Question: Here in rural Oklahoma one of my challenges is fitting diabetic shoes and socks for patients with severe edema and/or obesity. We want lots of volume across the instep and forefoot area, but a shoe cut low around the heel to accommodate the ankle girth. The only one I've heard of is by Apis Footwear. Does anyone have any other suggestions? Does anyone know of some seam-free diabetic socks that can accommodate lower legs with a circumference of 23" and over??

Question: I know I have seen comments on this subject in the past, but I think it is a gray area.  What are some thoughts on billing for L5000?  Some will only bill for this code if the patient has a complete transmet amputation.  Others will bill for complete transmet amputation as well as hallux amputation.  Does a hallux toe filler meet criteria for an L5000 (Partial foot, shoe insert with longitudinal arch, toe filler)?

Response: I would say I have been happy with our current education as a whole and appreciate the thorough information I have learned in the past ten years. I think I may have heard a talk about diabetic foot complications every year both pre- and post-cert. That said, there are still a lot of different topics I know nothing about and I don't believe were ever covered in my pre-cert classes years ago. For instance, I'll be honest, I had no clue what a Baxter's nerve entrapment was when Mr. Janisse posted for treatments in an earlier issue of PNW. I would be quite interested in learning about some less common complications and their solutions like this.

Comment: Well, Region C finally received the all expected prepayment reviews. I thought that we had prepared our business to deal the progress note issue based on the prepayment review in Region B. However, in the last section regarding the progress note required, Region C essentially negated our plan. In response to the new criteria, my business had to redefine our shoe program. I always thought that the best way to run a shoe program was to have patients walk in and get fitted. From a business standpoint, I have changed my view.  

 

I have not been able to deliver any shoes to my diabetic patients since the prepayment review was initiated. My patients are left wondering what's happening. The physicians are upset that they have to provide more documentation after they have already signed a prescription and a certifying statement. Now we're back asking for proof that what they signed is essentially documented by a third party. It's RIDICULOUS!   

 

It will take time to launch the new strategy and I have no choice but to add several of my employees to the ranks of the unemployed. If the Federal Government wants to re-instill public confidence in our economy, CMS has to be realistic about the "Change" that our current administration promised. Our TSD programs were originally set up by government to help prevent amputations. This change is choking the life this public confidence program.

Response: I will share my comments that I made directly to Tony.

 

Early in our endeavors as a Medicare supplier, we determined that one's attitude about supplying Medicare covered DME technology had to be one of thinking like the Pony Express.  That is, you better have a fresh pony available every 20 miles to dismount off the exhausted pony and jump onto the fresh pony for the next 20-mile journey. 

 

When it was published 7-8 years ago that the OIG was putting TSD benefits in the top 10 list of items to investigate, we determined that ramping up retail sales was the sanest approach to stay in the TSD supply business but to hedge our involvement with Medicare by growing non-Medicare revenues.   Yes, a retail customer can come back with buyer's remorse (or lower pricing found on the internet) and request a refund.   At least, we can find compensation for time invested with this type of customer by using our store policy of assessing a restocking fee and a sterilization of shoes fee if the customer does not want to exchange our shoes for another pair of our shoes.

 

A Medicare audit involves increased costs of copying files and of tracking Medicare's response.  With TSD's slim margins destroyed by chasing primary care physician charts notes that still miss one element of Medicare's requirements after multiple communications by our customer service staff, there is no longer any reason to be a Medicare TSD supplier.  Why risk a recoupment of money that had no profit margin?

 

As of June 17th, when the CIGNA email showed up that TSD claims would be subject to a pre-payment audit before payment, we stopped accepting Medicare referrals.  These referrals became "self pay" as the only option on June 18th.   Are we losing revenues?  Maybe.  But at least we know on the spot if it is money that will stay in our checking account or not.  No putting out inventory only to find a letter in our mail a month later that the claim is subject to a pre-payment audit and worse that the PCP left out one arcane element that the auditor could use to deny payment for that claim.

