NEW QUESTIONS AND COMMENTS
Question: Does anyone out there have a preprinted form that
can be presented to the certifying doctor to fill out, that satisfies the
"valid written order" requirement? One would think that the
certifying statement form the physician has to also sign would be
enough. Why doesn't Medicare come up with a standardized form that
would incorporate all these requirements they are now enforcing?

QUESTIONS AND COMMENTS WITH RESPONSES
Comment: I just wanted to take a minute and wish all of my former students good
fortune in the coming year. I will not be seeing you in Orlando this year as
the PFA has disbanded the Pedorthic Educators Committee and discontinued a very
long tradition of providing the educators a community table at the event. I
understand that traditional educators no longer hold as much importance to them
as they move forward with lobbying efforts towards a two-year degree program,
on your behalf. You might even get a chance to vote on a bylaw change to
officially do away with the Committee (and the educators' input to the Board),
then, maybe not. If you have an opinion on this matter, you may want to let the
PFA leadership know before the symposium. Their number is (800) 673-8447. Ask
for Brian. If not, please enjoy the flight, taxi ride, hotel and symposium. It
looks to be just about as good as it was last year.
- Rick E. Sevier BOCO C. Ped.
Response: All pedorthic pre-certification organizations were present
at April's educator stakeholders meeting to discuss the future of pre-cert
education. At that meeting, all of the stakeholders reached a majority
and agreed that pedorthic pre-certification education needed to be taken
to the next level in order for future pedorthists to compete on a level playing
field with the rest of O & P. The traditional educators will play
a key role in delivering the future AA degree in pedorthics that is
envisioned. Pedorthists deserve the same opportunities as the rest of the
O & P community to practice to the fullest extent of their scope of
practice; be less burdened with government regulations; provide their
patients/clients with the highest level and widest range of pedorthic services;
enhance the respect and awareness of the pedorthic community of
practitioners amongst other allied healthcare professionals, state and federal
government decision makers, the credentialing organizations and the general
public.
This
year's 2010 North American Pedorthic Congress, jointly produced by PFA and the
Pedorthic Association of Canada, which will be the largest pedorthic
educational event in history. The lineup of general and breakout session
speakers is extraordinary, as are the networking opportunities, social events and
location. This is reflected in the tremendous international response
to exhibit and sponsorship opportunities and attendee registration.
Finally to correct Mr. Sevier's comments about voting on bylaws
changes, the PFA Board of Directors voted in April to disband its
Pedorthic Educators Committee because it was not fulfilling the needs of the
membership or the organization as the rest of PFA's standing committee
structure does. All of the educators were notified of this move
shortly after the board vote. Furthermore, the decision to put this
before a board vote was done after a majority of the educators
themselves expressed that the committee was not working as originally
envisioned, and overall participation on the committee was poor. This
in no way reflects a lessening of the relationship between PFA and the
pedorthic educator community.

Question: With Peripheral Neuropathy being a condition that
does not take time off, what have other pedorthists come up with for patients
who want to swim? I have recently been pondering what would be the best
type of protective footwear for diabetics to use while swimming. I know
patients cannot swim in their regular diabetic shoes. Crocs will
"float" off, aqua sox are tight and seams could rub. I
have a patient who, after months of wearing a pneumatic walking boot, has
finally healed her right 1st mpj ulcer. She has a rigid cavus foot type
and 1st and 5th mpjs are very prominent. She recently went to the local
YMCA and went swimming. She walked barefoot and with no fat pad under 1st mpj
she re-ulcerated and again has an infection and is back in aircast pneumatic
walker. Anybody come across this and have any good ideas?
- Joe Eads C. Ped., COF
Response: Check out Vibram's "FIVE
FINGERS" footwear, I think that would work great.
- PNW Reader
Question: The increasing
popularity of the toning shoes (MBT, Reebok, Skechers, etc.) has led to two
distinct issues in my practice. One is that physicians aren't well
educated on these shoes and view them as simply a "rocker sole shoe"
and are advising their patients with ankle fusions or solid ankle AFOs to buy
them instead of referring them to a pedorthist to have rockers added to their
shoes. The issue of course with this is that these people obviously
lack the range of motion to wear an instability shoe and the super-soft
heel could actually be very dangerous for a person like that.
The second issue is that I am getting a lot of people who insist
on wearing these shoes yet come in with a Rx for custom foot orthoses. In
many cases to me this seems contradictory. My orthoses are often designed
to provide stability and support and then they insist on using them in a shoe
purposely designed to be unstable. I try to explain as thoroughly and
gently as possible, but a lot of times, my expertise doesn't seem to trump
Reebok's and Sketchers' advertising campaigns and/or the trendiness of what
their friends are all wearing.
Question: I don't claim
to be an expert on instability shoes so I would really appreciate any
thoughts/advice on either of these issues? Thanks! - Erick J. Janisse, C.O., C. Ped., National Pedorthic Services, Inc. Response: The fitness/toning category is dominated by the
advertising of Skechers and Reebok, coupled with the brand awareness of MBT. It
doesn't surprise me that both physicians and consumers view these brands as the
only commercially available OTC rocker sole shoes. There are a growing number of heel-to-toe rocker
sole shoe brands that have excellent medial/lateral stability, with many new
brands joining the "stable" side of the category. Finn Comfort's
"Finnamic" brand, "Sano" by Mephisto, Ryn and Cogent are
all existing brands with excellent medial/lateral stability, coupled with
sufficient depth to accept prescription or OTC orthoses. New entries to the
"stable" side of the category include "BodyWorks" by
Aetrex, Waldlaufer and Spring Footwear. Stable rockers are a far better
solution for those with arthritic ankles, ankle fusions and AFOs. - Cam White, CFts, Total Relief Footwear

