Question: Does anyone have a clear understanding of the issue(s) concerning physicians with and without PECOS numbers and billing Medicare for therapeutic shoes, and medical supplies for that matter? Also, does anyone have a clear understanding on physician's PROGRESS NOTES and documentation now required?

Comment: Shane Hayes has posted that my comments on the abhorrent state of affairs of being a pedorthic Medicare TSD supply company were on the money [for both comments see the July 21 Issue #374] . . . .  but took issue with my comment about questioning the need to incur the costs of being 'certified' any longer since I no longer owned a Medicare supply company.   Shane's response did not address the issue I raised of what is ABC and/or PFA doing to "brand" certified pedorthists from all the non-certified pedorthists that put Pedorthist on their business card and tell their customers why as a pedorthist they are qualified to provide SOLUTIONS to their foot problems.  

 

At least these individuals are not falsifying their credentials relative to actual certified pedorthists.

 

This week I had the opportunity to be in another local shoe store that the owner who is not certified, who has never has been certified, yet who over the years has portrayed himself as being a certified pedorthist in our community.   And lo and behold, there is his Certificate of Attendance for a PFA conference from several years ago on display behind the cash register.   And after his name on the certificate is C. Ped.   Now I have reported this individual multiple times to ABC about his PNW posting where he added CPed to his name and about his putting signage on his store about the availability of a board certified pedorthist when no CPed worked for him, and previously for displaying this PFA seminar attendance certificate that has CPed after his name.  Yet every customer still gets to see at check out that he is a C. Ped according to PFA. 

 

Not that any of the customers would know the difference between a CPed and a Pedorthist, except for whatever he tells them.

 

Is ABC toothless in its ability to protect real certified pedorthists from knock offs?   Is PFA so oblivious about anyone who registers at a PFA seminar may or may not be a CPed so no one at PFA checks their credentials (or lack of) on the ABC or BOC websites before putting CPed or BOC Pedorthist on an attendance certificate?   I think the answer to both these question is obvious in my community.   How many other CPeds are spending a great deal of money each year to renew their ABC or BOC certificate, spending money to get the continuing ed hours to maintain their certification only to have individuals falsely pose as a CPed in their market?  Or due to the lack of public understanding (branding), the uncertified pedorthist gets to tell the customer their version of their skills and knowledge.

 

So Shane, the question I posed early about whether or not I should keep spending money to be a CPed now that I shut down my Medicare TSD supply company is a real question that needs a legitimate answer.  And meaningful actions to protect its paying CPed practitioners, ABC.  And common sense before handing out certificates, PFA.

Comment: I'm looking forward to the 2010 North American Pedorthic Congress this November. With an impressive line-up of speakers and topics, I'm sure it will be a great event. However, with hotel fees at $200/night plus all the expenses incurred for traveling, I'm wondering how many of us are going to be able to financially afford to be there. Ironically, I'm one of the speakers. Now I'm kicking myself because I have no idea where the money is going to come from to get me there.

 

Is anyone else in this same boat? I don't know about other C. Peds but in the 5+ years I've been in this profession, my salary has barely provided me with enough money to live on. I've yet to earn enough income to put a penny in a savings account and am wondering if it's even worth staying in the profession.

 

A year and a half ago I was laid off from a job as a C. Ped. I spent 6 months looking for work as a C. Ped. in five different states. I was offered $11-12/hour for part-time work. I decided I was better off being unemployed and waited for something 'better.' But the better still isn't good enough.

 

I have no idea what other C. Peds are earning. But I know I didn't work my butt off for a Bachelor of Science Degree in a health/medical field and earn my Certification in Pedorthics to be right on the edge of poverty.

 

I've yet to work for an employer who will pay for me to keep up all of my Certification points, PFA membership, and annual ABCOP dues. I don't necessarily expect an employer to do that, but each year I struggle to come up with the money to contribute towards all of these necessary expenses.

 

All I can say is it's very discouraging. I pursued the profession because I loved it. But it's getting me nowhere...I can no longer keep my head above water.

 

I'd be interested to hear input from other C. Peds.

Response: This guy surely wasn't looking for work in my area, or we would have swept him off his feet!  My part-time NON-credentialed people make $10 an hour and they don't have college degrees.  If he really knows SOMETHING  (which being certified does NOT prove!)  I would have certainly hired him full time at a SIGNIFICANTLY higher salary and I would have been thrilled to take care of CEU's and memberships.  I always have paid for these for my C. Peds.  Maybe we just do things differently down South.

Response: When you're up to your butt in alligators, it's hard to remember that your objective was to drain the swamp.  Your debt picture has fogged up your vision.  While there is indeed light at the end of the tunnel, I can't bite my tongue long enough to impart that wisdom first; you'll have to wait for a paragraph or two. 

 

As an individual who started working outside of my home in grade school and never stopped, I have a low tolerance for those who expect others to pay for their education (initial or continuing education).  "Waiting for something better"?  This is Aesop's fable of the grasshopper spending the warm months singing away, while the ant worked to store up food for winter. 