 

Being indignant, upset, and righteous does not pay the overhead and employees' payroll.  Let it go, Tony.  Move on.  It is a frightening, new world of politicians and bureaucrats and insurance executives controlling our health.  But that is really not new.  It is just new to those who have not seen it coming for many years and put all their eggs in the TSD basket.

 

The other frightening thought for ABC and PFA is this: Why incur the costs of being a Certified Pedorthist if most of your competitors in your market are not certified but giving out business cards that say "Pedorthist?"   There is zip branding of Certified Pedorthist versus pedorthist in our community and I imagine many other communities across America.  At the one-on-one level of customer service, it is your knowledge, not your title, that keeps the customer coming back and telling their friends, neighbors, and co-workers to come to you.  

 

So ABC and PFA may reap what they have not sown if one does not need to be a    C. Ped. "qualified practitioner" to make a living after dropping out of Medicare.

What are PNW readers doing to capitalize on this "revolution?"

Question: We have had several requests as of late for pedorthic related products that will run on an I-Phone/Pod/Pad or Blackberry. These are generally Pre-cert or CE in nature. We have several CE apps that will run in that environment but are always looking for ideas to develop more. My first question is how many practitioners out there are utilizing current technology (iPhone/Pad/Pod, Blackberry, etc) in their work environment, and secondly, who would if they only knew how to better utilize this technology?

Question: No, really? Just received the latest PFA Magazine. There is an advertisement for High Heel orthotics on page 17. Is this for real? I cannot see how any orthotic would do any good in any high heel (especially in those "exquisite spikes" shown). How can any orthotic give any support in a high heel shoe. Didn't "Oprah" have something like this on one of her shows? This brings me to the following question: should there be advertising for very questionable products in a health related magazine? As a C. Ped. I see myself as a footwear specialist who works toward long-term maintenance of healthy feet and the prevention not the promotion of foot damage. What is your take on this?

A Classification of Diabetic Foot Infections Using ICD-9 Codes: Application to a Large Computerized Medical Database


Diabetic foot infections are common, serious, and varied. Diagnostic and treatment strategies are correspondingly diverse.

It is unclear how patients are managed in actual practice and how outcomes might be improved. Clarification will require study of large numbers of patients, such as are available in medical databases.

We have developed and evaluated a system for identifying and classifying diabetic foot infections that can be used for this purpose.

Methods: We used the (VA) Diabetes Epidemiology Cohorts (DEpiC) database to conduct a retrospective observational study of patients with diabetic foot infections. DEpiC contains computerized VA and Medicare patient-level data for patients with diabetes since 1998. Full text

Diabetes and Summertime Foot Care

One of the most difficult and common effects of diabetes is foot problems. There were many questions related to diabetic foot care. We may say, what's more natural than bare feet and sandals in the summer? Unfortunately, there is no off-season for diabetes - constant monitoring is crucial. I urge diabetes patients to be aware that prolonged hot and humid weather can lead to a host of foot woes - among them third-degree burns, if they don't protect their feet properly.

The concern here is that in extreme heat - as experienced during summer months in the Bahamas - diabetics can suffer swelling, dryness and cracking from wearing sandals and problems associated with walking barefoot outdoors such as puncture wounds, burns and blisters from hot pavement. Full Text

Overuse Injuries Plague Youngsters

In the June 7 edition of Sports Illustrated, one of the country's best known and respected sports orthopedic surgeons is quoted as saying, "We're seeing kids hurt before they even have a chance to become athletes".

The article by Mark Hyman, author of "Until it hurts: America's obsession with youth sports and how it harms our kids," stresses the serious rise of youth sports injuries -- many that require surgery.

Dr. James Andrews, who has operated on some of the most famous professional athletes in all sports for decades says, "I don't think epidemic is too strong a word."

He says that over 60 percent of the athletes he does surgery on are high school age or younger. He has helped start an organization called STOP (sports trauma and overuse prevention). Athletes like Bo Jackson, Charles Barkley, Jack Nicklaus, Tom Brady and others have joined Dr. Andrews as spokespersons to help get the word out. Full Text

Pedorthic Pre-Certification Training Course July 26th - August 8th, 2010

In cooperation with the ABC and BOC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS.
