Comment: Healthcare reform was passed amid much controversy. Whether you are Republican or Democrat or Tea-Party, does not matter. Healthcare reform is here. What we know is that more than 32 million Americans will be insured. So how is the system going to insure previously uninsured Americans while still holding down the costs of healthcare? The answer is prevention. Healthcare Reform is set to waive the annual deductibles and co-pays for preventative healthcare. As an industry, the therapeutic shoes for diabetics, TSD  has always been discussed as preventative. But what is TSD preventing?

Our shoe programs are really NOT about shoes. Our programs are about identifying high risk patients and attempting to prevent foot ulcers. A diabetic foot ulcer is the leading cause of a lower limb amputation. Lower limb amputations are expensive to our healthcare system. In addition, statistic indicate that within 3 years of an amputation half of the amputees will die. We know that dying in the United States is expensive. Last year Medicare paid hospitals and doctors $50 billion dollars during patients' last two month of life.
 
Medicare is well aware of this statistic. In fact, Medicare has established a program via the Physicians Quality Reporting Initiative, PQRI  that allows for therapeutic shoe programs to perform foot ulcer prevention foot exams and to measure diabetics feet for proper shoe sizing. Medicare's rationale for this, shoe trauma leads to foot injury that leads to ulceration that leads to amputation.  CMS is also aware that physicians do not make time to exam diabetic feet. These examinations are part of what TSD is designed to do. 

So, why would Medicare, CMS, the OIG, LCD and all the other governing bodies want to harm the one industry that was created to help prevent the costly amputation? Why are we being required to have physician progress notes. The medical industry is well aware that physicians are notorious for their lack of documentation. How many physicians have ever had their hospital credentials placed on hold until they signed off on their inpatient charts?

We need to defend our diabetic shoe industry as we part of the solution in the a war for prevention.  But who should take the lead in defending the pedorthic industry?

Comment: Medicare requested information on 13 of our patients.  Medicare then reviewed the information and denied 12 of the claims.  When I contacted them they stated the M.D. or D.O. did not have documentation in the medical records we submitted that substantiated the TSB. 

 

Medicare instructed us to get all medical records prior to delivering any items. Medicare will not allow us to create a form that would include all the information they are expecting the doctors to document.  We have lost a fourth of our business and our customers are outraged.  Physicians are sending documentation that does not meet Medicare's policy and we are upsetting Physician's by requesting more info.  We can't upset Physicians or they will stop sending their patients our way.

 

Medicare requires that we have medical records from the M.D. or D.O. that treats the patients' diabetes.  I spoke with POE and Medicare and cannot get an explanation of what they classify a comprehensive plan.  Per Medicare the medical records supplied must include:

 

1) The diagnosis marked on the TSB

2) A comprehensive plan of treatment

3) The need for shoes/inserts/modifications and the quantity

 

Medicare states we cannot use notes from another physician.  Most M.D.'s and D.O.'s send their patients to a Podiatrist to treat their feet.  We have sent all the documentation suggested by Medicare to the physician treating the patient's diabetes.  There are other providers in our area who our patients have told us are not requiring this information.  If Medicare is going to enforce this, then everyone dispensing items from the TSB should be required to follow these rules.  Does anyone out there have any suggestions?

Response: In regards to the new "documentation standards," why did we just go through facility accreditation and bonding? What's it good for? After 16 years of proud service to the community I am thinking seriously of throwing in the towel.  At some point it's just not worth it. Sad day.

Response: This is the deal, in my opinion. Diabetic patients SHOULD be under a comprehensive plan of care. There are so many vital organs that are affected by diabetes. The "captain of the ship" the PCP SHOULD have a comprehensive plan of care for his diabetic patient. However, we all know that PCPs are under severe time constraints. CMS knows this as well. In fact, CMS has a provision under PQRI that allows "non-MDs" to perform basic foot ulcer prevention examinations. CPeds fall under the scope of practice that allow for these foot ulcer prevention screenings. What has to happen is we must get that information into the PCPs' hands. They must have possession of our assessment, which should include the multi-disciplines involved in the patients' care. I have a comprehensive plan of care that may be of assistance to you.

Question: Has anyone else had this happen yet?

 

We received four of the Prepayment Reviews from Medicare, we called the doctors and received the notes for the patients and sent them all back for review. Three went through fine and we received payment. The fourth is a lady from the Air Force Base Hospital in town; this one did not get approval because the base physicians do not follow Medicare guidelines and do not test their patients for neuropathy yearly, and there was no documentation for poor circulation or foot deformity that they circled on the CPF. Now we have to bill the patient, and this is a regular yearly customer, for the shoes and custom foot orthoses -- which means we have lost a patient because they won't be back next year, and I don't blame them. So, is my only option here to not accept patients from the base hospital just in case Medicare decides to do this again? Many of my patients are from this military hospital.  

 

It sure is tough making a living in this profession. We did a cost analysis for custom molded diabetic foot orthoses made in our lab. Cost is about $70.00 a pair for materials, salaries, and time. This does not include fitting, adjustments and follow-up appointments. Aetna pays $32 a pair, the same amount they pay for A5512's. They don't cover our costs on shoes either. Who can afford to lose money for each Aetna patient? We can't and won't.