Board: Aug US Consumer Confidence Index Inches Up To 53.5
By Kathleen Madigan
U.S. consumer confidence
increased more than expected in August, pulled up by expectations about future
economic activity, according to a report released Tuesday. The Conference
Board, a private research group, said its index of consumer confidence rose to
53.5 this month from a revised 51.0 in July, first reported as 50.4. The August
reading was better than the 51.0 expected by economists surveyed by Dow Jones
Newswires. The present situation index, a gauge of consumers' assessment of
current economic conditions, fell to 24.9 from a revised 26.4 in July, first
reported as 26.1. Consumer expectations for economic activity over the next six
months rebounded to 72.5 in August, after plunging to a revised 67.5,
originally reported as 66.6.
"Expectations about
future business and labor market conditions have brightened somewhat, but
overall, consumers remain apprehensive about the future," says Lynn
Franco, director of the Conference Board Consumer Research Center. Full Text

It's
Gotta Be The Shoes
SUSIE GHARIB: Well, it seems like a
simple process, go into a shoe store, see something you like, and try a pair
on. But as we spend more time during the day on our feet, leading longer active
lives, the shoes we wear can make a difference in how we feel. As Jeff Yastine
reports, that's a key strategy in the growth of one footwear chain. Full

Sears
Looks To Boost Online Shoe Savvy
By Sarah Mahoney
As part of its continuing efforts to
shore up its online offerings, Sears says it is launching Sears.com Shoe
Experience Website, a 450-brand emporium that it will push through social
marketing efforts. The retailer hopes its microsite, hosted by fashion stylist
Francesca Mills, will blend the feeling of blogs, social media and commerce
with more engaging -- and even useful -- commentary. For example, a well-known
mom blogger runs down the ins and outs of back-to-school shoe shopping.
The content covers trends for
fashion, kids, fitness, and work. It even includes a feature called
"Personal Shoe Shopper," which allows users to upload photos of an
outfit as well as their style parameters, and within 24 hours, they receive a
selection of shoes. Full

developed neuro-degenerative disease that affects cognitive functions - the
disease characterized by sudden memory loss, behavior, and social adaptation -
and 65% had plaque deposits in their brain. Of the patients that had plaque

Preventing
Complications of Type 2 Diabetes
By Lila Havens
Having type 2 diabetes means either your body doesn't make
enough insulin or your cells don't use insulin properly. Insulin helps sugar
(glucose) move from the blood into the cells, where it is used for energy.
Without the proper amount of insulin, sugar builds up in your blood. High blood
sugar levels can damage nerves and blood vessels throughout your body. This can
cause a range of serious health problems called complications.
Blood glucose monitoring is very important to your health if
you have diabetes. Checking and logging your blood sugar readings shows how
well you are keeping your blood sugar in the target range. It can also help
your doctor know whether your treatment is working and make adjustments as
needed. Good blood sugar control may help prevent or slow the complications of
diabetes. Full Text

choice are discount stores and mass merchants, department stores are gaining
some traction with this demographic.
According to NPD's Consumer Tracking

design, manufacture, modification and/or fit of footwear. Pedorthics
encompasses shoes, orthoses and foot devices that prevent or alleviate foot
problems caused by disease, overuse, injury or congenital defect. Certified
Pedorthists work with patients and their footwear to conform to a doctor's
footwear prescription as part of the patient's treatment
To become a certified pedorthist, you must take a
