NEW QUESTIONS AND COMMENTS
Question: Does anyone have a clear understanding of the
issue(s) concerning physicians with and without PECOS numbers and billing
Medicare for therapeutic shoes, and medical supplies for that matter? Also, does
anyone have a clear understanding on physician's PROGRESS NOTES and
documentation now required? - Stuart Pressman, Florida
PREVIOUS QUESTIONS AND COMMENTS WITH RESPONSES
Comment: I'm looking forward to
the 2010 North American Pedorthic Congress this November. With an impressive
line-up of speakers and topics, I'm sure it will be a great event. However,
with hotel fees at $200/night plus all the expenses incurred for traveling, I'm
wondering how many of us are going to be able to financially afford to be
there. Ironically, I'm one of the speakers. Now I'm kicking myself because I
have no idea where the money is going to come from to get me there. Is anyone else in this same boat? I don't know
about other C. Peds but in the 5+ years I've been in this profession, my salary
has barely provided me with enough money to live on. I've yet to earn enough
income to put a penny in a savings account and am wondering if it's even worth
staying in the profession. A year and a half ago I was laid off from a job
as a C. Ped. I spent 6 months looking for work as a C. Ped. in five different
states. I was offered $11-12/hour for part-time work. I decided I was better
off being unemployed and waited for something 'better.' But the better still
isn't good enough. I have no idea what other C. Peds are earning.
But I know I didn't work my butt off for a Bachelor of Science Degree in a
health/medical field and earn my Certification in Pedorthics to be right on the
edge of poverty. I've yet to work for an employer who will pay
for me to keep up all of my Certification points, PFA membership, and annual
ABCOP dues. I don't necessarily expect an employer to do that, but each year I
struggle to come up with the money to contribute towards all of these necessary
expenses. All I can say is it's very discouraging. I
pursued the profession because I loved it. But it's getting me nowhere...I can
no longer keep my head above water. I'd be interested to hear input from other C.
Peds. - PNW Reader Response: This guy surely
wasn't looking for work in my area, or we would have swept him off his
feet! My part-time NON-credentialed people make $10 an hour and they
don't have college degrees. If he really knows SOMETHING (which
being certified does NOT prove!) I would have certainly hired him full
time at a SIGNIFICANTLY higher salary and I would have been thrilled to
take care of CEU's and memberships. I always have paid for these for my
C. Peds. Maybe we just do things differently down South. - Jean Sears, C. Ped. Response: When you're up to
your butt in alligators, it's hard to remember that your objective was to drain
the swamp. Your debt picture has fogged up your vision. While there
is indeed light at the end of the tunnel, I can't bite my tongue long enough to
impart that wisdom first; you'll have to wait for a paragraph or two. As
an individual who started working outside of my home in grade school and never
stopped, I have a low tolerance for those who expect others to pay for their
education (initial or continuing education). "Waiting for something
better"? This is Aesop's fable of the grasshopper spending the warm
months singing away, while the ant worked to store up food for winter. Every
year I hear somebody utilizing your very same verbiage of woe is me. Create the opportunity for yourself. No matter what the expense, you are
about to have the opportunity to attend the largest pedorthic gathering in
history. If you can't create an opportunity for yourself with all the
potential employers, knowledgeable colleagues, a host of helpful vendors [with
connections] who are attending; then I'm not sure how to help you. Make a plan, figure out your parameters and
hit Orlando hard with an open mind. -
Shane Hayes, C. Ped.
Question: I have neuropathy CMT with drop foot that
makes walking difficult. I wear braces whenever I'm going to walk
outdoors for a while. I have difficulty keeping my balance if the
surface is slightly uneven and it's hard for me to walk up a slight
incline. I would like to know - whenever I'm not wearing
braces, would it be of any help to wear orthotics to walk around the
house or when going for short walks?I asked this question to a doctor, who said, "Wearing
orthotics for the above reasons would not help." I would very much appreciate any information
you can give me. - Frank Biondo Response: I have patients with C-M-T, that, when they
have some flexibility, can do well with a heel +/- sole flare on the outside of
their shoes. It is fairly cost effective vs. orthotics at times, can help
reduce some of the lateral roll to the foot. I concur with the opinions re:
orthotics at times being faulty in design. I have a patient in her
mid-20's that we do a flare for, that worked wonderfully while she was in
college. - David Castellanos, C. Ped, National Pedorthic
Services, Madison, WI Response: Thanks so much [these] responses....They helped
me understand a little more of what's involved in getting or not getting
orthotics. - Frank Biondo
PREVIOUS QUESTIONS WITHOUT RESPONSES
Question: I was wondering
what the regulations on prescription are regarding RX/DX information on the
prescription. I get several prescriptions for Therapeutic footwear and all that
is written on the prescription is "Diabetic shoes." Most of the time
there is no DX at all or it may say "diabetic." Is this enough to go
ahead and see/evaluate the patient (covered under Medicare) and order footwear
under the therapeutic footwear bill? Is
the prescription or the physician statement for therapeutic footwear the
priority document when providing diabetic footwear? -

