NEW QUESTIONS AND COMMENTS

Question: As C. Peds., we are required to maintain a level of continuing education in order to remain credentialed. Yet, over the past few months of reading Pedorthic Newswire, the only "courses" I have seen listed are for Pre-Certification and Shoe Fitters, neither of which is appropriate for someone already certified. Yes, we can pick up CEU credits by attending the PFA Symposium. But what if, for whatever reason, attendance is impossible? I'm going to the Orlando event and I will pick up the points I need to remain credentialed there. But it just seems to me that everyone is jumping of the Pre-Cert/Fitter bandwagon and no one is offering any hard core pedorthic post-certification instruction on how to treat pathologies that are more involved than plantar fasciitis, pre-ulcerative callus, or interdigital neuromas. Am I wrong?

Question: I am interested in knowing how to determine the value of my business. I am a C.Ped, CMfts. The business is in a small town in North Georgia. I service the Tri-state area-Georgia, Tennessee, and North Carolina. I am interested in working 1-2 days per week, until the new owner would be up to speed. Please give me any advice you can. PREVIOUS QUESTIONS AND COMMENTS WITHOUT RESPONSES

Question: Has anyone else had this happen yet?

We received four of the Prepayment Reviews from Medicare, we called the doctors and received the notes for the patients and sent them all back for review. Three went through fine and we received payment. The fourth is a lady from the Air Force Base Hospital in town; this one did not get approval because the base physicians do not follow Medicare guidelines and do not test their patients for neuropathy yearly, and there was no documentation for poor circulation or foot deformity that they circled on the CPF. Now we have to bill the patient, and this is a regular yearly customer, for the shoes and custom foot orthoses -- which means we have lost a patient because they won't be back next year, and I don't blame them. So, is my only option here to not accept patients from the base hospital just in case Medicare decides to do this again? Many of my patients are from this military hospital.

It sure is tough making a living in this profession. We did a cost analysis for custom molded diabetic foot orthoses made in our lab. Cost is about $70.00 a pair for materials, salaries, and time. This does not include fitting, adjustments and follow-up appointments. Aetna pays $32 a pair, the same amount they pay for A5512's. They don't cover our costs on shoes either. Who can afford to lose money for each Aetna patient? We can't and won't.

Is it worth it? I wonder.

Question: With the recently announced meeting between NCOPE, ABC, PFA, BOC and the Pedorthic Educators, it seems that "formal" pedorthic education (which we all know translates into "associate's degree") is finally looming on the very near horizon. As one of the 30 people invited to this function, I heard from many of the organizational heads of state that they thought this was the way to go. So, what do you think? Is this a good idea? If so, are you willing to upgrade your skills to match (Anatomy/Physiology/Biology/Physics)? What is this going to do to the already slim supply of pedorthists in conjunction with all of the new patients entering the system through the new Healthcare setup? Can you afford to send your employee to school in another State for 2 years, and help pay their student loans off over the next 10 years? Good questions, and this is a great forum to let the organizations know what the men and women on the street think.

Crocs to Focus on Medical Footwear; Ties up with Surya Pharma

US-based premium footwear brand Crocs Inc today said it is targeting the country's market for its specialized range of medical footwear and aims to sell at least 1.5 lakh pairs within a year.

The company, which entered the Indian market in 2007, also said it has appointed Indian drug maker Surya Pharmaceuticals as its sole marketing partner in India for its 'Rx' range of medical footwear.

"We launched our 'Rx' line in India last year through regular retail channels. However, we found the normal route inadequate for specialized footwear so we are now aligning with Surya, which has its own retail pharma chain that we seek to explore," Crocs India Managing Director Murli Desingh said. Bare Facts

I'm writing in response to the May 17 Datebook article "Put Bare Foot Forward" by Katherine Seligman. I am a certified pedorthist in San Francisco and for 35 years have been addressing the complaints of people with foot problems, from runners and weekend warriors to people suffering from the serious effects of disease, trauma, birth defects and aging.

As a specialist in biomechanics and a proponent of orthotic devices and specialized footwear, I was initially defensive regarding the ideas proposed in the book referred to in the article, "Born to Run" by Christopher McDougall. Running barefoot went against everything I had learned and heretofore experienced in my practice with clients. Full Text

NRF Supports Renewal of Deprecation Rule to Help Save Retail Jobs

The National Retail Federation (NRF) on May 26, 2010 asked the U.S House of Representatives to extend a tax rule that allows retailers to depreciate remodeling and other improvements to their stores over a 15-year period, rather than the previous 39 years. The NRF, which is a Washington, D.C.-based trade association for retailers, claims this move would help save "critically needed" retail jobs.

"If the depreciation provision is not extended, the cost of making needed improvements to a retail store or restaurant must be written off over nearly four decades," said Steve Pfister, NRF's senior vice president for government relations. "This would have a significant negative impact on a retailer's decision to make improvements and, in this economic climate, could result in more store closures, costing retail industry jobs." Donated Footwear Lined Grand Rapids Bridge

Shoes, boots, sneakers, and sandals were donated during the past few weeks to 95.7 WLHT's Shoes Across the Grand drive to benefit In The Image, a non-profit provider of gently used clothing, furniture, and appliances to the needy located at 1823 S. Division Ave. SE.

The bridge was closed for six hours as donations were spread along the center of the road. Many pedestrians crossing the bridge were surprised by the sight and a few bent over looking to select a certain style and fit. Full Text

Runner Therapy: Reader Q&A, Big Pain in Big Toe

Question: Recently during work when I get up from my desk I have the strangest thing happen, a stabbing pain in my one big toe. No place else, just a stabbing pain under my one big toe. Is this associated with plantar fasciitis, as I have been prone to this?

Pedorthic Pre-Certification Training Course July 26th - August 8th, 2010

In cooperation with the ABC and BOC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute. The course includes the required tests for competency to sit for the ABC or BOC exams.
