Archaeologists Discover 5,500-Year-Old Shoe

(AP) About 5,500 years ago someone in the mountains of Armenia put his best foot forward in what is now the oldest leather shoe ever found.

It will never be confused with a modern shoe, but the well-preserved footwear was made of a single piece of leather, laced up the front and back, researchers reported Wednesday in PLoS One, a journal of The Public Library of Science.

Worn and shaped by the wearer's right foot, the shoe was found in a cave along with other evidence of human occupation. The shoe had been stuffed with grass, which dated to the same time as the leather of the shoe - between 5,637 and 5,387 years ago. Full Text.

Question: With summer sandal season here, we are modifying our sandals that do not have a removable footbeds to have a higher arch. We have been using white felt scaphoid pads. Is there any source for felt scaphoid pads that are not white (black, brown) or is there an easy way to dye felt to other colors? Or is there an alternative way to increase arch support in sandals without removable foot beds that does not use felt?

Question: I no longer take assignment on diabetic shoes or inserts. If someone purchases from me and asks me to send in to Medicare non-assigned so he can get reimbursed, who is responsible to get all the paperwork and save it in a chart? Should I just send in claim without GY modifier and see what Medicare does? (I am sure they will kick it back out.) Should I give him the therapeutic shoe form to be filled out by a certifying doctor and also the RX and let him do the work? If Medicare isn't paying me but rather the patient, then I assume they can't come after me for a refund. Or will they simply not do an audit on a non-assign claim?

Response: To try to respond to your question about whether or not to submit a claim or let the patient handle paperwork, I spoke with our billing staff. You had a lot of different issues that they found challenging to sort out. They indicated that as a non-participating Medicare supplier your company would still have to submit the claim for the beneficiary. The GY modifier should be used when the supplier believes the benefit is statutorily excluded, i.e. individual does not have diabetes, or the beneficiary's medical condition would not meet Medicare requirements for coverage. Asking the individual to take the Certifying Statement and Rx to their physician versus using a fax or mail is not relevant to whether a supplier has to file a claim. You did not reference whether or not you u were going to have the beneficiary sign an ABN informing that individual that you believed the items would not be covered. My billing staff thought that this would be appropriate for this claim if you were planning on using the GY modifier.

Relative to Medicare not coming after the supplier on this claim to recoup a payment made to the beneficiary (Medicare would not reimburse with the GY modifier), that question really hits to the heart of supplying TSD benefits under the current climate of non-valid PCP chart note audits, program integrity claim auditors being paid a percentage of what is recouped, physician signature legibility hurdles, PECOS, and who knows what next week. Response: Scott! Read the rules. Whether it is assigned or not assigned, if you don't know this you should not be filing at all until you read the rules and regulations for providers and understand it. You are still the provider and are responsible for all paperwork being in your file. You are responsible to make sure the doctor is on the correct list so they qualify to sign the forms, and on and on and on it goes. You can also be forced to reimburse the patient unless you have them sign a form stating they may not be paid or be fined thousands of dollars. The only way not to be responsible is to opt out as a provider at all. Medicare agents will tell you to let the patient send it in, once you opt out, but they still will not pay it even when the agents said they would.

I stopped filing Medicare at all in September 2009. I never took assignment unless it was a Medicare/Medicaid patient and it was still too much work for too little reimbursement. Even when it was to help the patient get money back, If it is refused you have to do all the work over again until the claim is paid and you cannot charge the patient for the time spent, which could be hours and hours of phone calls and paperwork depending upon Medicare. So as a provider you continue to loose profit margin. The nightmare on old claims still hasn't ended 9 months later.

Unfortunately I do not see much future for the pedorthic Industry. ABC has done us no favors. Many of the people I talked with at the PFA Symposium last fall are struggling with Medicare and the Industry as well. Many people have stopped Medicare including many doctors. Where is Healthcare going for the Provider? My opinion: not a very good direction.