ANNOUNCEMENTS
BOC Participates in CEO Forum on
Certification
The
Board of Certification/Accreditation, International (BOC) participated in an
invitation-only CEO Forum in mid-July, sponsored by the Institute for
Credentialing Excellence (ICE). Accredited by the American National Standards
Institute as a Standards Developer, ICE offers NCCA accreditation to
credentialing programs. NCCA
is the acronym for The National Commission for Certifying Agencies; NCCA uses a
peer review process to evaluate the administrative, technical and psychometric
aspects of how a certification's requirements and assessment tools are
developed and maintained. The process includes a detailed review of the
development of certification requirements and psychometric assessment tools,
including the qualification and diversity of subject matter experts. NCCA
accreditation demonstrates that a certification program is valid and reliable.
NCCA accredits numerous healthcare credentialing bodies' certification
programs, including the American College of Sports Medicine, the American
Nurses Credentialing Center, and the National Board for Respiratory Care. All
BOC certifications are NCCA-accredited. Full Text Source: BOC
ABC Achieves Revisions to Taxonomy Code
Descriptions and Obtains First Time Pedorthist Code he American Board for Certification in
Orthotics, Prosthetics and Pedorthics, Inc. (ABC) is proud to announce
mostly positive changes to the Taxonomy Code from the National Uniform Claim
Committee (NUCC). ABC requested changes
to the existing code descriptions for orthotists, prosthetists, mastectomy and
orthotic fitters in order to better reflect contemporary practice in these
professions. ABC also launched the
effort to establish a specific code for pedorthists, who previously did not
have a taxonomy code. Through ABC's efforts, these changes become effective
October 1, 2010. ABC disagrees with NUCC's decision not to
establish a specific taxonomy code for therapeutic shoe fitters and will resubmit
its request at the earliest possible opportunity. Further, ABC will seek an appeal regarding
the language include by NUCC in the orthotic fitter description as
it inappropriately includes custom orthoses. Full Text Source: ABC The
Midwest Chapter of The American Academy of Orthotists & Prosthetists is holding its annual meeting in the Grand Geneva Resort, WI in

Your
Flip-Flops Might Land You in the ER
By
Jennifer Barton
You
may think you're doing your feet a favor by slipping on a pair of flip-flops,
but it turns out the ubiquitous summer sandals are more perilous than they
appear.
In
the U.K. alone, the National Health Service spends the equivalent of $62
million a year
treating injuries, falls and long-term problems caused by the footwear,
with over 200,000 people going to seeking medical treatment as a result of

Soles4Souls, the Shoe Charity, Currently Giving Away One Pair of Shoes Every 7 Seconds
Soles4Souls
Inc., the charity dedicated to providing free shoes to people in need,
announced today that it is currently giving away one pair of shoes every seven
seconds. The non-profit organization provides new, gently worn and
refurbished shoes to people in need around the world.
"We
keep track of this figure so we can show our supporters how their donations are
changing lives every day," said Wayne Elsey, the Founder and CEO of the
charity. "The incredible response we've received this year has
helped us draw closer to our ultimate goal of giving away a pair of shoes every
second," he said.
Soles4Souls
first began to keep track of how many shoes it was donating in July 2007, when
staff members calculated they were giving away a pair every 28 seconds.
"Since then, our charity has gained 45 employees, thousands of
corporate sponsorships and millions of individual supporters," said Elsey.
"This success is due to the valuable partnerships we're fortunate to
have, from the largest footwear companies to the smallest neighborhood school,
and the people who make up each organization," he said. Full Text

