure on the seams of socks with a mechanical pressure measurement device (not on humans) and determined that there was a significant variation in pressure under the seams as compared with other locations on the sock.  However, a subsequent (unpublished) study was done by the same researchers; and it was discovered that the pressure measurements were within the range of normal variation of the measurement device.  Translation: invalid conclusion from the first study, and no scientific basis for any efficacy claims for seamless socks.  Claims for seamless socks have gained notoriety largely due to marketing efforts by manufacturers of "thin" diabetic socks, in which a seam certainly could create pressure.  Notably, Thorlos diabetic socks are "densely padded" as the Feldman and Davis article described, and have over-the-toe padding such that the seam does not cause any friction or pressure.  If the Dr. Specified brand has any such research we would recommend they put it forth; but to our knowledge, there is but one sock product that has peer-reviewed, published research to support efficacy claims for pressure reduction, blister reduction and pain reduction: Thorlos.  The studies to which I refer can be found at this link:http://www.ipfh.org/research/summary_peer_research.php

Question: Every once in a while a client returns a full-length carbon fiber foot plate because it is fractured. What are the conditions you have found to cause this to happen? What are the preventions?

Response: This is a normal occurrence, and you should explain to all of your clients that the potential to fracture is there, and you will not replace them/refund them.  The normal gait structure involves bending of the toe joint.  The plate retards that.  But normal stress will cause the plate to weaken and eventually crack.

Question: We have Medicare questions needing clarification by PNW readers:

 

1) Medicare states that the physician must confirm that the patient meets the criteria for the Therapeutic Shoe Bill. I understand all the diagnoses/requirements but the "poor circulation one." There is no ICD9 diagnosis for "poor circulation." Therefore, I am looking for insight into whether a "congestive heart failure" would imply "poor circulation" and would be acceptable to Medicare (in your opinion) or any other systemic diseases that we can "assume has poor circulation because of the diagnosis."

 

2) Does the pedorthic community believe that a "hammertoe, mallet toe or claw toe" meets Medicare's criteria of "foot deformity?" If a misshaped digit of this nature can meet the criteria for "foot deformity," then can a curly toe, medially or laterally deviated digit upon its long axis, also be considered a "foot deformity?" Is an everted calcaneus considered a "foot deformity?"

 

3) I remember the day when Janis Gregory, who is now considered "our pedorthic expert on Medicare," was our mentor.  I would be interested in discussing how we might put together a short PPT on this topic to aid in our students' understanding, and most importantly to ensure that we are compliant in our internship program as we enter nursing homes to provide pedorthic services.

Previous Response: CHF is caused by poor left ventricular function, or left side heart muscle failure. CHF, in my opinion, is a stretch for poor circulation. I believe that the only way to prove that a person has poor circulation is by ankle brachial index, or a diagnosis of peripheral arterial disease, PAD. Ultimately, poor circulation is the only reason that a person will develop a foot ulcer. A foot deformity coupled with good circulation will not result in a foot ulcer. But a foot deformity with "poor circulation" is very dangerous.

New Response: I respectfully disagree with the response offered by Mr. Davila and feel it is important enough to speak up.   Based both on clinical experience working together with vascular surgeons in a wound center environment and mostly on the vast current literature on these subjects, an ankle brachial index is known to be a poor indicator of true vascular status in diabetics.  Moreover, poor circulation is clearly not the only reason a patient will develop a foot ulcer.  Neuropathy, malnutrition, vasculitis, connective tissue disease, neoplasms (cancers), bacterial infections, fungal infections, viral infections, etc are just a few that we see with some regularity in clinic.

Question: I have been listening to all the talk about changing the educational requirements to a two-year Associate's Degree.  While I agree that the requirements need to be changed, I have one question for thought.  Why would one want to go to two years of education to become a C. Ped. to work basically on the foot and ankle when in the same two years of education, you can become an Orthotist?  This field opens the doors to more modalities that can be done, and the potential to making more money.  Then I have another question.  What will happen to the Pedorthic field?  Some questions for thought!  The future of Pedorthics is at hand.

Holiday Retail Sales Forecast


Holiday retail sales forecasting begins in earnest during the month of September. "Back to School" and Labor Day sales are the early indicators. Usually apparel sales perform better during this period than during the holiday season. The day after Thanksgiving is the next big indicator. The holiday shopping season usually peaks on the Saturday immediately before Christmas. The majority of shoppers begin their holiday browsing in October, but most begin purchasing gifts in early November.


In 2009, holiday retail sales amounted to approximately $458 billion, an increase of about 3.6 percent from 2008 sales. However, it is important to note that the 2009 holiday shopping season benefited from an additional day of sales - there were 28 days in the holiday season of 2009, whereas there were only 27 days in 2008. Cobbler Also Crafts Prosthetics and Braces


When Esteban Botello was 7 years old, his grandfather put him to work in his cobbler's shop. "You need to learn this skill because one day you may be the only one left who can do this," his grandfather told him. Today Botello bills himself in ads for his Baton Rouge shop, R & L Orthopedic Shoes Inc., as "the only cobbler in the state" - and says he's one of the few anywhere making footwear the slow, painstaking way of the craftsman rather than on an assembly line.


His grandfather, in Brownsville, Texas, made fine shoes and boots to order. (He was a friend of the famed Lucchese boot-making family of San Antonio). But he also made prosthetic limbs and braces. And he soon had his young grandson carving peg legs.

In his small shop in the Corner Square shopping center at Florida and North Sherwood Boulevard, Botello practices his skill as a cobbler as well as a "prosthetist, orthotist and pedorthist." And, for a hobby, an upholsterer. Full Text

Drug Reverses Debilitating Condition in Diabetics


Researchers at the University of Kansas have developed a drug that stops and reverses diabetic peripheral neuropathy (DPN) in mice, a condition that that ultimately leads to nerve death in the extremities of diabetics. This drug, KU-32, restored sensory function to damaged tissue when orally administered to diabetic mice with DPN once a week. It appears to be completely non-toxic according to tests and it is very well absorbed into the bloodstream. Full Text

Getting to the Sole of 'Toning Shoes': Local Podiatrist Offers Advice on Footwear Fad

While the newest fad for getting fit may sound as simple slipping on new shoes, one local doctor cautions that results from so-called "toning shoes" can vary widely. In recent years, the footwear market has exploded with an array of toning shoes, also known as fitness shoes, which claim to promote muscle toning by making leg muscles work harder.

According to Bartlesville podiatrist Dr. James Webb Jr., such shoes can either have positive or negative effects, depending upon the age and physical condition of the potential purchaser. For example, Webb said that if properly utilized, the shoes may help some with heel problems and relieve heel pain. However, he also pointed out that because of how the shoe is designed, instability is a main concern for those who have poor balance and may be vulnerable to falling. Full Text

Pedorthic Pre-Certification Training Course January 24th - February 7th, 2011

 

In cooperation with the ABC and BOC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute. The course includes the required tests for competency to sit for the ABC or BOC exams.

 

Upon completing the course and successfully passing the competency tests, and fulfilling the 1,000 hour "Clinical practice Requirement" students are eligible to sit for the ABC or BOC exam to earn their Pedorthic Certification (CPED or BOC Pedorthist).
