Question: I no longer take assignment on diabetic shoes or inserts.  If someone purchases from me and asks me to send in to Medicare non-assigned so he can get reimbursed, who is responsible to get all the paperwork and save it in a chart?  Should I just send in claim without GY modifier and see what Medicare does?  (I am sure they will kick it back out.) Should I give him the therapeutic shoe form to be filled out by a certifying doctor and also the RX and let him do the work?  If Medicare isn't paying me but rather the patient, then I assume they can't come after me for a refund.  Or will they simply not do an audit on a non-assign claim?

Question: As C. Peds., we are required to maintain a level of continuing education in order to remain credentialed. Yet, over the past few months of reading Pedorthic Newswire, the only "courses" I have seen listed are for Pre-Certification and Shoe Fitters, neither of which is appropriate for someone already certified. Yes, we can pick up CEU credits by attending the PFA Symposium. But what if, for whatever reason, attendance is impossible? I'm going to the Orlando event and I will pick up the points I need to remain credentialed there. But it just seems to me that everyone is jumping of the Pre-Cert/Fitter bandwagon and no one is offering any hard core pedorthic post-certification instruction on how to treat pathologies that are more involved than plantar fasciitis, pre-ulcerative callus, or interdigital neuromas. Am I wrong?

Response: For continuing education, you should check out www.learnpedorthics.com.  The last time I checked there were 18 DVD educational programs that deliver a reasonable cost per CEU hour without the hassle of airport TSA security checks, without overpriced resort room rates, and without exorbitant meal costs at a resort.  In an economy that has dinged a lot of small business owners, these DVDs are an excellent way to get needed CEU hours.  Also, I used one DVD program from NSRA to get a new hire C. Ped. employee the hours at the last minute.  It worked to keep his credentials in place.  While there is a great deal of benefit from PFA seminars for networking and brainstorming, using DVD technology and online testing can help keep costs down on a portion on our CE requirements.  PFA has offered online seminars, but the timing for anyone who works west of the Eastern Standard Time zone means schedule conflicts occurs on every online seminar.  Until PFA offers their online seminars after work hours for pedorthists in the Mountain and Pacific Time zones, these online options are less viable -- which is a shame, because some of the topics have been very appealing.

PFA is currently working with many other educators and educational facilities to add even more continuing educational programs in the coming months and years. Response: I taught a couple of sessions at the PFA symposium a few years back. The topic was "Foot Examination and Assessment Techniques." It covered the basics through some intermediate skills and techniques. To this day, I still get people commenting how useful it was, and continues to be, to their daily practice. Our company (CFS Allied Health Education) provides CE distance learning tools for O & P /Pedorthic practitioners. Our foundational video products are the most popular by far. We have recently submitted several new products including Basic Kinesiology and Basic Pharmacology, both referenced as required knowledge and skills by the certification agencies, but not covered/required when/where I went to school. We would love to develop more, but need your input. I have asked for topics for years, but have received little response. Response: I have several thoughts in response to your query.  The most obvious is that 4 of the 6 Pedorthic Newswire (PNW) Advisory Board members are associated with schools that offer mainly pre-cert & shoe fitter courses.  Aetrex, which owns PNW, has generously allowed educators to post their courses on this site, while organizations like PFA, ABC & BOC generally list their courses on their individual sites.  The PFA site under Continuing Education Webinars offers numerous webinars during the year to help your pocketbook by not having to travel to a live course.  Also on the site, www.pedorthics.org click "Links;" it will give you several organizations offering courses.  One site not listed there is www.learnoandp.com, which offers a variety of Web-based distance learning opportunities.

 

To answer the part about what if you can't attend this year's PFA symposium - both ABC & BOC require the points be gained within a 5-year time frame.  If you do a little something each year, don't wait until the last minute, and make the PFA symposium every other year, you're covered.  Or you can follow my example of attending every PFA symposium for the past 27 years in a row and getting the full 5-year point requirement every year. (It's OK to gag now, I won't be offended).

Response: Richard - It can be challenging to find relevant courses and opportunities for continuing education in any profession.  The American Board for Certification in Orthotics, Prosthetics and Pedorthics, Inc. Peds.  Simply go to the ABC website and you'll find a detailed list of both live courses and distance learning opportunities by discipline.  In June alone there are 29 live courses or conferences available for C. More Structured Flip-flops Are Easier on your Legs

Just in time for summer, there's news on the flip-flop front. If you want to make it easier on your legs and feet, look for more structured flip-flops that hold your foot in place, possibly ones with heel cups and arches, says Justin Shroyer, an assistant professor of kinesiology at the University of Louisiana-Lafayette.

 

"The less flip in your flop, the better it is for your feet and legs," Shroyer says.

He has studied more than 100 people wearing flip-flops, looking at how the shoes affect legs and feet. He presented some of his research Wednesday at the American College of Sports Medicine's annual meeting in Baltimore. Full Text

Expressions Shoe Center Is Not Your Typical Shoe Store

Dean and Joi Brandt have 85 years of retail experience between them, so it's no wonder their Pequot Lakes store is not your typical shoe store. Expressions Shoe Center on Main Street now offers the Aetrex System, a computerized system that helps people with foot problems. Only about 10 stores in the state have the system, and only a handful of those have the computer display.

Dialysis Linked to Foot Ulceration in Diabetic Patients

Dialysis treatment is independently associated with foot ulceration in patients with diabetes and stage IV or V chronic kidney disease (CKD), according to the results of a cross-sectional study reported online May 18 in Diabetes Care.

"Foot ulceration is a serious problem for people with diabetes which additionally results in huge economic costs," write Agbor Ndip, MD, from Central Manchester University Hospitals NHS Foundation Trust in Manchester, United Kingdom, and colleagues. "Causal pathways to foot ulceration are multifactorial and involve combinations of physiologic and mechanical factors, self-care and treatment factors. Diabetic nephropathy has been identified to be an important risk factor for foot ulceration and amputation." Full Text

Tips for Determining if a Foot Wound Is a Diabetic Ulcer or a Pressure Ulcer

Question: Do you have any tips for determining if a foot wound is a diabetic (neuropathic) ulcer or a pressure ulcer?

 

Both diabetic ulcers and pressure ulcers can occur on the feet. The diabetic ulcer typically occurs over the plantar (bottom) surface of the foot on load-bearing areas such as the ball of the foot (the metatarsal heads) or under the heel. But diabetic ulcers also can occur on the top or between the toes.

 

Many patients with diabetic foot wounds also will have foot deformities, including hammertoes, Charcot foot and bunions. Characteristics of the diabetic wound include: even wound margins, a rounded or oblong shape, deep wound bed with necrotic tissue, moderate amounts of drainage, surrounding callus, and often painlessness due to decreased sensation in the foot (peripheral neuropathy). Pedorthic Pre-Certification Training Course July 26th - August 8th, 2010

In cooperation with the ABC and BOC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute. The course includes the required tests for competency to sit for the ABC or BOC exams.

 

Upon completing the course and successfully passing the competency tests, and fulfilling the 1,000 hour "Clinical practice Requirement" students are eligible to sit for the ABC or BOC exam to earn their Pedorthic Certification (CPED or BOC Pedorthist).
