Question: Many Americans
love to wear flip-flop sandals in the warmer weather. We have always warned
against flip-flops that have no arch support and recommended flip-flops with
good arch support (Birkenstock, Finn Comfort, FitFlop, Mephisto, Merrell, Naot,
Reef, and Teva just to name a few). What are PNW readers learning about the
effects of this footwear on various population demographics, teens and young
adults; overweight individuals, pronators, cavus feet, and others? What
problems are they experiencing, heel pain, tired and aching feet, Achilles
related problems, forefoot pathology, etc? Are certain types of flip-flops
causing the problem more than others? What flip-flop brands are readers
recommending as good for their client's feet?

PREVIOUS QUESTIONS AND COMMENTS WITH RESPONSES
Question: I know I have seen comments on this
subject in the past, but I think it is a gray area. What are some
thoughts on billing for L5000? Some will only bill for this code if the
patient has a complete transmet amputation. Others will bill for complete
transmet amputation as well as hallux amputation. Does a hallux toe
filler meet criteria for an L5000 (Partial foot, shoe insert with longitudinal
arch, toe filler)?

Response: It took me a long time to get this information
from Medicare on L5000 policy and as usual it is rather vague. You can
find this information on www.medicarenhic.com web

Comment: Shane Hayes has posted that my comments on the abhorrent state of
affairs of being a pedorthic Medicare TSD supply company were on the money [for
both comments see the July 21 Issue #374] . . . . but took issue with my
comment about questioning the need to incur the costs of being 'certified' any
longer since I no longer owned a Medicare supply company. Shane's
response did not address the issue I raised of what is ABC and/or PFA doing to
"brand" certified pedorthists from all the non-certified pedorthists
that put Pedorthist on their business card and tell their customers why as a
pedorthist they are qualified to provide SOLUTIONS to their foot
problems.
At least these
individuals are not falsifying their credentials relative to actual certified
pedorthists.
This week I had
the opportunity to be in another local shoe store that the owner who is not
certified, who has never has been certified, yet who over the years has
portrayed himself as being a certified pedorthist in our community.
And lo and behold, there is his Certificate of Attendance for a PFA conference
from several years ago on display behind the cash register. And
after his name on the certificate is C. Ped. Now I have reported
this individual multiple times to ABC about his PNW posting where he added CPed
to his name and about his putting signage on his store about the availability
of a board certified pedorthist when no CPed worked for him, and previously for
displaying this PFA seminar attendance certificate that has CPed after his
name. Yet every customer still gets to see at check out that he is a C.
Ped according to PFA.
Not that any of
the customers would know the difference between a CPed and a Pedorthist, except
for whatever he tells them.
Is ABC
toothless in its ability to protect real certified pedorthists from knock
offs? Is PFA so oblivious about anyone who registers at a PFA
seminar may or may not be a CPed so no one at PFA checks their credentials (or
lack of) on the ABC or BOC websites before putting CPed or BOC Pedorthist on an
attendance certificate? I think the answer to both these question
is obvious in my community. How many other CPeds are spending a
great deal of money each year to renew their ABC or BOC certificate, spending
money to get the continuing ed hours to maintain their certification only to
have individuals falsely pose as a CPed in their market? Or due to the
lack of public understanding (branding), the uncertified pedorthist gets to
tell the customer their version of their skills and knowledge.
So Shane, the
question I posed early about whether or not I should keep spending money to be
a CPed now that I shut down my Medicare TSD supply company is a real question
that needs a legitimate answer. And meaningful actions to protect its
paying CPed practitioners, ABC. And common sense before handing out
certificates, PFA.

Response:I couldn't agree with John more.
About six or seven months ago I informed
ABC about an employee of a multi-location podiatric practice being
listed on their website as a "C. Ped." but who was not listed on
ABC's or BOC's website as being credentialed.
I just checked and nothing has changed. And, as
far as I know, there was no follow-up by ABC at all, this despite the fact that
the term "C. Ped." is trademarked.
I closed my pedorthic practice in part because
the costs, risks, and requirements now mandated to maintain a Medicare
Supplier Number and went work for a podiatrist. I have none of the
overhead, no back office paperwork to do, do not have to worry about
scheduling or appointment reminders, and am back to enjoying working with
patients and doing the work I enjoy.
And if ABC (and BOC?) are as unconcerned
as they appear about promoting and protecting the entegrity of the C.Ped.
credential, I also have to wonder....is keeping up with CEU's, etc, worth
the expense of maintaining certification?