 

Every year I hear somebody utilizing your very same verbiage of woe is me.  Create the opportunity for yourself. No matter what the expense, you are about to have the opportunity to attend the largest pedorthic gathering in history.  If you can't create an opportunity for yourself with all the potential employers, knowledgeable colleagues, a host of helpful vendors [with connections] who are attending; then I'm not sure how to help you.  Make a plan, figure out your parameters and hit Orlando hard with an open mind.

Question: I have neuropathy CMT with drop foot that makes walking difficult.

I wear braces whenever I'm going to walk outdoors for a while. I have difficulty keeping my balance if the surface is slightly uneven and it's hard for me to walk up a slight incline.

 

I would like to know - whenever I'm not wearing braces, would it be of any help to wear orthotics to walk around the house or when going for short walks?I asked this question to a doctor, who said, "Wearing orthotics for the above reasons would not help."

 

I would very much appreciate any information you can give me.

Response: I have patients with C-M-T, that, when they have some flexibility, can do well with a heel +/- sole flare on the outside of their shoes.  It is fairly cost effective vs. orthotics at times, can help reduce some of the lateral roll to the foot. I concur with the opinions re: orthotics at times being faulty in design.  I have a patient in her mid-20's that we do a flare for, that worked wonderfully while she was in college.

Question: I was wondering what the regulations on prescription are regarding RX/DX information on the prescription. I get several prescriptions for Therapeutic footwear and all that is written on the prescription is "Diabetic shoes." Most of the time there is no DX at all or it may say "diabetic." Is this enough to go ahead and see/evaluate the patient (covered under Medicare) and order footwear under the therapeutic footwear bill?

 

Is the prescription or the physician statement for therapeutic footwear the priority document when providing diabetic footwear?

ABC Supports NCOPE Move to Master's Level Standards for O&P

 

Alexandria, VA - The American Board for Certification in Orthotics, Prosthetics and

Pedorthics, Inc. (ABC) congratulates The National Commission for Orthotic and Prosthetic Education (NCOPE) on revising the orthotic and prosthetic education standards, raising the minimum education level for the profession to a master's degree.  NCOPE received approval of the standards revision at the July 23rd meeting of the Commission on Accreditation of Allied Health Education Programs (CAAHEP).

 

NCOPE's hard work over the last five years to revise the standards has culminated in an exciting move forward for the profession. The new standards are effective immediately for any new programs seeking accreditation and all currently accredited programs will transition to the new standards by the end of 2012.

New York: Consumers Will Soon Pay More For Clothes, Footwear

 

Shoppers statewide will soon be paying more for clothing and footwear items under $110. A state sales tax exemption will be lifted in October - meaning an added 4 percent on those items. Monroe County taxes consumers 4 percent on footwear and clothing, beginning October 1st, shoppers will pay 8 percent. Full Text

NPD: Consumers spending more, but shopping later for BTS
 
The NPD Group found that 38% of consumers polled in its back-to-school spending intentions survey plan to spend less this year, compared with 44% of respondents a year ago. Consumers are continuing to put off shopping until the last minute. Half the number of consumers this year (3%) compared with last year (6%) said they have purchased for back-to-school already, NPD reported. 
 
"More and more consumers keep telling us that they are putting off their shopping later and later each year," said Marshal Cohen, chief industry analyst, The NPD Group. State of Mind and Healing Linked in Diabetes Foot Ulcer Study

Scientists have found that healing of diabetes related foot ulcers differs depending on psychological state of mind. Researchers found a link between how patients with diabetes cope with potentially life threatening foot ulcers and rates of healing. The results of the study showed that anger, frustration and depression over potentially life threatening wounds associated with microvascular diabetes complications slows the rate of healing.

 

Professor Kavita Vedhara from the University of Nottingham says, "My colleagues and I believe that this confrontational approach may, inadvertently, be unhelpful in this context because these ulcers take a long time to heal. As a result, individuals with confrontational coping may experience distress and frustration because their attempts to take control do not result in rapid improvements." Clinical Pharmacology Study Group (CPSG) Announces Open Enrollment for Diabetic Neuropathy

The Clinical Pharmacology Study Group (CPSG) is currently participating in several clinical trials for diabetic peripheral neuropathy in its Worcester, MA facility.

Diabetic neuropathy is a common complication of diabetes in which nerves are damaged as a result of high blood sugar levels. Symptoms of diabetic neuropathy include pain, tingling, or numbness and loss of feeling in the hands, arms, feet, and legs. Nerve problems can occur in every organ system, including the digestive tract, heart, and sex organs. About 60 to 70 percent of people with diabetes have some form of neuropathy. This clinical study is specifically for diabetic peripheral neuropathy.

Subjects who enroll in a clinical trial are carefully selected from a list of inclusion and exclusion criteria designed to ensure safety for the subject. Patients' selected to participate in this study are followed by a team of experts in diabetic peripheral neuropathy. All study related medications and care are at no cost to the patient. The study group works closely with the patient care provider (PCP), or other specialist, to ensure that all the medical professionals are involved in the treatment of the patient. Full Text