Question: I am interested in knowing how to determine the value of my business. I am a C. Ped., CMfts. The business is in a small town in North Georgia. I service the Tri-state area-Georgia, Tennessee, and North Carolina. I am interested in working 1-2 days per week, until the new owner would be up to speed. Please give me any advice you can. Response: Sad to say there is no real value to your business beyond the value of the equipment and the merchandise that you have on hand less the bills that you owe. "Good Will," i.e. the name and location of your business, is really not a transferable asset (unless you also own the building). Clients and referring docs are used to dealing with YOU. A client who is used to shopping with YOU is just as likely to work with the new owner as to walk away and find someone else. The only other thing that you might be able to sell is a client list (like a dentist's office). There are many other factors also. For example, how long is your lease on the location? Will the owner of the building allow you to transfer the guarantee of rent to the new owner? I have someone in my area trying to sell a small pedorthic practice. He started out asking $85,000.00 he is now down to $65,000, and he has shoe repair equipment in his shop. A possible more profitable way to sell the business is to figure out the net assets and then include an advisory/training fee, plus a small stipend for the days that you work that involve working with clients.

Heels a Threat for Pregnant Women

Pregnant women who want to emulate celebrities by wearing high heels are putting their feet at risk, an expert warned. Lorraine Jones, from The Society of Chiropodists and Podiatrists, said women feel under pressure to wear stilettos but are risking long-term damage, as well as strains to their ankles and ligaments.

Furthermore, pregnant women routinely wear other unsuitable footwear, such as flip flops, ballet pumps and Ugg boots, she said.

A poll of 1,000 pregnant women for the Society found 66% regularly wear flip flops, 32% wear high heels, 53% ballet pumps and 30% Ugg boots. Full Text

It is quite a challenge to keep your feet cool when walking around in extremely hot weather. Therefore, your feet need some special care and attention during the summer months. Heat and humidity will aid in the growth of bacteria, so taking measures to reduce these will help enormously.

During the summer, foot injuries are also more prevalent - the increased level of outdoor athletic activities also increase the chances of muscle pain, heel pain, shin splints, plantar fasciitis, Morton's neuroma, achilles tendinitis, hip pain/bursitis, runner's knee, illiotibial band syndrome(ITB) and the list goes on.......

Molecular Imaging Helped Diagnose Diabetic Foot Infection

Using multiple imaging agents with a hybrid imaging technique may be a valuable tool to accurately diagnose and determine treatment for a variety of diabetes-associated foot diseases, according to a study presented yesterday at the Society of Nuclear Medicine's 57th Annual Meeting.

For the 3-year study, researchers used dual tracer single photon emission tomography and computed tomography (SPECT/CT) to identify foot disease in 23 patients with diabetes (16 men; mean age, 57 years). Full Text

Diabetes Linked to Brain Disorders

Malfunctions in how the body processes sugar that occur in diabetes and obesity could also explain mood and other mental disorders such as schizophrenia, researchers now reveal. People with diabetes have problems processing insulin, the hormone that helps regulate sugar in the body. Scientists knew diabetics had an increased risk of psychiatric disorders, said researcher Kevin Niswender, an endocrinologist at Vanderbilt University Medical Center in Nashville, Tenn.

"In the diabetic population, 25 percent are depressed - in the normal population, it's only 10 percent," researcher Aurelio Galli, a biophysicist at Vanderbilt, told LiveScience. Full Text

A Good Run for Sneakers in May

May continued the good run in athletic footwear we've seen this year. Sales for the month grew about 10% in dollars, and in the mid singles in units, yielding mid single digit increase in average selling price. Without Toning, sales would have grown in the low singles. Toning was 8.5% of the total Sport Footwear business for May. Toning is on track to exceed $1.5 billion in 2010.

Business has been so good and inventories so lean, it appears there is little clearance. May traditionally is a major clearance month so the lack of markdowns likely held the results back from being even better. Full Text

Pedorthic Pre-Certification Training Course July 26th - August 8th, 2010

In cooperation with the ABC and BOC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS.