Response: 1)While I realize one of your roles on the Advisory Board for
PNW is to be provocative to create readership - you're now bordering on a
puerile bit of commentary. When you minimize the scope of pedorthics to
nothing more than a diabetic shoe fitter with the TSD, you're undermining
pedorthics more than the non-certified person you referenced. As I've
previously stated, your skill set and the scope of pedorthics is far greater
than the TSD and will survive the TSD.
2) If you, or any of the readership, entered
pedorthics for the sole purpose of the TSD and no longer participate, then it's
"Mission Accomplished" like Bush in 2003. It's clearly anyone's
choice, as a one dimensional practitioner to bow out and leave pedorthics to
those of us who are serious enough to utilize our full scope of practice,
thriving, respected in the medical community and have the gratitude of our
multi-faceted patients. I would propose that those C. Peds with limited
skills set a new goal to get seriously involved in pedorthics, expand their skill/educational
base and maybe even run for the board to actually make a difference; verses
standing in the corner talking loud.
3) Nothing wrong with displaying an
"attendance" certificate, but the existence of unethical people in all
professions making questionable claims stinks. When one does exercise the
common sense you reference, they realize that a clerical error is not a policy
issue. The self-aggrandizement referenced is an ethical rather than legal
issue. While I'm not a legal counsel for ABC or BOC, I'm pretty sure their
legal jurisdiction covers only their membership. Direct communication with them
may solve the issue? Non-members might be a civil fraud matter that your
legal dollars could pursue? The law however, is a two-way street opening
the door for both plaintiff and defendant to be investigated (i.e. rocks &
glass houses).
4) Again, your questions of ABC, BOC & PFA
about their "branding" efforts should be sent to those organizations
directly. I know over the past year, PFA has answered the question in
Pedorthics Online, Current Pedorthics and their website. Believe it or
not, reading something besides PNW can broaden your horizons.

documentation now required?

Question: I was wondering
what the regulations on prescription are regarding RX/DX information on the
prescription. I get several prescriptions for Therapeutic footwear and all that
is written on the prescription is "Diabetic shoes." Most of the time
there is no DX at all or it may say "diabetic." Is this enough to go
ahead and see/evaluate the patient (covered under Medicare) and order footwear
under the therapeutic footwear bill?
Is
the prescription or the physician statement for therapeutic footwear the
priority document when providing diabetic footwear?

BOC Honors Founder Dr. Fedder
The Board of
Certification/Accreditation, International (BOC) has honored Donald O. Fedder,
DrPH, MPH, BSP, FAPhA, BOC's founder and first CEO, by dedicating its new
headquarters wing to him. The "Don Fedder Suite" expands BOC's Owings Mills, MD
offices by more than one-third, to a total of nearly 4,000 square feet.
All four of BOC's CEOs -
Fedder; James Newberry, Jr., BOCPO, BOCPD, who succeeded Fedder; Greg Safko,

ABC Announces
Additional Exam Dates for 2011
The American Board for
Certification in Orthotics, Prosthetics and Pedorthics, Inc. (ABC) has added a
new set of dates for administering its certification exams for 2011. The
new dates will give ABC a total of four exam administrations each year,
allowing candidates additional opportunities to take the exams.
The dates for the 2011 ABC
certification examinations for orthotists, prosthetists, pedorthists, orthotic
fitters, mastectomy fitters and therapeutic shoe fitters are:

February 14-19
·
May 9-14
· August 8-13
· November 14-19
The ABC Board made the
decision to add the additional exam administration to increase candidate access
to the exam. The change also reflects the changing academic environment
where orthotic, prosthetic and pedorthic education occurs year round as opposed
to the traditional academic year. "This change allows candidates for the
ABC board exams more convenient access and gives those students who complete
their education at various times throughout the year more opportunity to sit
for their exam," said Catherine A. Carter, ABC Executive Director.

Pedorthist Puts the Right Foot Forward

long as they take us where we want to go, we tend to take our feet for granted.
But it turns out they are complicated appendages. "Each
one contains 26 bones, and when you take a step, every one moves,'' said
Michael McHale, the owner of the Cobbler Shop in Mount Holly. McHale is an expert in all matters relating
to the foot. So when he tells his customers that the biggest cause of foot
problems are ill-fitting shoes, they tend to listen. "Sixty
to 70 percent of people who are having problems are wearing shoes that are the
wrong size and are improperly fitted,'' he said. A
Pennsylvania native who lives in Southampton, McHale, 59, learned the
shoe-repair profession from his father. He quickly realized there was more to
it than the old-fashioned image of the friendly cobbler hammering nails into

Dog
Amputates his Diabetic Master's Toe - Possibly Saves His Life
Kiko the terrier not only detected his master's diabetes, he
went so far as to perform surgery - chewing off his owner's toe, and possibly
saving his life. The Jack Russell terrier apparently sensed an infection
festering in his master's right big toe - and, unlike his master, took steps to
resolve the situation.
A trip to the hospital afterwards confirmed that Jerry
Douthett's toe required amputation, and Douthett credits the dog
