NEW QUESTIONS AND COMMENTS
Comment: I'm looking forward to
the 2010 North American Pedorthic Congress this November. With an impressive
line-up of speakers and topics, I'm sure it will be a great event. However,
with hotel fees at $200/night plus all the expenses incurred for traveling, I'm
wondering how many of us are going to be able to financially afford to be
there. Ironically, I'm one of the speakers. Now I'm kicking myself because I
have no idea where the money is going to come from to get me there. Is anyone else in this same boat? I don't know
about other C. Peds but in the 5+ years I've been in this profession, my salary
has barely provided me with enough money to live on. I've yet to earn enough
income to put a penny in a savings account and am wondering if it's even worth
staying in the profession. A year and a half ago I was laid off from a job
as a C. Ped. I spent 6 months looking for work as a C. Ped. in five different
states. I was offered $11-12/hour for part-time work. I decided I was better
off being unemployed and waited for something 'better.' But the better still
isn't good enough. I have no idea what other C. Peds are earning.
But I know I didn't work my butt off for a Bachelor of Science Degree in a
health/medical field and earn my Certification in Pedorthics to be right on the
edge of poverty. I've yet to work for an employer who will pay
for me to keep up all of my Certification points, PFA membership, and annual
ABCOP dues. I don't necessarily expect an employer to do that, but each year I
struggle to come up with the money to contribute towards all of these necessary
expenses. All I can say is it's very discouraging. I
pursued the profession because I loved it. But it's getting me nowhere...I can
no longer keep my head above water. I'd be interested to hear input from other C.

Comment: I just received an
update from one of the shoe manufacturers outlining what components they think
are needed to be included in a patient exam when dispensing Therapeutic Shoes.
The following excerpt is from page 18 of the CMS DMEPOS Standards, currently

Therapeutic Shoes and Inserts: Perform
an in person diagnosis-specific functional clinical examination as related to
the beneficiary's use and need of the orthoses/prostheses (e.g., sensory
function, range of motion, joint stability, skin condition (integrity, color,
and temperature), presence of edema and/or wounds, vascularity, pain, manual
muscle testing, compliance, cognitive ability and medical history); ANYTIME
your business is on the line, go to the source, NOT three salesmen sitting in
an office with a fax machine, for your information. Caveat

PREVIOUS QUESTIONS AND COMMENTS WITH RESPONSES
Question: I have neuropathy CMT with drop foot that
makes walking difficult. I wear braces whenever I'm going to walk
outdoors for a while. I have difficulty keeping my balance if the
surface is slightly uneven and it's hard for me to walk up a slight
incline. I would like to know - whenever I'm not wearing
braces, would it be of any help to wear orthotics to walk around the
house or when going for short walks? I asked this question to a doctor, who said, "Wearing
orthotics for the above reasons would not help." I would very much appreciate any information
you can give me. - Frank Biondo Response: CMT patients typically have a high degree
cavo-varus foot. This foot type is prone to inversion instability (rolling out)
and ankle sprains. A typical foot orthotic will not address the foot drop
issue, but you can control the instability with the proper shoe/device
combination. Unfortunately, I see too many of these feet in a rigid device with
a very high arch and no accommodation for the plantar flexed 1st ray. Ask your
practitioner to do a Coleman Block test on you. It's very simple and it will
determine two things. If you correct on the block, that means you have a
forefoot driven cavus foot. Your big toe joint is lower than the adjacent
metatarsal joints so it acts like a kickstand and locks your foot up (this
causes your heel to tilt out and be unstable). If you correct, it also tells us that your
hind foot is flexible enough to correct with the proper device. Some
practitioners know how to properly fabricate a device for that foot. Sadly,
many do not. This is not a slam but an observation I have made from seeing
several hundred cavus patients per year with a bag full of non-functional
devices. You may want to consider a light-weight hiking boot. The higher cuff,
rocker sole and shank will help in with side-to-side stability as well as toe
off. Perhaps you can tell us what area you are from and maybe you can be
directed to someone that can help. If you don't have any luck with that, here