 

Is it worth it? I wonder.

Question: With the recently announced meeting between NCOPE, ABC, PFA, BOC and the Pedorthic Educators, it seems that "formal" pedorthic education (which we all know translates into "associates degree") is finally looming on the very near horizon. As one of the 30 people invited to this function, I heard from many of the organizational heads of state that they thought this was the way to go.  So, what do you think? Is this a good idea? If so, are you willing to upgrade your skills to match (Anatomy/Physiology/Biology/Physics)? What is this going to do to the already slim supply of pedorthists in conjunction with all of the new patients entering the system through the new Healthcare setup? Can you afford to send your employee to school in another State for 2 years, and help pay their student loans off over the next 10 years? Good questions, and this is a great forum to let the organizations know what the men and women on the street think.

OPERF Announces 2010 Educator and Student Award Programs

The Orthotic and Prosthetic Education and Research Foundation (OPERF), Inc. is pleased to announce two new award programs; one for educators and one for students.

The O&P Educator Award Program is designed to support prosthetic and orthotic educators who currently hold teaching positions at Commission on Accreditation of Allied Health Education Programs (CAAHEP)-accredited O&P education institutions. It is meant for faculty who are pursuing higher degrees to improve their skills as O&P instructors. The Educator Award is a $2,500 cash grant. This year up to three awards are available.

The OPERF Student Award Program is designed to support students currently enrolled in a CAAHEP-accredited O&P program and who have completed at least one semester toward their degree program. The award is a $200 cash grant and this year, up to ten Student Awards are available.

Instructions and forms for submission of applications are available online at www.OPERF.org. Educator Award Program Applications are due June 30, 2010 and awards will be made on or about October 1, 2010. Student Award Program Applications are due by October 1, 2010 and awards will be made on or about December 15, 2010.

Red Wing Shoe Gets $261K Grant to Expand, add 42 Jobs

 

Red Wing Shoe Co. has received a $261,270 grant to expand its Potosi, MO plant and add 42 new full-time jobs within two years. The Industrial Infrastructure grant, made through the Community Development Block Grant program, will help spur at least $736,600 in private investment, according to Gov. Jay Nixon's office.

 

Nixon visited the plant Friday to announce the grant, which will go toward increasing production, adding card-reader access, installing new truck driver entrances, putting on a new roof, re-paving parking lots and upgrading lighting systems. Full Text

Crocs: Off the Endangered List

Can Crocsbite back? The footwear purveyor posted heartening first-quarter results, but the long-term sustainability of its growth remains in question.

 

Crocs swung to a first-quarter profit of $5.7 million, or $0.07 per share. Last year this time, it reported a harrowing net loss of $22.4 million, or $0.27 per share. Revenue surged 23.7% to $166.9 million, but that's an easy comparison to last year's malaise, when first-quarter revenue dropped 32%. Full Text

Proper Foot Care a Must for Patients with Peripheral Arterial Disease

 

People who have peripheral arterial disease (P.A.D.) - or clogged arteries of the legs --are likely to develop foot problems. When the blood vessels in the feet and legs are narrowed or blocked, not enough blood flows to these parts of the body. As a result, minor problems such as cuts, sores or blisters cannot heal quickly, or heal at all.

P.A.D. is very common in adults with diabetes and these patients have an even greater risk of foot problems. In addition to poor circulation, people with P.A.D. and diabetes also may develop nerve damage from high blood glucose (sugar) levels, causing loss of feeling in their feet. One may not feel a pebble inside their sock that is causing a sore or not feel a blister develop due to friction or poorly fitting shoes. These small foot injuries can cause sores or ulcers that can turn into serious infections. If not treated promptly, these problems may lead to losing a toe, foot or leg. In fact, P.A.D. is one of the leading causes of foot or leg amputations in the United States and perhaps around the world. Full Text

Diabetes Nerve Pain May Worsen at Night

People with diabetes-related nerve damage may experience worse pain in the evening hours, preliminary research suggests. If confirmed in larger studies, the findings suggest that people with diabetic peripheral neuropathy might need more pain medication late at night.

 

Peripheral neuropathy is a type of nerve damage associated with type 2 diabetes most often characterized by pain, tingling, and numbness in the hands and feet.

 

"It could be that patients need more [pain] medication at night or that if they are taking a once-a-day medication, it should be taken in the evening," says Brett Stacey, MD, medical director of the Comprehensive Pain Center at Oregon Health & Science University in Portland. But it's too early to make recommendations on the basis of this study, he tells WebMD. Full Text

Just months after opening its first U.S. store in Portand, Oregon's Pearl District, Li Ning Co. is preparing to unveil its expanded retail showroom, with street-level access and more merchandise, on Saturday. The Chinese athletic shoe and clothing maker's current location at 910 N.W. Hoyt St. has merged into adjoining space at 916 N.W. Hoyt, increasing its retail footprint from about 850 square feet to 2,200. The building also houses its U.S. corporate offices as well as a design center.

Pedorthic Pre-Certification Training Course July 26th - August 8th, 2010

In cooperation with the ABC and BOC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute.
