NEW QUESTIONS AND COMMENTS
Question: With Peripheral Neuropathy being a condition that
does not take time off, what have other pedorthists come up with for patients
who want to swim? I have recently been pondering what would be the best
type of protective footwear for diabetics to use while swimming. I know
patients cannot swim in their regular diabetic shoes. Crocs will
"float" off, aqua sox are tight and seams could rub. I
have a patient who, after months of wearing a pnuematic walking boot, has
finally healed her right 1st mpj ulcer. She has a rigid cavus foot type
and 1st and 5th mpjs are very prominent. She recently went to the local
YMCA and went swimming. She walked barefoot and with no fat pad under 1st mpj
she re-ulcerated and again has an infection and is back in aircast pnuematic
walker. Anybody come across this and have any good ideas?

Question: The increasing
popularity of the toning shoes (MBT, Reebok, Skechers, etc.) has led to two
distinct issues in my practice. One is that physicians aren't well
educated on these shoes and view them as simply a "rocker sole shoe"
and are advising their patients with ankle fusions or solid ankle AFOs to buy
them instead of referring them to a pedorthist to have rockers added to their
shoes. The issue of course with this is that these people obviously
lack the range of motion to wear an instability shoe and the super-soft
heel could actually be very dangerous for a person like that.
The second issue is that I am getting a lot of people who insist on
wearing these shoes yet come in with a Rx for custom foot orthoses. In
many cases to me this seems contradictory. My orthoses are often designed
to provide stability and support and then they insist on using them in a shoe
purposely designed to be unstable. I try to explain as thoroughly and
gently as possible, but a lot of times, my expertise doesn't seem to trump
Reebok's and Sketchers' advertising campaigns and/or the trendiness of what
their friends are all wearing.
I don't claim to be an expert on instability shoes so I would really
appreciate any thoughts/advice on either of these issues?

Question: There is an
interesting discussion taking place on Podiatry Arena about the effects and
advantages of Barefoot Running. Yet another new footwear explosion. Has anyone
had an experience with them? Since we (Pedorthists) are the foremost experts in
shoes and feet, what are your thoughts?

Comment: I just wanted to take a minute and wish all of my former students good
fortune in the coming year. I will not be seeing you in Orlando this year as
the PFA has disbanded the Pedorthic Educators Committee and discontinued a very
long tradition of providing the educators a community table at the event. I
understand that traditional educators no longer hold as much importance to them
as they move forward with lobbying efforts towards a two-year degree program,
on your behalf. You might even get a chance to vote on a bylaw change to
officially do away with the Committee (and the educators' input to the Board),
then, maybe not. If you have an opinion on this matter, you may want to let the
PFA leadership know before the symposium. Their number is (800) 673-8447. Ask
for Brian. If not, please enjoy the flight, taxi ride, hotel and symposium. It
looks to be just about as good as it was last year.

Footwear Sales Get Pumped Up

When Liz Lusby wants to splurge on a
pair of fancy, must-have boots, she divides the price by the number of times
she expects to wear them, and then divides the result again, to cover each
foot.
"If I break it down that way, it
seems more realistic, and the price isn't that bad," says Ms. Lusby, who works
at an advertising agency.
She's not the only one justifying
her shoe purchases during a recession that has ravaged shoppers and retailers
alike. Across the United States, Ms. Lusby and her ilk have pumped up women's
shoe sales by 9% for the 12 months ended July 31, to $44.2 billion, while total
shoe sales increased 4% over last year, according to trend research firm NPD
Group Inc. Sales of women's apparel grew just 1.2% for the same period.
Department stores and footwear chains alike are feeling the love. The only
losers seem to be local mom-and-pop shops that are getting squeezed by

Sixty-four years ago, Joe Marzano
thought he would be a chemical engineer. But he left Youngstown College to
serve in World War II. When he returned, it wasn't to an engineering career but
rather one in shoes.
"I was just cut out to be a shoe
man; I'm a good fitter," said
Marzano works part time at Diorio
Shoes on U.S. Route 224 in Poland, OH. He began at Kirby's shoe store in
Youngstown, becoming a manager, and later was employed at Mooney's shoe store.
In 1977, Marzano became a certified pedorthist - a specialist in using footwear

Tops Foot Ulcer Healing

advanced biologic therapies, according to an analysis of a larger wound care
database. When used as the initial advanced biologic therapy, engineered skin
-- or bilayered living cell therapy (Apligraf) -- reduced the time to wound
healing by 31% compared with topical recombinant growth factor (becaplermin,

Team Approach to Foot Care Lowers Risk of Amputation in Diabetes

People with diabetic foot problems
can lower their risk of leg amputation by relying on coordinated care that
includes a podiatrist, according to a recent study. For instance, those with
diabetes-related foot ulcers can reduce their risk of amputation by 31 percent.
Diabetes leads to poor circulation and damaged nerves, which make foot injuries
more likely. When injuries occur, diabetes slows the healing process. That is
why 30 percent of people with diabetes over age 40 have diabetes-related foot
problems. If you have diabetes, your lifetime risk of developing a foot ulcer
is 15 percent or more. About a quarter of diabetic foot ulcers refuse to heal
and eventually require amputation.
In a study published online in the journal Health Services Research,
researchers classified diabetic foot problems into four stages:

Stage one: Nerve problems - when the patient loses the ability to feel pain
in feet and might not notice injuries.
- Stage two: Cellulitis - when the cells just below the skin become infected.
In addition, Charcot foot - which involves deteriorated ligaments and
cartilage.

Stage three: Foot ulcers.

Stage four: Gangrene, which could require amputation.

Take Steps to
Ease Stress On Your Feet
Each foot has 28 bones, 33 joints and 107 ligaments.
Our feet strike the ground about 1,800 times during a mile of walking, and 75
percent of us will experience a foot health problem sometime in our life. One
of the trickiest to diagnose is a stress fracture.
Dr. William McGarvey, a foot and ankle surgeon at
Memorial Hermann Sports Medicine Institute, said "insidious" is the
best way to describe it. "People have a vague discomfort that sneaks up on
them," McGarvey said. "It develops over time to where they get used
to it. So they continue to do their activities, and then it gets to the point
where they can no longer tolerate it."
An X-ray may or may not show a stress fracture. There
may or may not be swelling. But always, there is pain, especially upon
prodding. "There is tenderness over one specific area of the bone,"
McGarvey said. "I put my finger on it and it makes the patient sit
straight up - but everywhere else is fine."
An MRI or bone scan can help diagnose a stress
fracture, but McGarvey will usually make the diagnosis based on symptoms. Full Text

Pedorthic
Pre-Certification Training Course January 24th - February 7th, 2011
In cooperation with the ABC and BOC
requirements, the Eneslow Pedorthic Institute (EPI) is offering a
three-level competency training course for aspiring CPEDS. To provide maximum
benefit in a minimum of time, a portion of the level one curriculum will be
administered through a distance learning program that includes required
readings, and testing of all mandatory topics. Upon passing the level one test,
students will attend the remainder of the pedorthic pre-certification course at
the Eneslow Pedorthic Institute. The course includes the required tests for
competency to sit for the ABC or BOC exams.
Upon completing the course and successfully
passing the competency tests, and fulfilling the 1,000 hour "Clinical practice
Requirement" students are eligible to sit for the ABC or BOC exam to earn their