through any clinic or O&P shop. - Brian Graham C. Ped. Response: We have a
number of cases with neuropathy and drop foot, and a few of them just flat
out refuse to wear any type of brace. We have modified the shoes, sandals and
boots with leather on the tip, sometimes almost half of the sole depending on
the case, which prevents them from tripping over the drop foot, and
that has worked well. I would imagine though that because of the CMT you
would need a total contact insert as well.It also depends on how bad your drop foot is. - A. Powell, C. Ped.
Question: Here in rural Oklahoma one of my challenges is fitting diabetic shoes
and socks for patients with severe edema and/or obesity. We want lots of volume
across the instep and forefoot area, but a shoe cut low around the heel to
accommodate the ankle girth. The only one I've heard of is by Apis Footwear.
Does anyone have any other suggestions? Does anyone know of some seam-free
diabetic socks that can accommodate lower legs with a circumference of 23" and
over?? - P. Mitchel, C. Ped. Response: Realizing the critical
connection between foot and ankle edema and proper sock (and shoe!), I
think you might be missing an opportunity to be of clinical importance to the
patient and the doctor if you do not interject, when appropriate, the role
of graduated compression supports in controlling the swelling in the first
place: 1) unmanaged edema is in itself and unhealthy condition (contributing to
hypertension increased cardiac output demands, stasis dermatitis, skin
breakdown, etc.); and 2) it is usually painful, motion restricting and, as you
have noted, makes proper diabetic footwear fit and function
problematic. Not within the C. Ped's scope of practice you
say? Not necessarily. Supplying graduated compression
supports requires no licensure or doctor's order in my neck of the woods
(Texas) and probably doesn't in yours either. Yes, it requires extensive
training and experience to do correctly, but over the years, I've had much
success fitting them -- with grateful patients and doctors (who sometimes
have to be prompted in their use) thanking me regularly. It's an almost
exclusive cash business and a diversification away from Medicare
therapeutic shoe and insert payments (see Shane Hayes' comments in previous
postings in earlier issues of PNW). I've never understood why more C.
Peds are not more involved in this area. (You should know that I have no axe to grind on
this subject. I am not paid by anyone who supports those manufacturers --
I'm just a clinician looking to help solve the problems I see every day.) - James Garner, R.Ph., C. Ped., C.O. Response: Thorlos Padds and

Question: I was wondering
what the regulations on prescription are regarding RX/DX information on the
prescription. I get several prescriptions for Therapeutic footwear and all that
is written on the prescription is "Diabetic shoes." Most of the time
there is no DX at all or it may say "diabetic." Is this enough to go
ahead and see/evaluate the patient (covered under Medicare) and order footwear
under the therapeutic footwear bill? Is
the prescription or the physician statement for therapeutic footwear the
priority document when providing diabetic footwear? -

Question: No, really? Just received the latest PFA Magazine. There is an
advertisement for High Heel orthotics on page 17. Is this for real? I cannot
see how any orthotic would do any good in any high heel (especially in those
"exquisite spikes" shown). How can any orthotic give any support in a
high heel shoe. Didn't "Oprah" have something like this on one of her
shows? This brings me to the following question: should there be advertising
for very questionable products in a health related magazine? As a C. Ped. I see
myself as a footwear specialist who works toward long-term
maintenance of healthy feet and the prevention not the promotion of foot