Grandma Shoes Get Glam
By
Navdeep Kaur Marwah
Comfort
is the new cool when it comes to shoes. Fashionistas across the world seem to
have developed a newfound fetish for unconventional varieties of footwear,
including the Worishofer sandals-orthopaedic demi-wedges with a cork sole born
70 years ago in the Bavarian town of Bad Wörishofen. 'Grandma shoes', as they
are commonly called, lace up the front, revealing a demure hint of toenail, and
have a wide base for ease of motion. Celebrities such as Maggie Gyllenhaal,
Michelle Williams and Kirsten Dunst have been spotted wearing Worishofers
recently. Comfort is the driving force, say designers.
"After wearing high heels for years and standing on your feet for hours
together, these are an obvious choice. They're cushioned and foot-friendly,"
says footwear designer Rina Shah. Full Text

Urges Action on Bill Closing Internet Sales Tax Loophole
The
National Retail Federation urged Congress last week to move quickly on
legislation that would make it easier for states to require Internet retailers
and other out-of-state merchants to collect sales tax the same as local
bricks-and-mortar stores.
"This
bill would create a level playing field between online merchants and local
stores while ensuring that state and local governments can collect the sales
tax revenue they need to support vital services like police and fire
departments, ambulances and schools," NRF Vice President and Government and
Industry Relations Counsel Maureen Riehl said. "The merchandise sold online is
no different than what is sold in a store. There is no reason one group of
merchants should be given an unfair price advantage over another." Full

Prevention Programs for Foot Ulcers, Amputations Needed in Patients with Diabetes
Programs for the prevention of foot
ulcers and amputations in patients with type 2 diabetes are lacking, and
researchers suggest that appropriate prevention services may significantly
improve the high rate of amputation in this patient population.
Researchers pooled data from
electronic medical records to identify 150 patients on dialysis and 150
patients with a previous foot ulceration or amputation between 2000 and 2006.
Patient education, podiatry care and the use of professionally fitted
therapeutic shoes and insoles were monitored for 30 months. Full Text

Injury Spotlight: Plantar Fasciitis
By Casey Gisclair
San Antonio Spurs guard Tony Parker caught the basketball at the top of the key
and began to sprint toward the basket.
What comes next for the All-Star is usually like clockwork - a sharp crossover
dribble, then a teardrop floater for a quick two points.
But during this past season, things didn't always end that way for Parker,
because he suffered from plantar fasciitis, an injury that limited his jumping
and running ability and sent him to his worst season in five years.
"I'm a little bit slower, that's why," Parker told San Antonio
reporters when asked about his drop in output. "I don't think it's much
different. It's just my plantar fasciitis is killing me."
But what exactly is this injury, and why does it take so long to heal?
In this version of our injury series, SportsNet hopes to answer all of those
questions, plus many more about the painful foot injury that plagues thousands
of athletes each year. Full Text

BECOMING A CERTIFIED PEDORTHIST | |
Join A Growing Service Profession: Pedorthics
Pedorthics is a professional field covering the
design, manufacture, modification and/or fit of footwear. Pedorthics
encompasses shoes, orthoses and foot devices that prevent or alleviate foot
problems caused by disease, overuse, injury or congenital defect. Certified
Pedorthists work with patients and their footwear to conform to a doctor's
footwear prescription as part of the patient's treatment
To become a certified pedorthist, you must take a
pre-determined number of course hours in pedorthics and pass a certifying
examination overseen by either The American Board for Certification in
Orthotics, Prosthetics & Pedorthics® or Board for
Certification/Accreditation, Int'l. Both the ABC and BOC are independent
certification bodies that set standards for the profession and offer
certifications that are recognized by the National Commission for Certifying
Agencies.
