Comment: Medicare requested information on 13 of our patients. Medicare then reviewed the information and denied 12 of the claims. When I contacted them they stated the M.D. or D.O. did not have documentation in the medical records we submitted that substantiated the TSB.

Medicare instructed us to get all medical records prior to delivering any items. Medicare will not allow us to create a form that would include all the information they are expecting the doctors to document. We have lost a fourth of our business and our customers are outraged. Physicians are sending documentation that does not meet Medicare's policy and we are upsetting Physician's by requesting more info. We can't upset Physicians or they will stop sending their patients our way.

Medicare requires that we have medical records from the M.D. or D.O. that treats the patients' diabetes. I spoke with POE and Medicare and cannot get an explanation of what they classify a comprehensive plan. Per Medicare the medical records supplied must include:

1)The diagnosis marked on the TSB

2)A comprehensive plan of treatment

3)The need for shoes/inserts/modifications and the quantity

Medicare states we cannot use notes from another physician. Most M.D.'s and D.O.'s send their patients to a Podiatrist to treat their feet. We have sent all the documentation suggested by Medicare to the physician treating the patient's diabetes. There are other providers in our area who our patients have told us are not requiring this information. If Medicare is going to enforce this, then everyone dispensing items from the TSB should be required to follow these rules. Does anyone out there have any suggestions?

Question: Has anyone else had this happen yet?

We received four of the Prepayment Reviews from Medicare, we called the doctors and received the notes for the patients and sent them all back for review. Three went through fine and we received payment. The fourth is a lady from the Air Force Base Hospital in town; this one did not get approval because the base physicians do not follow Medicare guidelines and do not test their patients for neuropathy yearly, and there was no documentation for poor circulation or foot deformity that they circled on the CPF. Now we have to bill the patient, and this is a regular yearly customer, for the shoes and custom foot orthoses -- which means we have lost a patient because they won't be back next year, and I don't blame them. So, is my only option here to not accept patients from the base hospital just in case Medicare decides to do this again? Many of my patients are from this military hospital.

It sure is tough making a living in this profession. We did a cost analysis for custom molded diabetic foot orthoses made in our lab. Cost is about $70.00 a pair for materials, salaries, and time. This does not include fitting, adjustments and follow-up appointments. Aetna pays $32 a pair, the same amount they pay for A5512's. They don't cover our costs on shoes either. Who can afford to lose money for each Aetna patient? We can't and won't.

Is it worth it? I wonder.

Question: With the recently announced meeting between NCOPE, ABC, PFA, BOC and the Pedorthic Educators, it seems that "formal" pedorthic education (which we all know translates into "associates degree") is finally looming on the very near horizon. As one of the 30 people invited to this function, I heard from many of the organizational heads of state that they thought this was the way to go. So, what do you think? Is this a good idea? If so, are you willing to upgrade your skills to match (Anatomy/Physiology/Biology/Physics)? What is this going to do to the already slim supply of pedorthists in conjunction with all of the new patients entering the system through the new Healthcare setup? Can you afford to send your employee to school in another State for 2 years, and help pay their student loans off over the next 10 years? Good questions, and this is a great forum to let the organizations know what the men and women on the street think.

N.C. Man's Interest in Fashion Spawns Homemade Shoes

Kieran Ionescu's interest in fashion dates back to the presents his uncles used to send him for Christmas and birthdays. The presents tended to be side projects for the uncles' friends, such as Diesel jeans and T-shirts and hats by Shankbone. Regardless, the presents were advanced for the fashion scene at Durham's Riverside High School back when Ionescu was a student, and they left an impression. "That instilled a fascination with garments and fashion," Ionescu says.

A decade later, Ionescu, 26, is designing and making shoes by hand out of his West Durham home. His business is BlackArm Bespoke; "black arm" is a nod to the tattoo that wraps his right forearm from wrist to elbow, and bespoke is an antiquated word for custom made. Full Text

Swiss Military Unveils New High-tech Socks

The Swiss defence ministry on Friday unveiled new high-tech sweat-absorbing socks that were tested by dozens of soldiers roaming the countryside with a different sock on each foot. Sixty recruits equipped with a pair of boots and 10 socks each of three different types were sent off on a "merciless" five-day test mission, with orders to wear odd socks, the defence ministry said in a statement.

"The recruits were not informed beforehand about the socks they would wear, in order to obtain results that were as objective as possible," it added. Full Text

Frugality Among Consumers is Outliving Recession

Even as the economic recovery plods ahead, many American consumers are refusing to come along. They're not spending freely - and they have no plans to.

Many of them have steady income. They aren't saddled by high debts. They don't fear losing their jobs. Yet despite recent gains, they've lost so much household wealth that they're far more cautious about spending than before the recession.

Their behavior suggests that the Great Recession may have bred a new frugality that will endure well into the recovery. And because consumers fuel about 70 percent of the economy, their tightfisted habits means the rebound could stay unusually sluggish. That's the picture that emerges from an Associated Press survey of leading economists and interviews with more than two dozen ordinary Americans. The new AP Economy Survey asked 44 leading economists whether the recession created a "new frugality" among consumers that will outlive the recession. Two-thirds said yes. Full Text

April Retail Sales Seen Up, Clues Sought on Economy

Warm weather and a desire to refresh wardrobes should have helped retailers increase sales in April, showing consumers are willing to spend a little more in an improving economy. Retail chains ranging from Target Corp to J.C. Penney Co to Abercrombie & Fitch will report sales at stores open at least year -- an industry gauge known as same-store sales -- on Wednesday and Thursday. Analysts are expecting same-store sales to have risen 1.7 percent in April, compared with a drop of 2.7 percent last year, according to Thomson Reuters data. Full Text

Routine MRI Findings of the Asymptomatic Foot in Diabetic Patients with Unilateral Charcot Foot

Imaging studies of bones in patients with sensory deficits are scarce. Aim: to investigate bone MR images of the lower limb in diabetic patients with severe sensory polyneuropathy, and in control subjects without sensory deficits.

Methods: Routine T1 weighted and T2-fat-suppressed-STIR-sequences without contrast media were performed of the asymptomatic foot in 10 diabetic patients with polyneuropathy and unilateral inactive Charcot foot, and in10 matched and 10 younger, non-obese unmatched control subjects. Simultaneously, a Gadolinium containing phantom was also assessed for reference. Full Text

Certain Prostate Cancer Drugs May Cause Diabetes

The FDA will be taking a closer look at drugs to treat men diagnosed with prostate cancer because of a possible link of increased diabetes risk. The drugs in question work by reducing the male hormone testosterone, but may also raise risk of diabetes as well as heart disease in men taking them. Reducing testosterone has been shown to reduce the size of prostate cancers, thus the drugs are very effective at treating men diagnosed with the disease. Full Text

NPD Reports on the Global Running Shoe Market

According to The NPD Group, Inc., overall sales of running shoes are showing signs of softening in some key global markets, but sales of running shoes used by runners are posting modest gains globally.

The key markets where total sales of running shoes are softening are in the U.S. and in Europe. In the U.S. after three years of gains sales of running shoes fell slightly, by -0.9 percent in 2009. Sales also fell slightly in the European Big 5* countries, by -0.1 percent for the year. The international markets still posting growth are Canada where overall sales of running shoes were up 4.6 percent and in Japan sales were up 1.0 percent for 2009.

There is a bright spot of note and that is, in every geographic region NPD tracks the athletic footwear market sales of shoes 'used for running' outpaced the growth for overall running shoe sales. Full Text

Pedorthic Pre-Certification Training Course July 26th - August 8th, 2010

In cooperation with the ABC and BOC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute.