ABC Obtains
Taxonomy Code for Pedorthists The American Board for Certification in
Orthotics, Prosthetics and Pedorthics, Inc. (ABC) is proud to announce that it
has successfully secured a unique Taxonomy Code for Pedorthists from the
National Uniform Claim Committee (NUCC).
Through ABC's efforts, this new code was approved on July 1, 2010. The Level II taxonomy code for Pedorthists is
224L00000X and will become effective on October 1, 2010. Taxonomy
codes, also called Specialty Codes, are an administrative code set for
identifying the provider type and area of specialization for health care
providers. These codes are maintained by NUCC and selected by providers to
indicate their specialty area of practice. Some payers are beginning to request
a provider's taxonomy code to identify their specialty designation/s during an
enrollment process. As the move towards better utilization of electronic data
exchange continues, taxonomy codes are now being linked within a provider's
enrollment or re-enrollment; including the process to obtain a National
Provider Identifier through the National Plan & Provider Enumeration
System. It should be noted that
taxonomy codes are self selected and does not infer an expansion of an
individual's ability to provide certain services. ABC believes that
taxonomy codes should accurately reflect the nationally recognized scope of
practice of credentialed and/or licensed individuals and the new specialty code
for Pedorthists accomplishes that goal. According
to Cathy Carter, Executive Director of ABC, "The creation of a specific
taxonomy code for pedorthists is something that ABC feels is an important step
in the continuing effort to raise awareness of the profession and sustain its
growth for the future. Obtaining the
code involved a lengthy process but was something that ABC was absolutely committed

NRF Predicts a 10 Percent Bump in Back-to-School Shopping
The National Retail Federation is
projecting that families will spend about 10 percent more on back-to-school
items this year than they did in 2009. Such an increase could push the
industry's back-to-school sales figures past $55 billion, making it the
second-biggest consumer spending period behind the holiday shopping season. The
forecast predicts that families will spend an average of $606 in 2010 on

Guided Injection of Dexamethasone versus Placebo for Treatment of Plantar
Fasciitis
Plantar
fasciitis is the most commonly reported cause of chronic pain beneath the heel.
Management of this condition commonly involves the use of corticosteroid
injection in cases where less invasive treatments have failed.
However, despite widespread use, only two randomized trials have tested the
effect of this treatment in comparison to placebo. These trials currently offer
the best available evidence by which to guide clinical practice, though both
were limited by methodological issues such as insufficient statistical power.
Therefore, the aim of this randomized trial is to compare the effect of
ultrasound-guided corticosteroid injection versus placebo for treatment of
plantar fasciitis. Full

Submits SPA for Cogenzia(TM) Phase III Clinical Trials for the Adjuvant
Treatment of Diabetic Foot Infections
Innocoll,
Inc. announced that its wholly-owned subsidiary, Innocoll Technologies Ltd.,
has submitted a Special Protocol Assessment (SPA) for a proposed phase 3
clinical trial to support the U.S. approval of Cogenzia™: Innocoll's
topical antibiotic therapy for the adjuvant treatment of infected diabetic foot
ulcers.
The
protocol was developed following an End of Phase 2 meeting with FDA where the
results of Innocoll's phase 2 clinical studies were presented and discussed. In
the planned phase 3 randomized, placebo-controlled, double-blind clinical
trial, patients with a moderate or severe diabetic foot infection will be
treated with Cogenzia on a daily basis as an adjunct to standard-of-care, which
includes systemic antibiotic therapy administered by the oral or intravenous
route. Patients will be treated for a maximum of 28 days and the primary
endpoint will be the percentage of patients who are clinically cured of
infection 10 days after discontinuation of all antibiotic therapy. The study
design is based on a highly encouraging phase 2 trial where 100% of
Cogenzia-treated patients who completed the study achieved clinical cure, which
was statistically significantly superior to 70% of patients in the control
group (P = 0.024). Patients
in the Cogenzia group also had a significantly higher rate of baseline pathogen
eradication and a reduced time to pathogen eradication (P < 0.001). Full

200,000 Canadians, or 15 per cent of people with diabetes, will develop leg or
foot ulcers in their lifetime as a consequence of their disease, with 14 to 24
per cent of them eventually requiring amputation. To reduce the prevalence of
amputations resulting from foot ulcers, the Canadian Association of Wound Care
(CAWC) is stepping up its awareness and public education campaign.
