Question: An article about "toning shoes" recently appeared in USA Today (note from editor: see "A Revolutionary Sneaker, or Overhyped Gimmick?" in the Sports and Fitness News Section below)that has a potential impact on pedorthists as providers of toning shoes by "experts who can advise and train customers on their proper use and application." What are PNW readers doing to capitalize on this "revolution?"

Comment: Well, Region C finally received the all expected prepayment reviews. I thought that we had prepared our business to deal the progress note issue based on the prepayment review in Region B. However, in the last section regarding the progress note required, Region C essentially negated our plan. In response to the new criteria, my business had to redefine our shoe program. I always thought that the best way to run a shoe program was to have patients walk in and get fitted. From a business standpoint, I have changed my view.  

 

I have not been able to deliver any shoes to my diabetic patients since the prepayment review was initiated. My patients are left wondering what's happening. The physicians are upset that they have to provide more documentation after they have already signed a prescription and a certifying statement. Now we're back asking for proof that what they signed is essentially documented by a third party. It's RIDICULOUS!   

 

It will take time to launch the new strategy and I have no choice but to add several of my employees to the ranks of the unemployed. If the Federal Government wants to re-instill public confidence in our economy, CMS has to be realistic about the "Change" that our current administration promised. Our TSD programs were originally set up by government to help prevent amputations. This change is choking the life this public confidence program.

Comment: To all Texas C. Peds: We need to discuss licensure in the State of Texas. The State Legislature will meet in January of 2011. It's imperative that we meet as soon as possible. Scott and White Hospital is offering a Symposium that is Podiatrist based, but they will have a pedorthic track. They are allowing us to meet after our classes to discuss licensure. 

 

We will also need to elect officers. A nominating committee has been formed. Please contact Allan Johnson or Ladd Fowler if you are interested in serving on the Board of Directors.

PREVIOUS QUESTIONS AND COMMENTS WITH RESPONSES




Question: I have a patient with bi-lateral TMA. He requested a PWM "Hercules" boot because of his farm work. I am thinking I would add carbon fiber plates inside the boot under the toe-filler for added stiffness. Would you say the carbon plate is an acceptable option instead of adding a full-length steel shank?

Response: In response to the TMJ foot and putting a carbon fiber plate inside under the toe filler, unless it is attached to the shoe, any movement inside the shoe could create irritation to the foot.  Five years ago I had a single TMJ come in where the doctors were talking about doing a below the knee amputation due to the distal end of the foot breaking down.  The patient wore a size 11 women's on the right foot; on the TMJ foot they were putting a steel plate and rocker on the size 11 mate.  Because the rocker was in front of the amputated part of the foot, pressure inside the shoe at push off was breaking the skin open.  Since her TMJ foot was nearly a size 4 women's, I placed her in a size 5 with enough room to accommodate the FO with a forefoot filler.  I then put a rocker on the shoe placing the apex of the rocker just posterior to the TMJ.  Caped the shoe with hearingbone, which added to the stiffness of the shoe.  I found that a steel or carbon fiber plate was not needed.  As a result of this procedure, the patients wound healed in a few weeks and five years later she still has her leg.

PREVIOUS QUESTIONS WITHOUT RESPONSES




Question: We have had several requests as of late for pedorthic related products that will run on an I-Phone/Pod/Pad or Blackberry. These are generally Pre-cert or CE in nature. We have several CE apps that will run in that environment but are always looking for ideas to develop more. My first question is how many practitioners out there are utilizing current technology (iPhone/Pad/Pod, Blackberry, etc) in their work environment, and secondly, who would if they only knew how to better utilize this technology?

Question: No, really? Just received the latest PFA Magazine. There is an advertisement for High Heel orthotics on page 17. Is this for real? I cannot see how any orthotic would do any good in any high heel (especially in those "exquisite spikes" shown). How can any orthotic give any support in a high heel shoe. Didn't "Oprah" have something like this on one of her shows? This brings me to the following question: should there be advertising for very questionable products in a health related magazine? As a CPed I see myself as a footwear specialist who works toward long-term maintenance of healthy feet and the prevention not the promotion of foot damage. What is your take on this?

Is That Right? Toning Flip-flops Really Shape Your Legs?

 

Flip-flops that promise to tighten your tush, chisel your calves and carve out your cankles -- really?

 

Well, if I can get a workout by simply walking around in my favorite summertime footwear, then I'm sold. But it doesn't seem to be quite that easy. Though toning sandals -- and their predecessors, those bulbous toning sneakers -- have been around for a few years, I focused on them recently when a Fitflops poster caught my eye in a Red Line Metro station. "Get a workout while you walk," it promised.

Consumer Confidential: Lots o' Shoes

 

The average woman spends $25,000 over the course of a lifetime on shoes. (Note to wife: Not that there's anything wrong with that.) This startling factoid comes courtesy of a British insurance company, GoCompare.com, which found that aside from that lifetime shoe bill, the average woman has 19 pairs of shoes -- three with heels, six pairs of flip-flops, flats, sandals or wedges, three pairs of boots and four pairs of "foxy style." The Iannones: A Regina Tradition for 50 Years


The footwear business has definitely been a perfect fit for Regina's Iannone family. For half a century, the Iannones have owned and operated a quaint little shoe store in the Cathedral area. Much has changed over the past 50 years. Shoe styles, for instance. But much has remained constant. The Effectiveness of Educational Practice in Diabetic Foot: A View from Brazil

 

Background and aim: The aim of the present study was to evaluate the prevention and self-inspection behavior of diabetic subjects with foot at ulcer risk, no previous episode, who participated in the routine visits and standardized education provided by the service and who received prescribed footwear. This evaluation was carried out using a questionnaire scoring from 0-10 (high scores reflect worse practice compliance).

Results: 60 patients were studied (30 of each sex); mean age was 62 years, mean duration of the disease was 17 years. As for compliance, 90% showed a total score [less than or equal to] 5, only 8.7% regularly wore the footwear supplied; self foot inspection 65%, 28,3% with additional familiar inspection; creaming 77%; proper washing and drying 88%; proper cutting of toe nails 83%; no cuticle cutting 83%; routine shoe inspection 77%; no use of pumice stones or similar abrasive 70%; no barefoot walking 95%. AHCCCS About to Cut Foot Care, Putting Poor Diabetics at Risk

 

Arizona's poorest residents are about to lose coverage for foot care, which will put diabetics in greater danger of losing limbs, some health providers say. As part of a statewide budget reduction, the Arizona Health Care Cost Containment System (AHCCCS) come Oct. 1 is slashing several health benefits for enrollees over 21.

 

Among benefits about to end: most dental care, insulin pumps, cochlear implants, certain transplants and all services provided by a podiatrist. The benefit cuts represent an annual savings of just over $6 million out of the state budget. The podiatric services alone will save $1 million annually, AHCCCS spokeswoman Monica Coury said.

 

About 1.4 million Arizonans are enrolled in AHCCCS, which is Arizona's form of Medicaid and is reserved for people who are extremely low-income or indigent. The income cap is generally 100 percent or less of the federal poverty level - an annual income of $22,050 for a family of four. Full Text

Pedorthic Pre-Certification Training Course July 26th - August 8th, 2010

In cooperation with the ABC and BOC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute.
